A fatty liver result does not, by itself, tell you whether the liver is inflamed or scarred. Blood tests, routine scans and elastography each measure different things: liver-cell injury, visible fat or fibrosis risk. Clinicians interpret them together with your health history and risk factors; no single enzyme value or scan phrase can determine disease stage.
What “fatty liver” means on a report
Current guidance generally uses the name metabolic dysfunction-associated steatotic liver disease (MASLD). Older records and articles may use nonalcoholic fatty liver disease (NAFLD), a previous name for a related condition. If a report says “hepatic steatosis,” “fatty infiltration” or “echogenic liver,” it is describing fat seen or suspected in the liver—not necessarily inflammation or scarring.

Think of the tests as answering separate questions: are liver cells showing signs of injury, is fat visible, and is there evidence suggesting fibrosis (scarring)? A clinician combines those findings rather than treating them as interchangeable.
How to read the blood tests
ALT and AST: signs of cell injury, not a fibrosis stage
ALT (alanine aminotransferase) and AST (aspartate aminotransferase) are enzymes often measured in blood tests. Raised levels can occur with MASLD, but they are not specific to it and do not measure scarring directly. An elevated result may also reflect another or additional condition, including viral or autoimmune hepatitis, hemochromatosis or alcohol-associated liver disease. A clinician interprets the result alongside the rest of the evaluation. Indian Health Service guidance on MASLD

Results within a laboratory’s reference range do not prove that liver fat or fibrosis risk is absent. Likewise, an elevated ALT or AST alone cannot establish the cause or severity.
FIB-4: a risk estimate using several results
FIB-4 combines age, AST, ALT and platelet count to estimate the likelihood of advanced fibrosis. It is a triage score—not a diagnosis of MASLD, MASH (metabolic dysfunction-associated steatohepatitis) or cirrhosis. A low score can support lower risk in a clinical pathway, but it does not mean there is no liver fat or that progression is impossible.

The 2024 EASL-EASD-EASO guideline treats FIB-4 above 1.3—or above 2.0 for people older than 65—as indicating increased risk of advanced fibrosis. It describes 1.3 to 2.67 as an intermediate range in which next steps depend on history, overall risk and available resources. These are pathway cutoffs to guide follow-up, not universal diagnostic boundaries.
In that guideline’s pathway, clinicians may use elastography to clarify an intermediate result, particularly when the score is closer to 2.67 or the person has higher risk. Another option is to address risk factors and repeat assessment after a year, with elastography if the score remains elevated. False-positive scores can lead to unnecessary follow-up, especially where advanced fibrosis is uncommon. Age affects interpretation, and FIB-4 should not be used in acutely ill patients; follow local clinical guidance rather than acting on a calculator result by itself. What liver imaging can—and cannot—show
Routine ultrasound, CT and MRI
Routine ultrasound, CT and MRI can show fat or other structural findings. But a routine scan cannot establish microscopic inflammation or fibrosis stage. The NIH explains: “Routine imaging tests can show fat in the liver, but they cannot show inflammation or fibrosis.” NIH: diagnosis of fatty liver disease
Conventional ultrasound may miss milder amounts of fat and is not a precise measure of severity. A report noting a fatty appearance should therefore be understood as evidence of fat, not a complete assessment of tissue injury or scarring. CT and MRI offer different imaging detail, but a routine CT or MRI is not equivalent to an elastography study that estimates stiffness. RadiologyInfo: fatty liver disease

Elastography: estimating stiffness
Elastography estimates liver stiffness as an indicator of fibrosis risk. Vibration-controlled transient elastography (VCTE), often called FibroScan, and magnetic resonance elastography (MRE) are examples. In the EASL-EASD-EASO pathway, elastography is commonly considered after a blood-based score when concern remains or risk is higher. The result depends on the technique and clinical setting; it is an estimate, not a microscopic tissue examination. 2024 EASL-EASD-EASO guideline

How the tests fit together
| Test | What it helps assess | What it cannot establish alone |
|---|---|---|
| ALT and AST | Possible liver-cell injury | Cause of the enzyme change or fibrosis stage |
| FIB-4 (age, AST, ALT and platelets) | Risk estimate for advanced fibrosis | A definitive diagnosis of MASLD, MASH or cirrhosis |
| Routine ultrasound, CT or MRI | Visible fat or structural findings | Microscopic inflammation or fibrosis stage |
| VCTE or MRE elastography | Estimated liver stiffness to help assess fibrosis risk | Direct microscopic confirmation of tissue changes |
| Liver biopsy | Tissue changes and microscopic features in selected cases | A risk-free or necessary routine test for everyone |

When further testing or a biopsy may be considered
Blood-based scores and elastography can help identify or lower concern about advanced fibrosis, but they do not reveal every microscopic feature of steatohepatitis. A biopsy can assess liver tissue and is still needed for a definite diagnosis of steatohepatitis; it may also help investigate alternative causes. It is invasive and is not required for most people’s routine management. Clinicians may consider it when advanced disease is suspected or other results leave important questions unanswered. Questions to take to your clinician
- Which finding does this test measure: possible liver-cell injury, visible fat or fibrosis risk?
- Was my FIB-4 calculated using the correct age, AST, ALT and platelet count, and could age or recent illness affect its interpretation?
- Does my imaging report include elastography, or is it a routine scan describing visible fat?
- How do my metabolic risk factors, alcohol exposure, medicines or other possible liver conditions affect the interpretation?
- Would repeat blood work, elastography, another test or specialist review change what happens next, and on what timeline?
Bring the full report and relevant health history to the discussion. The lab’s reference ranges, the imaging method, symptoms and other clinical factors all matter; a report phrase or isolated value cannot determine an individual’s diagnosis.
