Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteI built a scanner to focus on a specific failure: an AI agent can cause a consequential tool action without a trusted component verifying that the right person approved that exact action. The important question is not whether the model says “approval required.” It is whether execution is blocked until authorization and approval checks pass for the action that will actually happen.
How can an AI agent bypass human approval?
An agent can receive instructions indirectly through the material it reads: a web page, email, retrieved document, or tool result. Malicious content in one of those channels may steer the agent toward an unintended action. NIST describes this as agent hijacking through indirect prompt injection; OWASP also identifies prompt injection, tool abuse, excessive autonomy, approval manipulation, and cascading failures as agent-security risks.

Untrusted content becomes an instruction
Content that should be treated as data can influence the agent’s plan. Testing only direct user prompts misses this route: the adversarial input needs to arrive through the external-content channel being assessed. OWASP recommends separating untrusted input from trusted instructions, and NIST’s evaluations examine hijacking through ingested data.

A tool’s scope exceeds the task
If an agent has broader permissions than the task needs, manipulated behavior can reach unrelated operations. OWASP’s excessive-agency guidance recommends least privilege and executing with the relevant user context rather than giving the agent ambient authority.

Approval is advisory, broad, or replayable
A model may classify an action as risky or ask a person to approve it, but neither statement blocks execution on its own. Approval can also be too broad if it covers a class of actions rather than the concrete target and parameters, or unsafe if a previously granted approval can be reused. OWASP’s AI Agent Security Cheat Sheet calls for independent validation, exact-action binding, expiry, replay protection, and fail-closed behavior.
Arguments cross into unsafe operations
Model-generated shell commands, API requests, or code can turn untrusted values into unintended operations when arguments are not validated or safely parameterized. OWASP’s MCP command-injection guidance places particular importance on controls at the execution boundary.
A coding agent inherits workstation access
A coding agent may be able to run commands, install packages, change files, or use the network. If its context is manipulated, those capabilities can expose developer-level permissions. OWASP’s secure-coding guidance recommends sandboxing and scoped tools and credentials.

Where should approval be enforced for AI tools?
Enforce it at the trusted execution boundary: the component that can authorize or deny the tool call before it produces a side effect. Keep the model’s proposal separate from execution. The model can suggest an action, but downstream code or systems must independently check the actor’s authority, applicable policy, and required approval. OWASP’s LLM06:2025 guidance explicitly recommends implementing authorization downstream rather than relying on an LLM to decide whether an action is allowed.
Approval and authorization answer different questions. Authorization asks whether this actor may perform this operation on this target. Approval asks whether the required human review has occurred. A high-risk label, guardrail pass, or natural-language assurance from the model establishes neither by itself.
Bind approval to the action that will execute
An approval record should identify the actor, tool, target, normalized parameters, timestamp, and expiry, as recommended by OWASP. The execution component should compare the approved action with the proposed call. A material parameter change should invalidate the approval; irreversible operations should resist replay through short-lived authorization artifacts and replay protection.

Fail closed when checks fail
If approval verification, policy lookup, risk classification, or required audit recording fails, a high-impact action should not run. Unknown or unclassified operations should not silently inherit permission. Every downstream request needs the relevant authorization check, not just the first request in a workflow.

How do I test approval gates in an AI agent?
Assess the complete path from input to side effect, not just the prompt or the approval screen. The following are design criteria for an approval-bypass scanner; they should not be read as claims about measured coverage or a particular scanner’s implementation.
1. Map tools, identities, and side effects
- Inventory agent identities, tool names and descriptions, argument schemas, permission scopes, and downstream effects.
- Include dynamically discovered tools as well as configured tools where applicable.
- Trace which user identity and credentials are used for each operation.
2. Trace where instructions and data come from
- Mark direct user input, retrieved content, tool output, and delegated or peer-agent input as distinct trust channels.
- Place harmless adversarial test content in the channel under assessment—for example, a test document or instrumented tool result—rather than moving it into the user prompt.
- Check whether untrusted content can change a proposed target, operation, or parameter.
3. Identify actions that need review
Check whether policy requires approval for destructive, financial, administrative, externally visible, or system-modifying operations. Include unknown and unclassified actions in the evaluation, and verify that policy does not default to execution. OWASP’s MCP guidance also highlights human review for sensitive actions.

