A generic TENS unit, ear clip, or consumer stimulator is not a substitute for either.
First identify the device and what it is meant to treat
Before using a device, check its name, model, intended use, and instructions. The two systems discussed here have different procedures and medical purposes; their directions cannot be transferred from one to the other.

Using gammaCore at home
gammaCore is a prescription device. Its reviewed guide says U.S. federal law restricts its sale to or on the order of a licensed health care provider. The manufacturer provides regional safety materials; use the current instructions for your country and exact device rather than relying on an older guide or a short summary.

Prepare and position the device
The following are gammaCore-specific directions from the reviewed user guide, not general instructions for other stimulators. Use the illustrations and full current guide to identify the treatment location.
- Check the prescribed treatment plan. The device’s available therapy is loaded according to the health care provider’s prescription. Follow your prescribed indication and schedule rather than choosing or combining schedules yourself.
- Prepare the treatment area. Make sure the skin at the treatment location is clean and dry. Remove the device cap.
- Apply the gel. Put a pea-sized amount of the specified gel on the stimulation surfaces, following the guide. Do not substitute a different product unless your provider or the current instructions say it is suitable.
- Place the device as shown in the guide. The illustrated site is on the side of the neck, using landmarks near the pulse, in front of the large side-neck muscle, just below the jaw, and beside the windpipe. Do not guess at the position from a written description alone; use the device illustrations and ask your clinician to check your technique if you are unsure.
- Maintain gentle contact. Use mild to moderate pressure to keep the stimulation surfaces against the skin. The guide cautions against excessive pressure on the neck.
- Set stimulation as directed. The guide describes increasing intensity to the maximum tolerable level. Do this only as the guide and prescription direct; treatment should not be painful.
Follow your own schedule, not a universal dose
The gammaCore guide says each stimulation lasts two minutes and gives separate schedules and daily maximums for migraine and cluster headache. Those schedules are not interchangeable, and a provider may direct otherwise. The exact schedule depends on your prescription and applicable current guide; do not infer a number of daily treatments from another patient’s plan or from a general online summary.

Know what a session may feel like
The manufacturer describes tingling at the contact area that may spread toward the corner of the mouth, and a slight downward pull of the lip, as possible sensations. It says treatment should never be painful. Stop and contact your prescriber if you experience pain or are uncertain whether a sensation is expected. If a session does not help, the guide says to continue usual medications and seek medical attention if necessary; do not replace prescribed medication or urgent care with device use.
Check gammaCore safety before using it
The manufacturer’s FAQ summarizes contraindications and groups for whom safety and efficacy have not been evaluated. It is not a replacement for the complete, current instructions. Review those instructions with your prescriber, especially if any of the following applies:

- Active implanted electronic device: the manufacturer lists an active implanted medical device, such as a pacemaker, as a contraindication.
- Another device or portable electronics in use: the FAQ lists simultaneous use of another stimulation device, such as a TENS unit or muscle stimulator, or a portable electronic device as a contraindication. Ask your prescriber how to handle nearby or concurrent electronics rather than experimenting.
- Relevant medical history: the FAQ says safety and efficacy have not been evaluated for people with carotid atherosclerosis, a metallic implant at or near the neck, prior cervical vagotomy, children younger than 12, pregnancy, or clinically significant hypertension, hypotension, bradycardia, or tachycardia.
If you have an implanted device, one of these conditions, a prior neck procedure, or any doubt about using gammaCore alongside other electronics, do not improvise: ask the prescribing clinician before treatment. Product warnings and labeled uses can vary by country and product version, so confirm that the guide matches your device and location.

Vivistim is not a self-installed or general-wellness stimulator. The system is implanted for a specific stroke-rehabilitation purpose: its pulse generator is placed in the chest, and a lead runs under the skin to electrodes on the left side of the neck. A health care provider programs it.

For home rehabilitation, the patient receives a magnet and training to start stimulation during prescribed exercises. The FDA’s August 27, 2021 announcement says patients are trained to use the system at home and its safety features; home use does not mean that patients operate clinician programming software or the wireless transmitter. Follow the rehabilitation team’s training and exercise plan. Ask the implanting or treating team about personal medical-history concerns and device-specific warnings; do not apply gammaCore placement or dosing directions to an implanted system.

The FDA described a study of 108 participants at 19 clinical sites in the United States and United Kingdom. At six weeks, average Fugl-Meyer Upper Extremity (FMA-UE) assessment improvement was 5 points in the treatment group and 2.4 points in the control group. At 90 days after therapy, 47.2% of the treatment group and 23.6% of the control group had improved by at least 6 FMA-UE points. These are results for that study population, not a promise of an individual outcome. The FDA announcement also discusses adverse events observed in the study and topics to review with providers; that study-specific list should not be treated as a complete warning list or as guidance for noninvasive devices.

When to pause and contact your care team
- You cannot confirm the device model, current regional instructions, treatment location, or prescribed schedule.
- You have a pacemaker or another implanted electronic device and are considering gammaCore.
- You experience pain during gammaCore treatment, or a sensation does not match what the device instructions describe.
- You are considering using another stimulator or portable electronic device at the same time as gammaCore.
- Your symptoms do not improve or you need medical attention; device use is not a reason to stop usual medication or delay care.
- You use Vivistim and need clarification about magnet activation, trained exercises, or your implanted system’s safety instructions.
