How to Validate Medicare Advantage Quality Data Before Submission

Validate Medicare Advantage quality data with a contract-year scope, field-level lineage, completeness and format tests, source reconciliation, QRS/HEDIS controls and an evidence package ready for CMS-required retrospective validation.
Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Validate Medicare Advantage quality data by fixing the contract year and reporting scope first, then tracing every reported value to source records, testing completeness and definitions, reconciling results, documenting exceptions, and preserving evidence another reviewer can reproduce. Part C and Part D reporting data also require an independent retrospective validation by an external contractor, while QRS/HEDIS measures require source, capture, format, quality-control and rate-impact checks.

Start with the exact CMS reporting scope

A validation plan is only reliable when it is tied to a specific contract year, contract, reporting section and measure set. CMS states that reporting requirements and technical specifications can change from year to year, so a prior-year workbook or deadline cannot be carried forward without checking the current materials. The CMS Part C and Part D Reporting Requirements page listed the CY2026 requirements and technical specifications dated December 1, 2025, and was last modified August 5, 2026.

  • Medicare Advantage or Part D contract number and legal entity responsible for submission.
  • Contract year and the data period covered by each measure.
  • Reporting section, measure name, indicator or field identifiers, and submission mechanism.
  • Final CMS technical-specification title, version or date, and the date your team adopted it.
  • Business owner, data owner, preparer and independent reviewer.

Do not guess a deadline or required element from a general checklist. The applicable final CMS document controls the data elements, reporting level, format and due date for that submission.

Choose the correct validation pathway

“Quality data” can refer to different CMS programs with different evidence standards. Classify the data before selecting tests.

Data purpose Primary validation focus Independent review requirement Authoritative material
Part C or Part D Reporting Requirements Contract-year specification, reporting section, field definitions, completeness, reconciliation and reproducibility Annual retrospective validation in the year after the contract year by an external contractor selected by the sponsoring organization CMS Part C and Part D Data Validation
QRS/HEDIS-related quality measures Source capture, file formats, quality controls, effect on reported rates and primary-source evidence where needed HEDIS audit results include audited rates or calculations at measure and indicator level and indicate whether measures may be publicly reported CMS 2027 QRS Measure Technical Specifications
Risk-adjustment diagnoses Whether diagnosis codes are supported in enrollee medical records RADV audit scope; unsupported diagnoses may lead to overpayment recovery CMS Medicare Advantage Risk Adjustment Data Validation Program

Keep these pathways separate in governance, evidence and terminology. Reporting-data validation asks whether the submitted measure information was developed correctly. RADV asks whether risk-adjustment diagnoses are supported by medical records; it is not a substitute for quality-reporting controls.

Build a field-level data lineage record

For each reported field, numerator, denominator and category count, maintain a lineage record that lets an independent reviewer start with the submitted value and work backward to the source.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Identify the source and accountable owner

Name the originating system or file, the table or report used, the extraction date and the person accountable for its meaning. If a vendor, delegated entity or provider supplies data, retain the source contact, delivery date and file identifier. A source name alone is not enough when several extracts exist for the same period.

Describe extraction and transformation logic

Store the query, script, configuration or documented manual steps that produced the submission dataset. Record filters, joins, look-up tables, exclusions, deduplication rules, date conversions, code mappings and aggregation logic. Preserve the exact version used for the submitted file rather than only the current version of a script.

Map the result to the CMS definition

For every measure or field, link the internal variable to the CMS definition, reporting level, required format and allowed values in the applicable specification. Capture interpretation decisions, such as how an encounter date is selected or how a member is assigned to a denominator, in a decision log approved by the measure owner.

Track lineage through handoffs

When data move from a clinical system to a warehouse, quality platform and submission file, give each handoff an identifier and retain row counts before and after the transfer. This exposes silent truncation, failed joins and accidental reprocessing that a final aggregate cannot reveal.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Test completeness, format and validity before calculating rates

Run structural tests before reasonableness tests. A plausible rate can still be invalid if records are missing or coded incorrectly.

Required fields and record presence

  • Check that every required field in the current specification is populated or handled according to its stated rule.
  • Compare expected reporting populations with extracted populations by contract, period, product and other required dimensions.
  • Investigate nulls, blank strings, default values and records rejected during file creation.
  • Confirm that records expected from delegated entities or supplemental sources were received and loaded.

Formats and allowed values

  • Validate field types, lengths, decimal precision, date formats, sign conventions and character encoding.
  • Check categorical fields against the specification’s allowed values; do not silently convert an unrecognized code to a valid category.
  • Verify that identifiers retain leading zeroes and that member, contract and provider keys use the required representation.
  • Run duplicate tests at the grain defined by the measure. A duplicate may be a repeated event, repeated member-period record or repeated file row, depending on the specification.

