You can change your farmer after starting a save: visit the Shrine of Illusions in the Wizard’s Tower basement, interact with it, and pay 500g. The usual way to unlock the basement is to reach at least four hearts with the Wizard.
How to change your farmer’s appearance
- Build your friendship with the Wizard. Reach at least four hearts with him to unlock the trapdoor to the basement in the Wizard’s house.
- Enter the basement. Go down through the trapdoor in the Wizard’s Tower.
- Use the Shrine of Illusions. Interact with the Shrine and offer 500g to open the character-editing options.
- Make your changes. Adjust the available character settings and confirm them before leaving.
You do not need to start a new save to use the Shrine. Its documented options include appearance, name, favorite thing, and gender. Stardew Valley Wiki: Shrine of Illusions

What the Shrine can and cannot change
The Shrine edits your farmer’s character settings; it is not a general outfit or wardrobe menu. For clothes, use tailoring instead. The Tailoring system replaced the old shirt and pants color sliders in version 1.4. Stardew Valley Wiki: Tailoring

The tailoring page describes using Emily’s sewing machine at her house. This is the route to take when you want to make clothing rather than change your farmer’s underlying appearance settings.

Alternate route: the Witch’s Hut portal
If the Witch’s Hut is accessible in your save, its red portal also leads to the Wizard’s basement. This is an alternate way to reach the Shrine; it does not change the Shrine’s 500g fee. The Shrine reference describes the portal route but does not establish every quest or platform prerequisite for opening the Hut.

- Version 1.6 removed pet-appearance changes from the Shrine, so do not use it expecting to edit a pet.
