The reviews briefly appeared in ICE’s public records library in September 2026, then were taken offline. Reporting by Reason and the San Francisco Chronicle describes what the reviews said, but does not establish who ordered their removal or why.
When the reviews appeared—and disappeared
The Intercept reported that the mortality reviews appeared on ICE’s public records library on September 25, 2026. They were removed four days later, after a lawyer representing The Intercept in a Freedom of Information Act lawsuit discovered them. Reason reporter C.J. Ciaramella reported on the episode on October 1. Project Salt Box downloaded and republished the files, according to Reason and the San Francisco Chronicle.
The reviews concerned 33 deaths in ICE detention from September 2019 through January 2026. Their brief online availability and subsequent removal are reported by those outlets; the published accounts do not identify the person who ordered the removal or establish the reason for it.
What the reviews reportedly found
The accounts describe substantial concerns about care, but they report different denominators. Those figures should be kept separate unless the underlying reviews are examined and reconciled.

- Reason’s account: 31 reviews reached a conclusion. Of those, 27 found that care was “not provided within the safe limits of practice” or “deviated beyond safe limits.” Three reviews said deficiencies may have contributed directly or indirectly to a death.
- The Chronicle’s account: 27 of the 33 documents found care outside safe practice limits, while four found care within those limits.
These are not interchangeable counts: Reason describes 31 reviews with conclusions, while the Chronicle counts across 33 documents. Neither account, as summarized here, explains the status of every document in a way that resolves the difference.
Reason reported recurring problems that included sick calls being ignored or handled by phone, interruptions in medication, and deficient or absent lifesaving measures such as CPR. The findings are described in news reports about internal reviews; they should not be read as independently verified here or as a ruling about every ICE facility.

Two deaths illustrate the reported care concerns
Marie Ange Blaise
The Chronicle reported that Marie Ange Blaise, a 44-year-old Haitian woman, died of a heart attack in April 2025 at Florida’s Broward Transitional Center after 70 days in ICE custody. According to the Chronicle’s account of the internal review, staff deviated from basic life-support protocols, did not begin CPR despite Blaise being pulseless, and omitted vital signs from emergency-response documentation.
That account conflicts with ICE’s public death report, which, Reason reported, said staff promptly began CPR and continued until emergency medical services arrived. The available descriptions establish a discrepancy between the internal review and the public report; they do not resolve it.

Ismael Ayala-Uribe
The Chronicle reported that Ismael Ayala-Uribe died on September 22, 2025, from septic shock caused by an abscess. Reason’s account of the internal review said staff did not visually inspect the area he had complained about, did not administer medication or perform ordered wellness checks, and did not respond for more than 13 hours after guards asked medical staff to check on his ill appearance.
What ICE and a medical-care critic said
ICE told the Chronicle that it remained committed to safe, secure and humane conditions. The agency said: “All people in ICE custody receive medical, dental and mental health screening and 24-hour emergency care at each detention facility.” That describes ICE’s stated provisions; it does not answer the specific criticisms attributed to the mortality reviews.

The Chronicle quoted Dr. William Weber, medical director of the Medical Justice Alliance and a practicing emergency physician, as saying: “The ICE spokesperson responses do not acknowledge the glaring gaps in care highlighted by the government’s own reports.” That is Weber’s assessment, not an agency finding.

Electronic health-record weaknesses
A separate Department of Homeland Security Office of Inspector General audit, issued July 28, 2026, found that ICE’s electronic health-record system did not fully meet needs for transferring and accessing medical records, lacked a patient portal for detained people after release, and did not consistently meet availability requirements. The OIG said those shortcomings could lead to untimely or inaccurate healthcare decisions and attributed them to weak contract administration and oversight. The audit concerns system capacity and oversight; it is not a finding about the 33 mortality reviews.

Earlier death-review study
A peer-reviewed 2021 case series examined ICE detention death reviews from 2011–2018, predating the deaths covered in the newly reported reviews. It found medical-care standards violations in 43 reviews (78.2%). Among 47 deaths from medical causes, markedly abnormal vital signs were documented before death or terminal hospital transfer in 29 cases (61.7%). Because the time periods and review sets differ, these historical results are context, not a direct comparison with the 2019–2026 reviews.
Post-release death reporting is a separate issue
The Washington Post reported in June 2026 that ICE ended a requirement, adopted in 2021, to report deaths occurring within 30 days after release. That policy change concerns post-release death reporting. It is distinct from the online removal of the mortality reviews described above.
Why did ICE remove the reports?
The reporting establishes that the reviews appeared online and were later removed, but does not establish who made that decision or why. Without an attributed explanation or other evidence, a motive should not be inferred from the timing of the removal or the findings described in the reviews.
