There is no universal pre-run meal, hydration volume or toilet-access plan that works for everyone; use your own symptom pattern to guide choices, and seek professional advice before starting a restrictive diet.
What running may do to IBS symptoms
Running does not affect everyone with IBS in the same way. In a 2023 study, runners with IBS reported more gastrointestinal symptom severity than control runners both at rest and during running. Within the IBS group, however, the median total symptom-severity score was 150 at rest and 118 during or after running; the authors reported that difference as statistically significant. They concluded that running, particularly at moderate intensity, could benefit symptoms, but this is not a guarantee that a run will relieve yours. The study found a beneficial effect for moderate or intensive training, but not very intensive sessions. Read the 2023 study abstract.

The broader evidence is cautious, too: the American College of Gastroenterology describes exercise as a weak recommendation for overall IBS symptom improvement, based on very low-quality evidence. Treat exercise as one possible part of managing IBS, not as a substitute for care or a reliable way to stop symptoms. ACG monograph information.

Keep a regular, familiar routine
NICE advises people with IBS to eat regular meals, take time to eat, avoid skipping meals and avoid long gaps between meals. This is general IBS guidance, not a prescribed runner’s schedule: the available guidance does not establish a universal interval between eating and running or a single best pre-run meal. If you already know a meal pattern that sits well, avoid making a last-minute change before a run.

A simple log can help you spot your own patterns. Note meal timing and foods, run duration and intensity, symptoms, and relevant conditions such as hot weather. Look for repeated associations rather than treating one uncomfortable run as proof that a food is a trigger.
Do not copy someone else’s exclusions
A 2021 study of runners with IBS or inflammatory bowel disease (IBD) found that participants reported avoiding some foods before races while also selecting foods the authors considered potentially aggravating. Reported avoidances included milk products (53%), legumes (37%) and meat (31%); reported selections included gluten-containing grains (40%) and high-FODMAP fruits (38%). These figures describe that study’s combined IBS/IBD sample, not all runners with IBS, and they do not establish a universal trigger list. The findings are a reason to individualize choices rather than copy another athlete’s restrictions. Read the 2021 dietary-pattern study record.

Get support before trying restrictive diets
If general lifestyle and dietary steps are not enough, NICE says further dietary management—including avoiding individual foods or following a low-FODMAP diet—should be guided by a health professional with dietary expertise. The American Gastroenterological Association (AGA) also supports individualized diet advice and dietitian help when people need assistance putting changes into practice. A registered dietitian can help assess whether a change is suitable and how to keep the overall diet balanced. NICE IBS recommendations; AGA clinical guidance.

NICE recommends at least 8 cups of fluid a day, particularly water or non-caffeinated drinks, as general IBS advice. NICE’s quality-standard material gives approximately 2,000 ml as an equivalent. For prolonged exercise, sports nutrition guidance supports fluid strategies tailored to the person and conditions, whether planned or based on drinking to thirst. Both underhydration and overhydration matter; the sources do not establish one volume suitable for every runner, route or day. Consider run length, weather, your sweat response and personal tolerance when deciding whether to carry water, use a refill point or take a shorter loop. 2025 joint sports nutrition position statement.
| Fluid approach | When it may suit | What to consider |
|---|---|---|
| Carry water | When you prefer access to water along the route. | A belt with a soft flask is one optional way to carry it; it has not been shown to prevent IBS symptoms. |
| Use a refill point | When a route has a reliable place to refill and you would rather carry less. | Check access and opening availability for the time you plan to run. |
| Run a shorter loop | When you want to stay nearer home, water or other facilities. | It offers flexibility to finish early; it is not established as medically superior. |
Choose a route that gives you options
Route planning is a practical accommodation, not a validated IBS-specific treatment. Choose based on what makes you feel more comfortable and able to adjust if symptoms start.

When to seek medical advice
This guidance is general, not an individual diagnosis or treatment plan. IBS symptoms can overlap with other conditions. Do not assume new, severe or concerning symptoms can be handled simply by changing a meal or route; contact a qualified health professional for advice. If you are considering medication timing or use during a run, discuss it with a clinician or pharmacist rather than relying on a blanket protocol.
