A caption or thumbnail can help you find an image, but it does not prove that the full-resolution pixels are available—or that you may crop or republish them. For clinical or scientific cropping, obtain the image from an authorized source, preserve the original, and check whether the crop retains the context needed to interpret it.
What image search uses: captions, pixels, or both?
It depends on the search system. Web image search can use textual clues such as page text, captions, titles, filenames, and metadata; some biomedical systems also index image content or modality. It is not safe to assume that every service searches pixels, or that captions are its only signal.

Google documents image-search operators that can filter by source URL or exact dimensions, subject to indexing and retrieval limits. The src: operator finds pages that reference a supplied image URL, while imagesize: filters by image dimensions; neither guarantees a complete collection. Google’s image-search operator documentation explains these limits. Google also describes captions, titles, surrounding text, and filenames as clues that can help establish an image’s subject. A caption is useful textual context, but it is not the image itself. A study by Charles E. Kahn Jr. and Daniel L. Rubin evaluated concept-based indexing of captions from 84,846 radiology figures in 9,004 English-language peer-reviewed articles. In that study’s sampled concepts, estimated precision was 0.897 (95% confidence interval 0.857–0.937) and estimated recall was 0.930 (95% confidence interval 0.838–1.000). These figures describe that specific caption corpus and method, not the accuracy of image search generally. The study is indexed by PubMed.

Does a thumbnail give you full-resolution pixels?
No. A thumbnail is a preview, not evidence that you can open or download the source image at a usable resolution. Some research interfaces have linked thumbnails to higher-resolution figures, but access depends on the system, source, and permissions.
For example, the 2008 Yale Image Finder paper reported a collection of more than 140,000 images from more than 34,000 open-access biomedical papers and described thumbnail click-throughs to higher-resolution images. Those are historical figures and a system-specific workflow, not a current service count or a guarantee that another result offers the same access. The paper is indexed by PubMed.

Another biomedical-search paper describes interfaces that combine images with article text and captions. Separately, a 2023 dataset paper reported 135,596 images from 96,428 clinical cases, paired with labels and captions; its preprocessing included removing borders and splitting some compound figures into panels. These examples show how image files and text records may be presented or assembled separately. They do not establish access rights to images beyond the relevant source or dataset terms. How public image search differs from clinical repositories
A public web result is not the same as an image in a hospital’s picture archiving and communication system (PACS). A clinical image may be held in a controlled repository, where access, privacy protections, and approved uses are governed by the institution and applicable rules. The DIANA paper describes a system interfacing with hospital PACS for research and clinical AI applications; it does not show that public search results expose patient images. The DIANA paper is indexed by PubMed.

When comparing image-search or image-retrieval systems, check four separate capabilities:
- What is indexed: captions and other text, visual content, modality, or some combination.
- What a result displays: metadata, a thumbnail, an extracted figure, a subfigure, or modality information.
- Where the source pixels live: whether they can be opened at sufficient resolution and whether access controls apply.
- What collection is being searched: open literature, a licensed image collection, or a controlled clinical repository.
These are distinct questions. A system may find a figure by its caption while showing only a preview; another may expose a larger figure but still require permission to use it. A search result should not be treated as a diagnostic source or as validated evidence for patient care.

Can you crop an image found through search?
Only if you can access and use the source image under the applicable rules, and the crop preserves the information needed to interpret it. The HHS Office of Research Integrity says cropping is usually acceptable when it trims irrelevant edges or focuses attention without changing context. Its guidance warns against removing information that conflicts with a favored interpretation, recommends retaining the raw original, and notes that enlarging an image with few pixels can create artifacts. There is no single resolution threshold or universal clinical cropping rule established by that guidance. Follow the policies that apply to the specific clinical system, institution, journal, or publication. A crop that hides a relevant control, surrounding anatomy, or contradictory finding can mislead even if the edit looks technically clean.

Does image search grant permission to crop or republish?
No. A search result, caption, metadata field, or license badge is not a substitute for checking the actual terms. Google says its usage-rights filter relies on information supplied by image hosts or providers and recommends verifying licensing requirements. Check both the image host and the license provider for attribution requirements, commercial-use limits, modification rules, and the intended use. Google explains image details, and its usage-rights guidance advises reviewing the license terms.
A practical workflow before making a clinical crop
- Trace the result to its source. Open the original page or repository rather than relying on the search preview or snippet.
- Confirm access to the source pixels. Make sure you have the full image at a resolution appropriate to the task, with required access authorization.
- Verify permission and policy. Check the host’s terms, the rights holder or license provider’s terms, and any institutional or journal rules covering modification and publication.
- Keep an untouched original. Work from a copy so the source can be checked against the edited version.
- Crop only irrelevant edges or to focus without changing context. Retain relevant anatomy, controls, and findings; do not selectively remove information that could alter interpretation.
- Inspect the exported crop. Check that the crop remains clear and does not introduce artifacts through enlargement, then apply any required attribution.
If you cannot verify the source pixels, authorization, or reuse terms, do not treat the thumbnail as a clinical image ready for cropping or publication.
