That score is useful only if the metric represents the decision the deployed system must make. A prompt can improve accuracy while getting worse at ranking rare positive cases; the optimizer has not failed to follow instructions—the objective may be pointing it at the wrong hill.
What prompt optimization actually optimizes
“Prompt optimization is search,” writes Aamer Mihaysi in his DEV Community article published 2026-10-01. In a typical loop, an optimizer proposes prompts, an evaluation harness scores them, and the search process keeps or revises candidates based on those scores. The optimizer does not infer what a hospital, clinician, or product team ultimately needs. It follows the feedback it receives.

That distinction matters because a metric is a proxy for a deployment decision, not the decision itself. A prompt with a higher score on the harness’s chosen metric is better by that metric. It is not automatically safer, more useful, or more suitable for real-world use.

Accuracy counts the share of predictions that are correct. When positives are rare, a system can achieve a high accuracy score by predicting the majority class every time, even though it misses every positive case.
#1 Best Overall
Mihaysi illustrates the issue with a hypothetical dataset in which 4% of examples are positive: an always-negative predictor scores 0.96 accuracy while missing the rare finding. That is an author-provided example, not an independently verified estimate of disease prevalence. The authors of the preprint Ranking-Aware Prompt Optimization for Multimodal Clinical Diagnosis make the related point that “A constant-majority predictor can score above 90% accuracy while being clinically useless.” That sentence is from their arXiv preprint, version 1, submitted 2026-09-30—not a finding of independent clinical validation.
If the evaluation harness reduces each result to correct or incorrect, it also discards the model’s score for that case. Those scores can contain information about how strongly the model ranks one case above another. Once reduced to a boolean, that ranking signal cannot be recovered from the correctness values alone.

What Ranking-PE changes
The arXiv preprint introduces pair-level Pareto prompt evolution, or Ranking-PE, for optimizing multimodal large language models on clinical diagnosis tasks. Instead of using one evaluation-matrix row per instance, it uses rows for positive-negative pairs. A cell records whether a candidate prompt gives the positive case a higher score than its paired negative case.

For a prompt, the average of those pairwise comparisons corresponds to empirical AUROC by the Wilcoxon–Mann–Whitney identity: it measures how often a randomly selected positive case is scored above a randomly selected negative case. The authors apply the pair-based representation to the score matrix used for Pareto dominance, the per-example feedback sent to the reflection model, and final candidate selection. Their abstract says the change uses no additional model calls and no surrogate loss.

What the reported gains mean
Across three diseases on MIMIC, the preprint’s authors report that Ranking-PE improved AUROC over their accuracy-based recipe by 5.8 percentage points on fine-tuned Qwen3-VL-8B and 16.2 percentage points on MedGemma-4B. These are experimental results reported in the authors’ arXiv abstract in 2026. They are not independent replications, evidence of clinical deployment, or proof that the method will produce comparable gains on other models, datasets, or tasks.
Which metric should a prompt optimizer target?
Choose the metric from the action the system is meant to support. A ranked review queue and a threshold-based decision are different jobs, and they need not favor the same prompt.
Rank #4
| Deployment use | What to evaluate | What the metric tells you |
|---|---|---|
| Order cases for human review | Ranking performance, such as AUROC | How well scores tend to place positive cases above negative ones across possible thresholds |
| Trigger an action at a chosen threshold | A threshold-specific measure, such as precision at the operating point; consider recall as well when missed positives matter | How the system performs at the decision rule actually used |
| Use scores as probabilities or confidence estimates | Calibration at the relevant operating range, alongside discrimination measures | Whether score values correspond to observed outcome frequencies, rather than merely ordering cases |
AUROC describes discrimination across thresholds; it does not establish calibration or tell you whether precision is acceptable at the threshold used in deployment. Likewise, strong precision at one operating point does not, by itself, describe ranking quality across thresholds. Match evaluation to the workflow, and report complementary measures when the workflow depends on more than one property.
Limitations of pairwise prompt evaluation
Mihaysi identifies practical caveats for pair-based evaluation. The number of positive-negative pairs grows with the counts of both classes, so the matrix can become large. Sampling pairs can reduce that burden but may introduce variance. Ties and discrete model scores can thin the ranking signal because fewer pairs show a strict ordering. Mihaysi says he has not tested pair-sampling behavior at a scale where its variance becomes problematic; that is an author-stated limitation, not a measured threshold for when sampling becomes unreliable.
There is also a modeling prerequisite in the preprint: its authors state that a medical-grade visual backbone is necessary in their experiments. Prompt search cannot replace the underlying model capability. Improving the search objective is not the same as making an unsuitable backbone clinically capable.
Quick Recap
How to make an evaluation harness harder to fool
- Define the deployment action. Decide whether the output orders cases, triggers a threshold-based action, or communicates calibrated risk.
- Keep continuous scores. Preserve the model’s raw scores during evaluation instead of storing only correct/incorrect outcomes.
- Optimize for the intended use. Use ranking metrics for ranked review workflows; use threshold-specific measures at the actual operating point for threshold decisions.
- Report complementary measures. Pair accuracy with ranking and threshold-specific results where relevant, and assess calibration separately if the score is meant to represent risk.
- Keep claims within the evidence. Treat the preprint’s MIMIC results as reported experimental findings, not as independent validation or evidence that a clinical system is ready to deploy.
