In a 2026 U.S. evaluation, the CLAiR system’s image-based risk classification was compared with a standard 10-year cardiovascular risk estimate—not with blood-pressure and cholesterol tests as stand-alone alternatives. Treat retinal imaging as an emerging screening adjunct, with any concerning result followed up through a clinician.

In the CLAiR study, blood pressure and cholesterol were among the clinical factors used with age, sex, and smoking status to calculate the comparison risk estimate. That is why the study should not be read as showing that an eye photograph can substitute for either measurement. The American College of Cardiology’s 2026 report describes the evaluation; current professional and patient resources for lipid management are available through the 2026 AHA/ACC dyslipidemia guideline hub.

What the 2026 CLAiR evaluation found
The prospective U.S. evaluation included 874 adults ages 40–75 at 10 eye-care and primary-care sites. Participants had no known atherosclerosis and were not taking lipid-lowering medication. About 26% had a standard-estimator 10-year ASCVD risk of at least 7.5%.
Against the study’s standard-estimator classification, CLAiR reported 91.1% sensitivity and 86.2% specificity. These numbers describe how often the system identified positive and negative classifications relative to that calculated risk category. They do not mean the system prevents heart attacks or strokes, nor do they show that it can safely guide treatment by itself. The study was funded by Toku, the system’s developer; the ACC report identifies study lead author Michael V. McConnell, MD, as Toku’s chief health officer.

The report estimates that retinal image capture took about five minutes and an algorithm result about 30 seconds; those are study workflow estimates, not a guarantee for every clinic. The AI could use 94% of the acquired images in the study. McConnell said the approach “would not replace the standard cardiovascular risk evaluation,” and described it as a potential way to raise awareness among people who have not had a thorough evaluation.
Why promising agreement is not proof of clinical benefit
The comparator was a calculated 10-year ASCVD risk category based on age, sex, smoking status, blood pressure, and cholesterol. Agreement with that estimate is not the same as diagnosing heart disease, predicting an individual event with certainty, or showing that screening improves health outcomes. The evaluation did not establish fewer heart attacks, strokes, or deaths, or that retinal screening can replace clinician judgment and standard measurements.

The results also should not be assumed to apply to everyone: the study excluded people with known atherosclerosis and those taking lipid-lowering medicine. The ACC report says CLAiR received FDA Breakthrough Device designation and that the U.S. evaluation results would support an FDA submission; that designation does not establish FDA marketing authorization.

The 2024 NHLBI workshop roadmap describes research priorities rather than proven screening benefits. It calls for standardized image capture, longitudinal studies in diverse populations, improved measurement of retinal biomarkers, and stronger integration with clinical data. Those needs matter because proposed retinal biomarkers span several conditions—including coronary artery disease, heart failure, stroke, hypertension, and vascular dementia—and are still being studied.

What to do with an eye-based risk result
A result suggesting elevated risk should prompt a clinical conversation, not self-diagnosis or a treatment change based on the image alone. A clinician can assess blood pressure, cholesterol, other relevant factors, and the person’s overall history, then discuss appropriate prevention. McConnell has also emphasized the need for clear pathways from an elevated eye-exam result to a clinician and guideline-based preventive care; the ACC report identifies those referral pathways as unfinished work.
CLAiR was not designed for pregnant people or people with advanced eye disease that may affect retinal vessels. In the U.S., retinal imaging is not covered by every vision plan and may involve an extra charge. Ask the eye-care office what the test is intended to show, how results will be handled, and whether there is a cost before proceeding.

How to read the evidence in practice
- If you have not had blood pressure or cholesterol checked: an eye image does not supply those measurements. Ask a clinician which standard checks are appropriate for you.
- If a retinal screen flags possible risk: use it as a reason to seek clinical assessment, rather than as a diagnosis or a basis for starting or stopping medication.
- If you already have cardiovascular disease or take lipid-lowering medication: the reported study cohort did not include these groups as described, so its performance figures should not be assumed to apply to you.
The retrieved adult blood-pressure guidance is the 2017 ACC/AHA guideline; it is useful context for the role of measured pressure, but should not be described as the latest blood-pressure guideline.