Periods and eligibility windows

Confirm that service, enrollment and measurement dates fall inside the required period and that time zones or end-of-day rules do not shift events across boundaries. Test the relationship between enrollment status and measurement dates so members are not counted when they fail the measure’s eligibility rule.

File-level controls

Before transmission, compare the final record count, file size, control totals and rejection count with the values produced by the approved run. Open the file in a validation environment that matches the submission format; a spreadsheet view is not a substitute for a parser that checks every row.

Reconcile numerators, denominators and totals to source records

Reconciliation demonstrates that the submitted aggregates still represent the underlying events. Use independently calculated totals where practical rather than relying on the same query that generated the submission.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Perform two-way counts

For each measure, tie the denominator back to the eligible-member or eligible-event population in the source system. Tie the numerator to the subset satisfying the numerator definition, and reconcile category counts so their sum equals the relevant total when the specification requires mutually exclusive categories.

Explain every difference

Set a documented tolerance only when the governing specification or approved internal policy supports one. Otherwise, investigate every mismatch. Common causes include late-arriving claims, changed eligibility, duplicate events, code-map updates, excluded records and joins that fail on formatting differences.

Test rate reasonableness without inventing thresholds

Compare current results with prior periods, parallel source reports and expected population changes. An unusual rate is a trigger for investigation, not proof of an error. Record the business explanation for a genuine change, such as a benefit redesign or source-system migration, and retain evidence supporting that explanation.

Rerun after corrections

Do not edit a final aggregate manually to resolve a discrepancy. Correct the source logic or mapping, rerun the controlled process, regenerate the file and update the reconciliation. Retain the superseded output and identify it as replaced so reviewers can follow the change history.

Apply QRS and HEDIS-specific source controls

QRS/HEDIS validation goes beyond checking the final rate. CMS’s 2027 QRS technical specifications call for review of data sources, capture methods, file formats, quality-control processes, the effect on reported rates and primary-source evidence where appropriate.

Document each source type

For administrative claims, enrollment files, clinical data, surveys, registries or supplemental sources, record what the source captures, its coverage period, known exclusions and the process used to load it. A measure can be technically correct against an incomplete source and still understate performance.

Test capture and mapping

Trace a sample of qualifying events from the primary system through the warehouse and into the measure dataset. Test both positive cases that should count and negative cases that should not. Verify code-set versions, value-set mappings and effective dates used by the measure.

Evaluate impact on the reported rate

When a source is added, removed or corrected, calculate the resulting change in numerator, denominator and rate. Keep the before-and-after counts and the reason for the change. If a source affects only a subset of members or indicators, document that scope rather than describing the change as a measure-wide effect.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Prepare primary-source evidence

Retain the records needed to verify sampled cases, subject to applicable privacy and security controls. Index the evidence to member or event keys so an auditor can locate it without receiving unrestricted access to an entire production database.

Understand audit outputs

CMS guidance indicates that HEDIS audit results include audited rates or calculations at measure and indicator level and identify whether measures may be publicly reported. Store the audit result with the version of the data and specification it evaluated; a later rerun may not be covered by the same conclusion.

Prepare for Part C and Part D retrospective validation

CMS describes Part C and Part D reporting-data validation as an annual retrospective activity conducted in the year after the contract year. The sponsoring organization must select an external contractor to perform the independent work under CMS specifications; the organization’s own staff do not perform that independent validation.

Engage the external validator early

Confirm the contractor’s scope, data-request format, secure transfer method, sampling approach and milestones before the submission is frozen. Early coordination prevents a source extract from expiring or a transformation owner from becoming unavailable before the retrospective review.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Give the validator reproducible materials

Prepare a controlled package containing the adopted CMS specification, submitted file, source extracts or access-controlled evidence, transformation code or procedures, data dictionaries, reconciliation results, exception log and approvals. Label each artifact with a version and creation date.

Separate preparation from approval

The person who builds the file should not be the only person approving it. An independent reviewer should check the final file against the applicable specification, inspect unresolved exceptions and confirm that the evidence package can reproduce the reported values.

Track findings to closure

For each validator question or finding, record the affected measure or field, root cause, corrective action, owner, due date and evidence of resolution. If a correction cannot be made for the submitted period, document the limitation and its effect instead of altering historical evidence.

Keep reporting validation separate from RADV

RADV is a distinct Medicare Advantage audit focused on whether risk-adjustment diagnoses are supported in enrollee medical records. CMS notes that unsupported diagnoses may trigger overpayment recovery. A clean quality-reporting submission does not establish medical-record support for a risk-adjustment diagnosis, and a RADV result does not validate a QRS numerator or denominator.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use separate inventories, owners, access controls and evidence indexes for these programs. If one source system feeds both, document the shared extraction and then apply the different definitions and review requirements independently.

