Crashes, No Sound, or Screen Glitches?
Random freezes, missing sound and display glitches usually trace back to one bad driver. Find and replace yours safely.Free scan · under a minuteWindows Errors? Fix Them Before They Spread
Repair common Windows errors and clear accumulated junk for a smoother, more stable PC - no reinstall needed.Free scan · no reinstallStart with one failed sign-in event and find the stage where the flow stops: identity-provider authentication, SAML or OIDC exchange, MFA, or application-side validation. The event’s timestamp, error code, failure reason, and correlation or request ID help distinguish those cases and give support teams evidence to work with.
Start with a specific failed sign-in
Reproduce one affected login if possible, then record the event details before changing configuration. A precise event is more useful than a general report that “SSO is broken.”

- Failure reason and additional details shown in the sign-in record
- Where the user saw the error: at the identity provider, during an MFA prompt, or after returning to the application
For Microsoft Entra sign-ins
In Microsoft Entra, filter Sign-in logs by the user or application and select failed sign-ins. Microsoft documents the Reports Reader role as the least-privileged role for accessing activity logs; access requirements can vary with tenant configuration. If the event details do not explain the failure, Sign-in diagnostics can investigate a specific event using the user or application together with its correlation or request ID and time.
Microsoft’s diagnostics cover scenarios including incorrect credentials, MFA proof-up (first-time setup), per-user MFA, and other sign-in problems. Follow the diagnosis for that event rather than assuming every failure with an MFA prompt has the same cause.

Locate the failing stage before changing settings
| What the user sees | Likely failure location | Next evidence to inspect |
|---|---|---|
| An error on the identity-provider sign-in page | The identity provider may not recognize or accept the incoming request, or authentication may have failed before a response was issued. | Event details and the SAML request or OIDC authorization parameters. |
| Authentication succeeds, then the application shows an error | The identity provider may have issued a response that the application rejected. | For SAML, the response, NameID, claims, and signature expectations. For OIDC, the token-validation error, claims, and provider metadata. |
| An MFA prompt repeats, is abandoned, or blocks sign-in | The prompt may not have been completed, first-time setup may be incomplete, or an MFA policy may have interrupted the flow. | The event’s failure reason and additional details, plus sign-in diagnostics where available. |
| An OIDC callback reports a protocol or redirect mismatch | The redirect URI in the request may not match one registered for the application. | The actual redirect URI in the request and the application’s registered redirect URIs. |
These symptoms narrow the investigation; they do not prove a root cause by themselves. In particular, an application error after successful authentication points to a different stage than an identity-provider error before issuance.
Check SAML requests and responses
Use the identity platform’s SAML test or diagnostic experience, or another approved inspection method, to capture the request and response. Microsoft’s SAML troubleshooting guidance recommends request and response capture to obtain actionable evidence.
If the identity provider rejects the request
Compare the incoming request with the identity-provider and service-provider configuration. Check that:

- The request’s Destination matches the identity provider’s SAML single sign-on service URL.
- The request’s Issuer matches the configured application identifier.
- The AssertionConsumerServiceURL points to the expected application endpoint.
For Microsoft Entra integrations, AADSTS75005 means the SAML request is not a supported or valid SAML protocol message. Microsoft identifies missing required fields and request encoding as possible causes. Capture the request and verify compatibility with the application vendor before changing the integration.
If the application rejects the response
Compare what the identity provider issued with the service provider’s exact requirements. Inspect the NameID value and format, the attributes or claims the application expects, and the signing certificate or signature method. A missing attribute, an identity value the application cannot map to a user, or a signature-method mismatch can cause rejection. Confirm the expected values with the service-provider vendor rather than changing claims or algorithms by guesswork.
For Microsoft Entra, also review the application’s Identifier, Reply URL, metadata XML or certificate, and claims mapping. Entra’s SAML signing certificate settings provide a way to download metadata as an XML file. These are Entra-specific configuration details; labels and navigation can change.

Check OIDC requests and token validation
Verify the authorization request and callback
Compare the request with the application registration. Check the client or application ID, expected tenant or authority, the openid scope, and the redirect URI. The redirect URI in the request must exactly match a URI registered for the application; account for URL encoding when comparing the request to the registration.

Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Clear out junk files and repair common Windows errorsFree Scan →Microsoft Entra documents AADSTS50011 for a redirect mismatch, with the message “The redirect URI specified in the request does not match.” Compare the URI actually sent by the application—not just the one expected in its local settings—with the registered value.
If the application receives a token but rejects it
Start with the application’s token-validation error. Check the token signature and claims against the requirements of that client and application architecture. Use the identity provider’s OpenID configuration document and signing-key metadata to obtain the relevant validation information. An application that relies on a manually pinned, obsolete signing key can fail after key rotation; use the platform’s supported metadata and key-refresh approach rather than treating a static key as permanent.

Do not apply one client type’s validation rules indiscriminately to another. Follow the platform and application guidance for the specific architecture.
If the response concerns consent
Check whether the application requested a resource or permission for which the required user or administrator consent has not been granted. Microsoft’s consent troubleshooting guidance covers OIDC and OAuth2 consent problems; a similar-looking SAML error can have a different configuration cause, so identify the protocol and event before applying a fix.

Check whether MFA interrupted the flow
Use the sign-in event’s failure reason and additional details instead of presuming the second factor itself is faulty. Determine whether the user completed the prompt, whether first-time MFA setup was interrupted, and whether a policy requirement caused the interruption.

In Microsoft Entra, error 500121 is documented for an incomplete MFA prompt. Sign-in diagnostics can also identify MFA proof-up problems and whether MFA was required by Conditional Access or per-user settings. Follow the event-specific diagnostic and the organization’s policy design; an MFA requirement may be intentional even when the user did not expect the prompt.
Escalate with useful, safe evidence
If the event and protocol evidence do not isolate the cause, give the identity-provider or application support team the timestamp, correlation or request ID, exact error, and relevant configuration values. Include a sanitized SAML request or response, or token details appropriate to the issue, through the vendor’s secure support channel. Never attach credentials, client secrets, or live bearer tokens to a ticket. Microsoft specifically identifies the correlation ID and timestamp as useful when opening a support case.
Microsoft Entra error codes and console steps are not universal SSO standards. For another identity provider, use its equivalent sign-in logs, diagnostics, protocol capture, application-registration settings, and vendor documentation; verify application-side expectations with that application’s provider.