- Widely Compatible: Bluetooth Barcode Scanner for iPhone iPad Android Tablet PC, Support HID / SPP / BLE mode via bluetooth, Work with Windows XP/7/8/10, Mac OS, Windows Mobile, Android OS, iOS, Linux.
- Strong Recognition Ability: With the 2500 pixels high-resolution CCD sensor Engine, Rapidly decodes all 1D and stacked barcodes (including ISBN book), even worn, damaged or tightly spaced codes. Scan 1D codes directly from paper or screen, such as a computer monitor, smartphone, or tablet, or scan through glass surfaces, plastic shrink wrap, a CCD scanner is likely the best way to go.
- Automatic Scanning: NT-1228bc barcode scanner have three scanning modes: manual trigger mode, continuous scanning mode and auto-sensing scanning mode. In addition, there is a storage mode. Storage mode can be used when you are out of range of Bluetooth and wireless connectivity. Supports storage of up to 100,000 barcodes. Note: Before use, you need to scan the corresponding setting barcode on the manual.
- 2600mAh Battery Upgraded: Continuous scanning up to 200,000 times on a full charge. After a full charge the scanner can be used for one month at least, even in warehouses and at pos checkout counters where scanners are frequently used. In libraries and hospitals it can be used even longer.
- Programmable Configuration: Add custom prefixes/ suffixes, delete characters, Add keyboard keys/ combinations (terminator TAB, CR&LF, Home etc.), Enable or disable the barcode type as you want. Buzzer can be set to mute to allow for a quiet operation.(Note: It does not work with square POS / Divalto / DoorDash / Lightspeed POS system)
4. Verify exact-action binding
- Confirm the approval refers to the actor, tool, target, normalized arguments, timestamp, and expiry.
- Change a parameter after approval and verify that the changed call is rejected or requires fresh approval.
- Repeat an approved request and verify that an expired or already-used approval cannot authorize an unintended replay.
5. Check the execution decision independently
Observe whether downstream execution independently validates authorization and approval, or instead trusts a model-produced risk label, a prompt, or a natural-language claim that the user approved. Use instrumented tools or safe substitutes so a failed control cannot cause a real side effect.
6. Record expected and observed behavior
For each test case, state the expected policy outcome and record the observable result: whether execution was attempted, which checks ran, and whether the action was blocked. Audit records should connect the originating input, proposed action, authorization decision, approval state, and execution result. Protect sensitive data in those logs, as OWASP’s monitoring and auditability guidance recommends.
7. Retest as the system changes
Use task-specific adversarial cases, repeat attempts, and revise the evaluation as tools, permissions, and attack techniques change. NIST’s 2025 discussion of agent-hijacking evaluations recommends adaptive evaluation and notes that multiple attempts can give a more realistic picture than a single run. Its evaluation observations are qualitative, not a general prevalence estimate.

What should an approval-bypass test matrix cover?
A compact matrix keeps the test focused on both the route into the agent and the control at execution. Use dummy data and instrumented tools; the outcomes below are test expectations, not reported results from a benchmark.
| Test case | What to vary | Expected safe outcome |
|---|---|---|
| Indirect instruction | Place harmless adversarial content in a web page, document, email, or tool result. | The content cannot cause an unapproved consequential operation. |
| Excessive scope | Request a task that needs only a subset of available tools or permissions. | Unneeded operations remain unavailable or are denied under the user’s authorization. |
| Approval parameter change | Change the target or a normalized argument after approval. | The changed action is rejected or requires new approval. |
| Expired or repeated approval | Reuse an expired approval or repeat a previously authorized irreversible request. | Expiry and replay protections prevent unintended execution. |
| Malformed or unclassified call | Supply invalid arguments or an action not covered by policy. | Schema or policy validation blocks execution rather than guessing. |
| Control-service failure | Simulate unavailable approval verification, policy lookup, or required audit recording. | A high-impact side effect does not proceed. |
What can a scanner prove—and what can it not?
A scanner can expose missing or weak controls in the paths it exercises. It cannot prove that an LLM will never be manipulated, or that untested inputs and future changes are safe. OWASP warns that guardrail models have attack surfaces too, and that filters and prompts are illustrative layers rather than a complete defense against injection.
Interpret each finding as evidence about a specific path: the input involved, proposed tool call, policy decision, approval state, and observed outcome. A useful result points to a control that can be corrected at the execution boundary; it is not a universal guarantee about agent behavior.

- Give each agent only the tools and operation scopes required for its task, with downstream permissions matching the user’s authorization.
- Sandbox coding agents in restricted shells, containers, virtual machines, or ephemeral workspaces; constrain credentials, filesystem access, commands, and network egress to what the task needs.
- Validate model-produced arguments against schemas and use safe parameterized APIs or process invocation rather than assembling unsafe commands from untrusted values.
- Keep actionable audit trails, monitor for behavioral drift, and update adversarial evaluations as the system changes.
These controls complement execution-side approval; they do not turn a model’s own judgment into an authorization check. OWASP’s prompt-injection, secure-coding, excessive-agency, and MCP guidance supports this defense-in-depth approach.
