Enforce request and multipart limits first; validate and decode the image before processing it; normalize it within resource limits; then apply your moderation policy to both the image and its caption before publication.
What should the upload pipeline do, and in what order?
Do not publish an upload just because the request succeeded or a filename ends in .jpg. Use a staged pipeline that limits work early, checks the bytes rather than trusting request metadata, and only makes content visible after every required gate has passed.

- Constrain ingress. Set a request-body ceiling at the proxy or server boundary, then configure multipart limits for file size, number of files, text fields, and field nesting. These are resource and denial-of-service controls, not just form validation. Express’s Multer guidance explicitly notes that limits can help protect a site against denial-of-service attacks.
- Validate the upload. Treat the filename and declared content type as untrusted. Allow only formats the product needs, compare the declared type and extension with the bytes’ actual format, and decode the image with a maintained image library. Reject invalid files and images above your configured dimension or pixel ceiling.
- Normalize within bounds. Resize and re-encode accepted images to controlled output formats and dimensions. Bound both pixel work and output bytes; a small compressed file can still decode into an extremely large image.
- Moderate both modalities. Submit the caption as text and the image as an image to the selected moderation service, or use separate services where needed. Apply your own policy to the results; a flag or score alone is not a universal publication decision.
- Publish, reject, or queue. Make the normalized asset and caption public only after all required checks pass. Route uncertain or policy-sensitive cases to review, and keep rejected content inaccessible.
Keep temporary files or object-store objects private while these steps run. Use controlled client errors for size, format, and validation failures; log useful internal diagnostics without returning stack traces or implementation details.

Which upload checks matter before image processing?
Set limits at more than one boundary
A reverse proxy or server-level body cap can stop oversized requests before application code buffers them. Multipart limits then constrain individual files, file count, text fields, and nesting. A request-level ceiling and multipart limits address different ways an attacker or malfunctioning client can consume resources, so one should not be treated as a replacement for the other.

Choose limits according to the product’s intended use and the capacity available for decoding, moderation, storage, and review. There is no universal pixel or output-byte threshold established here. Apply a request timeout and concurrency controls as appropriate for your infrastructure, and return a predictable client error when a limit is exceeded.
Verify the actual file
Neither a filename extension nor a MIME header proves what the uploaded bytes contain. Compare expected type with file signatures, decode the content using a maintained image library, and reject malformed or unsupported input. Check decoded dimensions and pixel count before allowing expensive transformations. OWASP’s application-security guidance calls for matching file content to its expected type, image rewriting or specialized validation, quotas, and rejecting images above the maximum pixel size.
Keep the original client filename only if there is a concrete product need for it; never use it as a trusted storage path. Generate your own storage key and preserve the original name, if needed, only as escaped display metadata.

How should image normalization and compression work?
Normalize the uploaded asset
Use an image processor such as Sharp to decode, resize, and re-encode accepted images into the formats and dimensions your application supports. Sharp documents Buffer, stream, and filesystem input/output, and conversion to web-oriented JPEG, PNG, WebP, GIF, and AVIF. Select a narrow output policy rather than blindly preserving every incoming format. Set limits around decoded pixels, processing time or concurrency, and output bytes so accepted files cannot consume unbounded resources.
Normalization creates a controlled asset for storage and downstream use; it is not a moderation decision. Do not expose an asset until the moderation gates and any required review have completed.
Do not confuse image re-encoding with response compression
Sharp changes the image asset itself: it can resize and encode pixels into a chosen format. Express’s compression middleware instead compresses eligible HTTP response bodies, selecting compressible responses by content type by default and supporting deflate, gzip, and Brotli. It does not replace image resizing or format conversion.

Already-compressed image formats may gain little from another transport-compression pass, while that pass still consumes CPU. Exclude binary or already-compressed responses where appropriate, then measure CPU, latency, and payload size on your own traffic before enabling a broad policy.

How can one moderation request cover a caption and an image?
OpenAI documents omni-moderation-latest for text and image inputs. Its TypeScript reference shows a moderation request containing a text item and an image_url item; the response includes a flagged value, categories, and category scores. The documented image limit is 20 MB. Keep your normalized image below the applicable limit, and verify current provider documentation and request requirements when implementing because service behavior and limits can change.
A combined request can simplify the point at which the caption and image are evaluated together, but the returned flag is not a complete community policy. Decide which categories block publication, which trigger human review, and which are allowed. Make those decisions explicit in application code and record the policy outcome separately from the provider response.

}In a real handler, pass the actual image representation supported by the selected API, handle provider and storage errors explicitly, and ensure the default state is private or unpublished. A timeout or unavailable moderation service should not silently become an allow decision unless your documented risk policy deliberately says otherwise.
When is image-only moderation a better fit?
AWS Rekognition’s DetectModerationLabels is a documented still-image alternative. It returns hierarchical moderation labels with confidence values and supports a synchronous workflow using JPEG or PNG bytes or an S3 object. It does not moderate the caption, so this architecture still needs a separately chosen text-moderation route.

Those documented capabilities are not enough to establish comparative accuracy, latency, price, retention terms, or regional suitability. Check the current service terms and test representative content from your own community before choosing a provider or setting thresholds.
How should scores, flags, and review decisions be handled?
Provider labels and scores are inputs to a product decision, not policy by themselves. For a label-based system, raising the confidence threshold may reduce some false positives while also missing more content; lowering it can increase both detection and unnecessary review. Calibrate thresholds against your rules, representative examples, and the human-review capacity you can actually support.
- Allow: publish only when every required check passes and your policy permits the result.
- Reject: keep content private and return a controlled response when it violates a clear rule.
- Review: preserve the upload privately, record the reason for escalation, and let an authorized reviewer decide before publication.
Store enough information to investigate decisions—such as the policy outcome, relevant category results, and a request or content identifier—while applying your own access, retention, and privacy controls. Do not treat provider scores as interchangeable across categories or services.

Keep known or suspected child sexual abuse material (CSAM) out of the ordinary moderation path. OpenAI explicitly says not to send known or suspected CSAM to its Moderation API and states that the API is not designed for CSAM detection or handling. A production service needs dedicated child-safety safeguards and an incident-response path designed for its legal and operational obligations; ordinary image moderation is not a substitute.

- Oversized requests, too many files, excess text fields, and excessive multipart nesting are rejected with controlled errors before expensive processing.
