They can both leave you tired, and a person can have both. Symptoms can suggest what to discuss with a clinician, but they cannot confirm or rule out either condition.
How sleep apnea and insomnia differ
The central difference is what is disrupting sleep. With sleep apnea, breathing repeatedly stops or becomes shallow during sleep. With insomnia, the difficulty is sleeping: falling asleep, remaining asleep, or waking too early, often with too little or unrefreshing rest.

| Pattern | More suggestive of sleep apnea | More suggestive of insomnia |
|---|---|---|
| Main nighttime clue | A bed partner may notice pauses in breathing, loud snoring, or gasping. The sleeper may not remember these events. | You may lie awake for a long time, wake repeatedly, spend much of the night awake, or wake earlier than intended. |
| Daytime effects | Sleepiness, fatigue, trouble focusing, or slower reactions can occur. | Low energy, sleepiness, irritability, anxiety, or difficulty concentrating can occur. |
| How it is evaluated | A clinician assesses symptoms and may arrange a sleep study to identify whether apnea is present and, if so, its type and severity. | A clinician reviews sleep and medical history and may examine you for other causes; a sleep study may be considered in some cases. |
| Typical treatment direction | Options can include positive airway pressure, an oral device, lifestyle changes, or other clinician-selected treatments. | Options can include behavioral treatment such as cognitive behavioral therapy, lifestyle changes for short-term insomnia, and sometimes medicine. |
These are patterns to discuss, not a home diagnostic test. Sleep apnea can also be associated with insomnia-like awakenings, fatigue, headache, dry mouth, frequent nighttime urination, or sexual dysfunction, and symptoms vary between people. Loud snoring alone does not establish apnea, and trouble sleeping does not exclude it. NHLBI’s symptom overview describes the range of possible signs.

Both conditions can fragment sleep and cause daytime fatigue or sleepiness. A person with insomnia may report repeated awakenings, but those awakenings do not by themselves reveal whether breathing interruptions are also occurring. Conversely, someone with apnea may mainly notice tiredness rather than witnessed pauses. Insomnia can coexist with obstructive sleep apnea, and one sleep complaint can obscure another.
#1 Best Overall
- As a home-use personal sleep monitor with breathing airflow recording capability, the device gives you an affordable and efficient way to measure your sleep condition. The advanced AI algorithm will have in-depth analysis on the collected data and generate a professional respiratory report.
- Built-in three sensors in a probe to measure, record and analyze airflow to accurately monitor breathing. It starts to work right after you put it on with real-time monitoring. The appearance of the smooth waveform implies a normal breath, while any anomaly of the waveform indicates that a respiratory event is occurring and being analyzed.
- Check the sleep information through the app to analyze your nighttime breathing and see the way it changes after waking up in the morning. Understand all your sleep details in a clear way to monitor the occurrence of respiratory events and improve overall sleep health.
- The mask is made of food-grade silicone and is designed to achieve a perfect facial fit using three sizes of high elastic straps for all-night comfort. The small size and lightweight (10g) make it easier to wear and carry. Convenient to use at home or while traveling.
- IMPORTANT NOTES Before Purchasing: 1. Can not be used with C-P-A-P mask at the same time. 2. Designed to monitor nasal breathing, not recommended for mouth breather. 3. Not for SpO2 / Pulse Rate recording. 4. Monitor your sleep breathing data (flow, AHI, snoring, etc) only, not helping you to breathe. 5. Can not replace doctor's diagnosis.
A fitness watch or sleep app may help you notice patterns, but it cannot diagnose either condition. Diagnosis depends on clinical assessment; suspected sleep apnea is evaluated with sleep testing. A sleep diary can be useful for recording bedtime, awakenings, how rested you feel, and daytime sleepiness to discuss with a provider, but it is not a diagnostic tool. NHLBI explains apnea evaluation.
When to seek medical advice
Arrange a conversation with a healthcare provider if someone observes pauses in your breathing, you repeatedly snore loudly or gasp during sleep, your poor sleep persists, or sleepiness or fatigue is interfering with daily functioning. A provider can assess possible causes and decide whether referral to a sleep specialist or testing is appropriate.
Take drowsy driving seriously. If you feel sleepy behind the wheel, stop driving and take a break; drowsiness can contribute to a crash. MedlinePlus’ insomnia guidance discusses daytime effects and driving risk.

What evaluation may involve
If sleep apnea is suspected
A clinician reviews symptoms and risk factors and may refer you to a sleep specialist or testing center. A sleep study helps determine whether apnea is present and its type and severity. The appropriate test depends on the person’s circumstances.
For adults with signs and symptoms indicating increased risk of moderate-to-severe obstructive sleep apnea who are otherwise uncomplicated, the American Academy of Sleep Medicine recommends either polysomnography or technically adequate home sleep apnea testing. Its guideline recommends polysomnography rather than home testing in circumstances including significant cardiorespiratory disease, possible respiratory muscle weakness, awake or suspected sleep-related hypoventilation, chronic opioid use, prior stroke, or severe insomnia. The clinician should select the test; these recommendations are not a self-selection checklist. Read the AASM adult diagnostic guideline.
If insomnia is suspected
Assessment commonly includes a medical and sleep history and a physical examination to look for other causes. A sleep study is not necessarily needed for every person with insomnia, but may be recommended in some cases. MedlinePlus outlines insomnia evaluation.

Treatment depends on the diagnosis
Sleep apnea
For diagnosed apnea, treatment may include positive airway pressure or an oral device, healthy lifestyle changes, and other options. Surgery may be considered when other treatments have not worked. The appropriate approach depends on the individual and should be discussed with a clinician; purchasing a PAP device or oral appliance is not a substitute for evaluation. NHLBI describes sleep apnea treatment options.

Insomnia
For short-term insomnia, lifestyle changes may help. Cognitive behavioral therapy is a treatment option for chronic insomnia; medicine may also be considered in context. If another condition is contributing to insomnia, addressing it may be part of care. A clinician can help choose an approach rather than assuming a sleeping medicine is the right answer. MedlinePlus summarizes insomnia treatment.
