After ketamine, contact the clinician responsible for treatment about new or concerning symptoms. Confusion or unusual behavior and urinary problems should be reported promptly; nausea or vomiting should be reported if severe or persistent. Severe confusion, difficulty waking, breathing trouble, or other severe symptoms need urgent medical assessment. Advice depends on the exact ketamine product, route, and treatment context.
When to call a doctor after ketamine
Use the treating team’s discharge instructions and contact the clinician who knows what medicine was given, by which route, and for what purpose. For ketamine injection, MedlinePlus says to tell a doctor immediately about confusion, excitement, hallucinations, agitation, difficulty urinating, lower abdominal pain, or painful or frequent urination. It advises contacting the doctor if nausea or vomiting is severe or does not go away. MedlinePlus ketamine injection information

- Seek urgent assessment: severe or persistent confusion or altered awareness, difficulty waking, breathing difficulty, or another severe symptom. Do not wait to see whether a severe change passes.
- Contact the clinician promptly: urinary difficulty or pain, urgency, frequent urination, lower abdominal or genitourinary pain, incontinence, or blood in urine.
- Report nausea or vomiting: call if it is severe or continues; repeated vomiting, inability to keep fluids down, or vomiting with urinary or flank/pelvic pain or blood in urine merits prompt clinical attention.
Ketamine confusion after treatment or unusual behavior
Confusion, excitement, hallucinations, and agitation are listed as serious effects of ketamine injection. Tell a doctor immediately if they occur. Severe or persistent altered awareness, especially if the person is hard to awaken or has trouble breathing, calls for urgent assessment. An outpatient injection patient should have a responsible adult follow the treating team’s discharge instructions.

Ketamine can cause drowsiness. After ketamine injection, MedlinePlus advises not driving or operating machinery for at least 24 hours. This is injection-specific guidance; people using another formulation or route should ask their prescriber what precautions apply.

Nausea after ketamine
Nausea and vomiting can occur. For ketamine injection, contact the doctor if either is severe or does not go away. Repeated vomiting or inability to keep fluids down warrants prompt clinical advice. Seek prompt attention if vomiting occurs with flank or pelvic pain, urinary symptoms, or blood in the urine: FDA labeling includes nausea and vomiting among possible presentations of serious urinary tract complications in the context of long-term use.
Do not add an anti-nausea medicine or follow a specific food or fluid regimen without checking with the treating clinician, since the appropriate response depends on the treatment and the person’s health context.

Ketamine bladder symptoms: pain when urinating, urgency, or blood
Report difficulty urinating, painful urination, urgency, frequent urination, lower abdominal or genitourinary pain, incontinence, or blood in the urine to a clinician promptly. MedlinePlus identifies difficulty urinating, lower abdominal pain, and painful or frequent urination as symptoms to report immediately after ketamine injection.
FDA-approved KETALAR labeling describes urinary tract complications reported in people with a history of long-term ketamine use or abuse. These include cystitis and reduced bladder capacity, as well as ureteral stenosis or obstruction and hydronephrosis. Symptoms may include pain, urgency, frequency, incontinence, and blood in urine. This context does not mean chronic bladder injury is expected after one medically supervised dose. Do not try to treat possible ketamine-related bladder injury with a consumer urinary product; seek clinical assessment. FDA-approved KETALAR prescribing information

MedlinePlus’s patient information concerns ketamine injection, generally administered by a trained healthcare provider for anesthesia. KETALAR labeling is specific to that product and discusses urinary complications in a long-term-use or abuse context. Neither source is a complete guide to every ketamine product, route, or use pattern.
FDA separately warns about compounded ketamine products used for psychiatric disorders, particularly when they are not used with appropriate medical supervision. Compounded drugs are not reviewed by FDA before marketing to evaluate their safety, effectiveness, or quality. Contact the prescriber who knows the exact formulation, route, amount, and treatment plan rather than applying injection information uncritically to another product. Ketamine and esketamine are not interchangeable. FDA-approved esketamine has distinct supervision and restricted-program requirements; ask the treating team about the specific medicine received.
