A gateway can give a healthtech application one API key and a common routing layer, but it does not make model providers’ token prices, gateway charges, endpoint locations, or data terms the same. To compare moderation costs fairly, price the exact models and routes against representative, de-identified requests, then add gateway and operating costs separately. There is no defensible universal “cheapest” choice without a defined workload—and price alone does not establish moderation quality or contractual suitability.
What “one API key across model providers” actually unifies
A model gateway can centralize application credentials and provide a common interface for routing requests to different model providers. Depending on the product, it may also offer virtual keys, usage budgets, spend tracking, rate limits, fallbacks, or audit controls. Those capabilities simplify integration and administration; they do not erase the underlying provider bill or make every model interchangeable.
As an Amazon Associate I earn from qualifying purchases.
Think of a routed request as having distinct cost and governance layers:
- Model inference: the selected provider charges according to its rate card, model version, token counts, and any applicable cache or feature rates.
- Gateway: the gateway may charge a platform fee, subscription or enterprise license, or storage fee. With a self-hosted gateway, the software can be free while infrastructure and operations still cost money.
- Endpoint and operations: regional routing, retries, fallback calls, and production infrastructure can change the total.
- Data and contract terms: the gateway key does not determine provider retention, processing location, permitted use, or whether a particular arrangement meets your contractual requirements.
Keep these layers distinct in both estimates and procurement review. A single integration credential is not a single rate card, invoice, or data-processing agreement.

Build a real token-cost comparison for healthtech moderation
Compare the traffic your product will actually send, not a generic “moderation request.” The same number of requests can produce different bills when prompts, clinical context, model versions, output limits, cache behavior, or fallback rates differ.
1. Define the workload and candidates
Choose representative, de-identified moderation requests and the exact model IDs and versions you are considering. Include the relevant language mix, context length, expected response format, and realistic production volume. Do not send sensitive health data to a service merely to create a price sample; use an approved, de-identified set and follow your organization’s data-handling rules.
2. Measure tokens and request behavior
For each candidate route, record input tokens, output tokens, cached tokens where applicable, and retries or fallback calls. Measure at realistic request volume. A route that invokes a second model after a failure or policy check may incur more than one inference charge for a single application request.
3. Apply each provider’s rate card
Calculate input and output charges separately using the selected model’s current provider rates. Add cache and feature charges where the model’s pricing lists them. Check endpoint location too: the listed rates may not be the rates for the required processing region.

A useful structure for the estimate is:
Estimated total = provider input charges + provider output charges + cache and feature charges + gateway fees + self-hosting or license costs + regional adjustments + expected retry/fallback charges.
This is a comparison framework, not a quoted invoice. Use the provider’s and gateway’s current billing rules to determine how each fee is calculated; do not assume a gateway percentage applies to the same base or usage measure as another gateway’s fee.
4. Add gateway and operating costs
Include percentage-based platform fees or fixed license costs, as applicable. For self-hosting, include the infrastructure and staff effort needed to operate the service; a $0 software price is not a $0 deployed-service cost. Keep provider inference estimates separate from gateway charges so it is clear which layer drives a difference.

Which gateway charges and pricing details matter?
Published pricing illustrates why “one key” does not imply one bill. The figures below are vendor-published page figures accessed in 2026; rates and terms can change. They are not a complete rate card or a workload-specific monthly estimate.
| Option | Published gateway or billing detail | What to include in a comparison |
|---|---|---|
| LiteLLM | The pricing page lists its self-hosted open-source gateway at $0. Enterprise pricing is based on annual request capacity, deployment architecture, and support needs. | For self-hosting, add infrastructure and operations rather than treating the software price as the full cost. For Enterprise, obtain a quote for the relevant capacity and deployment. |
| OpenRouter | The pricing page lists a 5.5% standard platform fee and an 8% business platform fee. In-region routing is shown for Business and Enterprise. | Verify the current fee and its billing basis, then add it to provider inference charges. If in-region routing is required, confirm plan eligibility and the specific route. |
| LLM Gateway using customer-owned provider keys | Its documentation says customer-owned provider keys route directly at standard provider rates without gateway markup, with an optional data-retention storage charge. This is the vendor’s claim. | Check any storage charge and independently assess the gateway’s data handling and contractual fit; a no-markup claim does not establish healthcare suitability. |
| OpenAI access to external models | OpenAI describes third-party model access through OpenRouter and tiered monthly spend limits. These account and spend-limit terms are subject to change. | Check current availability and account limits before relying on this route. OpenAI also says external-model calls are subject to different terms and weaker safety guarantees than calls to OpenAI models. |
These options are not identical commercial arrangements. A percentage gateway fee, a self-hosted deployment, and a negotiated enterprise license need different inputs in the estimate. Ask for the billable basis and any minimums or included usage rather than inferring them from the existence of a common API.

Regional processing can carry a premium
OpenAI’s current pricing documentation describes a 10% regional-processing uplift for eligible models released on or after March 5, 2026. Anthropic’s pricing documentation lists a 10% regional and multi-region endpoint premium for Claude 4.5 and later models. These are distinct vendor-specific terms, not a universal regional surcharge. Confirm whether the exact model, release date, endpoint, and required geography qualify before applying either figure.
Cache and feature rates are not always the standard input rate
Anthropic lists model and cache rates, while OpenAI’s documentation separates input and output billing. If your prompt design or request pattern uses caching or other separately priced features, count those units under the applicable rate instead of treating all input tokens alike. The exact cost depends on the model and features used.

Marketplace invoices may hide model-level detail
Anthropic’s Claude Platform on AWS bills token usage through Claude Consumption Units (CCUs) at $0.01 per CCU, with hourly metering and monthly marketplace invoices. Its documentation describes a single CCU line item to AWS Marketplace. If your team needs per-model cost attribution, confirm what usage detail is available in the platform’s reporting before assuming the marketplace invoice will provide it.
Health data requires a separate eligibility and privacy review
A gateway key is not proof that a provider or route is appropriate for sensitive health data. OpenAI warns that calls to external models pass data to third parties and are subject to different terms and weaker safety guarantees than calls to OpenAI models. More generally, a gateway’s privacy controls do not establish the terms of the underlying model provider or endpoint.
Before routing sensitive data, verify the specific gateway, provider, endpoint geography, retention settings, and applicable contracts for the exact deployment. The published pricing and product details summarized here do not establish a complete business-associate agreement or eligibility picture for any gateway-plus-provider combination, and no stack should be treated as automatically HIPAA-compliant on the strength of having one API key.
A practical comparison worksheet
- Workload: request volume, language mix, prompt and context patterns, output size, and expected retries or fallbacks.
- Model: exact provider, model ID/version, moderation task, and independently validated quality requirements.
- Token charges: input, output, cached tokens, and separately priced features, using current provider rates.
- Gateway: platform fee or license, the fee’s billing basis, any storage charges, and self-hosting infrastructure and operations.
- Location: required processing geography, supported endpoint, and any regional or multi-region premium.
- Governance: data flow, retention and deletion settings, contractual commitments, eligibility, and access controls.
- Billing visibility: whether reporting distinguishes provider, model, route, and environment well enough for your cost allocation needs.
Run the same workload assumptions through each candidate and retain a separate line for every cost layer. Recheck vendor pricing, routing availability, and terms immediately before committing, because the cited fee and rate-card pages are current-page figures rather than fixed guarantees.