Preserve an evidence package that can be reproduced

Reproducibility means another qualified reviewer can obtain the same submitted values from the retained evidence, not merely read a narrative describing the process.

  • Scope evidence: contract, year, reporting section, measure set and adopted CMS specification.
  • Source evidence: dated extracts, source reports, delivery confirmations and data dictionaries.
  • Logic evidence: queries, scripts, mapping tables, parameter files and documented manual procedures.
  • Control evidence: completeness, format, duplicate, date-range and allowed-value test results.
  • Reconciliation evidence: source-to-submission counts, rate calculations, variance explanations and rerun outputs.
  • Exception evidence: issue description, impact assessment, resolution, approval and residual limitation.
  • Review evidence: reviewer sign-off, approval date, final-file checksum or equivalent file identity, and the exact copy submitted.
  • Sample evidence: primary records supporting selected QRS/HEDIS cases, stored with appropriate privacy protections.

Use immutable or access-controlled storage where available, and maintain a retention schedule that meets the organization’s obligations. A file without its parameters and source context may be impossible to reproduce even if it opens correctly.

Use a controlled timeline from specification to submission

  1. Scope: identify contract, year, reporting section, measures, owners and final specification version.
  2. Design: map every field to a source, definition, transformation and control.
  3. Extract: create dated source snapshots and record row counts and delivery details.
  4. Transform: run version-controlled logic and retain parameters, mappings and intermediate counts.
  5. Test: execute completeness, format, date, allowed-value, duplicate and internal-consistency checks.
  6. Reconcile: tie numerators, denominators and totals to source records; investigate and rerun after corrections.
  7. Review: have an independent reviewer inspect the file, exceptions and evidence package.
  8. Freeze: preserve the approved submission copy and record any open limitation.
  9. Coordinate: provide the retained materials needed for the external retrospective validation.
  10. Recheck: verify CMS updates and final instructions near submission because specifications can change annually.

Build time for late data, corrective reruns and secure transfer. Treat the submission date as the end of the review window, not the start of validation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Common failures and how to correct them

Using last year’s specification

Symptom: fields, deadlines or definitions do not match the current CMS file layout. Correction: replace the inherited mapping with the final contract-year specification, log the version and rerun all dependent tests.

Relying on an aggregate report with no source trail

Symptom: the team can show a rate but cannot identify the records or logic behind it. Correction: rebuild the lineage record, preserve source extracts and rerun from controlled logic before approval.

Accepting unexplained variances

Symptom: totals differ from source reports and the difference is described as rounding or “system timing” without evidence. Correction: isolate the affected population, identify the actual cause and retain the calculation supporting the explanation.

Fixing the file manually

Symptom: a spreadsheet edit changes a value but leaves no reproducible transformation. Correction: correct the source rule or mapping, rerun the process and preserve both the replaced and approved outputs.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Ignoring supplemental-source controls

Symptom: a supplemental file changes a rate but there is no documentation of capture, format or quality checks. Correction: document source coverage, validate the file structure, test mapped events and quantify the rate impact.

Confusing internal review with independent validation

Symptom: the plan assumes its own staff satisfy the Part C/Part D independent-validation requirement. Correction: arrange the external contractor engagement and retain evidence of the independent retrospective work.

Monitor integrity signals that can affect ratings

Completeness and integrity are not merely technical concerns. In its October 8, 2026 fact sheet for the 2027 Star Ratings, CMS described using Part C Reporting Requirements data to check the completeness of IRE appeals data and applying scaled reductions when it identified data-integrity issues. That makes population coverage, missing records and unexplained changes material to rating outcomes as well as submission acceptance.

Use this signal to prioritize controls around high-impact reporting sections: prove that the expected population was loaded, document exclusions, investigate abrupt shifts and retain the evidence used to support any correction. Do not infer a universal reduction formula or numeric threshold; the applicable Star Ratings materials and contract-year instructions govern the actual treatment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Final decision test before transmission

Transmit only when the team can answer “yes” to each question without relying on memory:

  • Is the contract year and final CMS specification identified and versioned?
  • Can every submitted value be traced to a dated source and documented transformation?
  • Did required fields, formats, dates, allowed values and duplicate rules pass?
  • Do numerators, denominators and totals reconcile to source records?
  • Are unusual results and exceptions explained with retained evidence?
  • For QRS/HEDIS data, are source capture, file formats, quality controls, rate effects and primary records addressed?
  • Has an independent reviewer approved the exact file being submitted?
  • Is the evidence package ready for the required external retrospective validation?

Recheck the current CMS reporting and validation pages immediately before the submission window closes. The governing documents for the contract year, not a standing internal template, determine what must be reported and how it will be validated.
