MCP Tool Poisoning: How to Stop Malicious Metadata Steering Agents

MCP tool poisoning can turn trusted agent permissions against you. Learn how definition changes and tool outputs influence agents—and how to defend at review and runtime.

MCP tool poisoning occurs when malicious or misleading instructions enter an AI agent through a tool’s description, schema or returned content, steering it to misuse its permissions. Defend against it by reviewing and pinning tool definitions, requiring approval for changes, limiting what agents can access and do, and enforcing policies at runtime. A tool’s annotations or a clean installation scan are not security guarantees.

How tool poisoning works

An MCP server supplies tool definitions to a client. Names, natural-language descriptions and parameter schemas help the agent decide which tool to use and how to call it; tool responses may also be added to the agent’s context. If those fields or responses contain adversarial instructions, the agent may treat them as operational guidance. The attack exploits the trust boundary between the agent, its tools and the information they exchange; it does not require a conventional exploit in the model itself.

The NSA’s May 2026 security-design guidance discusses semantic manipulation of tool metadata and hidden instructions in outputs as possible paths to prompt injection, data exposure and cascading influence in chained workflows. Its examples do not establish how often attacks occur across MCP deployments.

Description poisoning

An attacker embeds directions in a tool description or schema so the agent behaves differently when it encounters that tool. Microsoft Security Research describes a finance-workflow example in which a changed description led an agent to retrieve invoice records and pass a summary to an external enrichment call. The risk comes from the combination of the altered definition and the agent’s available permissions.

Rug pulls

A rug pull occurs when a definition appears acceptable during review but changes afterward. A one-time installation review cannot catch a later change unless the deployment compares definitions with an approved baseline and sends changes through review again.

Tool shadowing and cross-tool influence

Instructions or contaminated context from one tool can affect how an agent uses another. This is why reviewing tools one by one is not enough: the combination of available tools, permissions and shared context in a session matters. The 2025 arXiv preprint on MCP tool poisoning discusses cross-tool influence as part of this broader risk.

What makes a poisoning incident consequential

Malicious text is a risk, but the potential damage depends on what the agent can do with it. A concerning chain is: an attacker controls or compromises a server or its update path; a definition or response is poisoned; the agent loads it; and the agent uses its permissions to retrieve data, call another tool, or take an external action.

Google Cloud distinguishes workflows where a person approves each action from agent-only operation, where actions proceed without waiting for approval. Human approval can reduce risk, but it is not infallible: a person may approve a misleading request. Unattended operation places more weight on the agent’s programming and is more exposed to prompt injection and unsafe tool chaining.

  • Private data: An agent that can read sensitive records has more to expose than one with access only to public information.
  • Untrusted content: Instructions arriving in external documents, tool responses or other untrusted inputs can influence the same context used for decisions.
  • External actions: Network access, messaging, file writes, financial operations or account changes can turn a manipulated decision into a consequential action.
  • Autonomy and shared access: Broad permissions, unattended execution, shared credentials and tools that can reach one another increase the possible impact.

How definition-time governance and runtime enforcement differ

Use both. Governance helps prevent an unreviewed or changed definition from being silently trusted; runtime controls limit what an agent can do if a malicious instruction still reaches it.

Control layer When it acts What it can do Key limitation
Definition-time governance Before production use and when definitions change Verify publishers and update paths; review names, descriptions and schemas; baseline approved definitions; alert on changes and require renewed approval. A review or scan covers the definitions and behavior examined at that time. It does not by itself stop later changes or unsafe calls.
Runtime enforcement When the agent proposes a call or a tool returns data Check permissions and arguments; apply deterministic policies; isolate credentials and execution; block or require approval for sensitive actions; monitor activity. Coverage depends on integration and architecture. A control that does not inspect a particular call, parameter or response cannot enforce policy on it.

A practical defense sequence

1. Establish who owns each server

Keep an approved inventory of MCP servers, their owners, publishers and update paths. Prefer a trusted publisher where appropriate, but still review its definitions and behavior. Do not let an unverified server share credentials, filesystem access or network reachability with trusted tools.

2. Review and baseline definitions

Before production use, inspect tool names, descriptions, parameter schemas and relevant output behavior. Store a known-good definition baseline or fingerprint. Alert on changes, investigate them and require review—and explicit re-approval for sensitive integrations—before modified metadata reaches the agent. Treat a definition change as a production dependency change, not routine text editing.

3. Treat tool content as data, not authority

Tool descriptions and responses from untrusted sources should not outrank the agent’s authorized instructions or your policy. Validate or sanitize returned data before placing it in agent context, and isolate context between users, tenants or agents where appropriate. Delimiters and explicit prompt instructions may help organize context, but they are not dependable enforcement boundaries on their own.

4. Reduce permissions and autonomy

Give each agent only the access needed for its task. Avoid broad allow-all-tools behavior where it can be disabled; separate identities and credentials across servers; sandbox local execution; and restrict filesystem and network access. Require human approval for high-impact actions such as external sharing, financial operations or account changes.

5. Enforce policy at the point of action

Where the risk warrants it, put deterministic policy checks between a proposed model call and tool execution. Validate arguments and outputs, block disallowed calls, require approval for sensitive ones, and retain audit evidence. Microsoft’s Agent Governance Toolkit article describes definition scanning and per-call enforcement, but identifies the toolkit as Public Preview and says sequence-level correlation is not yet available. Microsoft’s Azure MCP deployment guidance also cautions that arbitrary MCP parameters and outputs are not automatically inspected in every architecture. Confirm what a control actually covers in your integration before relying on it.

6. Test combinations and changes

Red-team workflows that combine private data, untrusted content and external communication. Test a changed description after initial approval, a tool response containing instructions, and unexpected expansion of call parameters. Monitor new endpoints and anomalous action sequences. Passing an installation scan cannot establish that a multi-tool workflow remains safe after updates or during use.

What tool annotations can—and cannot—do

MCP annotations such as readOnlyHint, destructiveHint, idempotentHint and openWorldHint give clients vocabulary they may use when deciding whether to warn, ask for confirmation, retry or scrutinize output. The MCP project’s March 16, 2026 article explains that these are hints, not guarantees: servers may be wrong or untrusted, and clients differ in whether and how they use the fields. Treat annotations as one input to a policy decision, never as proof a tool is safe. Enforce requirements such as authorization and network restrictions outside the model.

What the published tests do—and do not—show

Reported attack results illustrate risks under particular test conditions; they are not estimates of how often MCP deployments are compromised.

  • Microsoft for Developers, April 22, 2026: An internal red-team benchmark used 60 prompts—45 adversarial and 15 valid—mapped to OWASP Agentic Top 10 risks. It reported a 26.67% policy-violation rate when relying on prompt-only safety instructions. This is the result of Microsoft’s evaluation, not a universal rate for MCP systems.
  • Cloud Security Alliance Lab Space, 2026: A laboratory research note covering more than 45 real-world MCP servers reported attack success rates above 60%, with the highest-performing tested agent model at 72.8%. These are results from that note’s testing, not evidence of real-world prevalence.
  • NSA guidance, May 2026: The document describes poisoning and hidden-output-instruction risks but the cited material does not establish a population-wide incident rate.
