Colorlib’s indexed V14 page lists the license as CC BY 3.0. That is the license label displayed by the publisher, not an independent inspection of the downloaded files or the license text. Before redistributing the snippet or making substantial reuse, check the terms supplied with the download and confirm they cover your intended use. The template page identifies Colorlib as publisher and Rok Krivec as author: Signup Form V14 on Colorlib.

The listing’s date labels are not fully reconciled: it shows a post date of March 8, 2021, separately labels a 2025 release, and includes 2026 in the displayed title.
