Fear in anticipation of sound can also involve anxiety. These patterns can overlap, and none can be diagnosed from one symptom alone.
How the sound reaction differs
The most useful distinctions are what triggers the reaction, what the reaction feels like, and whether loudness is the main factor. The table describes patterns, not a diagnostic test.

| Pattern | What tends to stand out | Important qualification |
|---|---|---|
| Misophonia | A particular sound or sound-associated cue prompts a strong emotional, physical, or behavioral response, even when it is not loud. Repetitive sounds, often made by other people, are common examples. | The Misophonia Consensus Committee’s 2022 Delphi paper offers a working definition, not a validated diagnostic standard. |
| Hyperacusis | Physical discomfort or pain is tied more to sound intensity than to the sound’s source; levels most people tolerate may feel painful or uncomfortable. | Calling an experience “sound sensitivity” does not by itself establish hyperacusis. The distinction is described in Henry and colleagues’ 2022 review in the American Journal of Audiology. |
| Noise sensitivity or broad sensory overwhelm | A noisy environment, which may include many sounds together, feels overwhelming or unusually irritating. | “Sensory overload” is often a descriptive phrase; the cited 2022 review does not establish it as a specific diagnosis. |
| Anticipatory fear or anxiety around sound | Fear or avoidance centers on the expectation that a sound will occur, cause discomfort, or worsen another condition. | Anxiety may coexist with other sound-tolerance conditions; it is not an automatic explanation for every sound reaction. |
Questions that can help you describe your pattern
These questions can help organize what you have noticed before discussing it with a clinician. They are not a self-test.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minute- Is it a particular cue or many sounds? A reaction to one repeated pattern, such as chewing, differs from a response to almost any sound once it becomes loud enough.
- What is the main response? Note whether it is anger or disgust, physical pain, broad overwhelm, or fear about what may happen.
- Does volume change the reaction? Record whether an ordinary-volume sound can trigger it, or whether distress rises mainly as sounds get louder.
- What does it affect? Consider whether it interferes with sleep, conversations, school, work, or relationships, and whether you avoid particular settings or activities.
A single person may fit more than one pattern. Misophonia can occur alongside psychiatric, neurological, auditory, or sensory-processing conditions, so a sound trigger alone cannot identify its cause.

What is established—and what remains uncertain
The Misophonia Consensus Committee’s 2022 peer-reviewed Delphi study defined misophonia as decreased tolerance to specific sounds or associated stimuli that tend to provoke strong negative emotional, physiological, or behavioral responses. In that working definition, the response is not explained simply by loudness; the sound’s pattern or meaning to the individual matters. The committee also identified differential diagnosis, including distinction from hyperacusis, as an important issue.
A working definition is not the same as settled diagnostic criteria or a validated assessment. The Misophonia Research Fund’s overview, accessed October 7, 2026, says that causes are not presently known and that diagnosis and evidence-based treatment remain active research needs. It also says misophonia is not currently defined in the DSM or ICD. These limits mean that a checklist—or a guess that anxiety, trauma, autism, or a particular brain mechanism explains an individual’s reaction—cannot establish a diagnosis.

When to seek an assessment and what to bring
If sound-related distress persists or interferes with daily life, consider discussing it with a qualified clinician. An audiologist can help assess hearing-related concerns and sound tolerance. A behavioral-health professional may be useful when anxiety, avoidance, mood symptoms, or other concerns are part of the picture. Duke Health describes individualized, team-based care that can involve audiologists and other specialists; it is one care model, not a universal referral rule.
Bring a short account of the pattern: the sounds involved, whether the trigger is specific or mainly loudness, your main physical or emotional response, when it began, and how it affects daily activities. The 2022 review by Henry and colleagues recommends aligning terminology among the patient and clinicians during assessment, which can help prevent different people from using “sensitivity” to mean different experiences.

Treatment and coping: avoid one-size-fits-all promises
Approaches described in clinical sources include counseling, tailored cognitive behavioral therapy (CBT), and sound-related strategies. Henry and colleagues wrote in their 2022 American Journal of Audiology review, “Treatment generally involves gradual and systematic sound desensitization and counseling.” They also describe inconsistencies in terminology, assessment, and treatment methods. Duke Health describes tailored CBT and audiology input, including sound generators used to neutralize triggering noises; Oxford Health NHS Foundation Trust says there is some evidence that CBT can help. None of these sources establishes one universal treatment or cure.
Noise-cancelling headphones and ear protection may be accommodations in some situations, but the International OCD Foundation’s 2015 overview cautions against treating protective devices as a treatment on their own. A sound generator or other masking aid is a different option to discuss in individualized care; evidence here does not establish that a consumer white-noise machine treats misophonia. Avoid assuming that constant sound avoidance, exposure, CBT, or any device will work for everyone.
