There are two different questions behind “Is my VNS working?”: whether the device is delivering its programmed stimulation, and whether treatment is improving the condition it was prescribed for. A sensation such as hoarseness cannot reliably answer either question. Track the outcomes that matter for your condition and ask your treating team to check the device and its settings.
Can you tell from how VNS feels?
Not reliably. Many people do not feel stimulation at all. Some notice a hoarse voice when stimulation activates, according to the Epilepsy Foundation. That sensation may coincide with stimulation, but it does not prove that the device is functioning as intended or that treatment is helping.

There is no universal at-home check established across VNS devices. Do not try to diagnose operation by using a magnet, changing settings, or attempting to test or repair the system yourself. Ask the team that manages your device what checks apply to your specific model.

If VNS is being used for epilepsy
Keep a record of seizure frequency, severity, and recovery, then review the pattern with your epilepsy team. The Epilepsy Foundation describes possible reductions in seizure frequency and severity and shorter recovery, but individual outcomes vary. A seizure and symptom journal can help organize observations for appointments; it cannot verify device operation or prove treatment efficacy.

The American Academy of Neurology’s 2013 guideline reviewed seizure-frequency reduction as an outcome and found evidence of possible benefit for some groups, not a guaranteed response for an individual. Across 13 Class III studies involving 470 children with partial or generalized epilepsy, 55% had a seizure reduction greater than 50%. Across four Class III studies involving 113 patients with Lennox-Gastaut syndrome, the same degree of reduction was reported in 55%. These pooled study results describe those populations, not an individual forecast. In two Class III studies, the guideline reported an approximately 7% increase in the proportion achieving at least 50% seizure-frequency reduction between one and five years after implantation. If VNS is being used for depression
Review changes in symptoms and day-to-day functioning with your clinician over time. An American Association of Neurological Surgeons patient page says studies leading to FDA approval for treatment-resistant depression did not show benefit in the first two or three months; after one year, 20–30% reported significant improvement. That describes the study context reported on the page, not a prediction for any one person or a universal timetable. Some patients improved later, while others did not improve or worsened. Discuss your own response and care plan with your clinician.

VNS response may take time and can build over months or years, but there is no fixed timeline or promise of benefit for an individual. Continue prescribed medication and other care unless your clinician changes the plan; for epilepsy, VNS is described as an add-on treatment.

Follow-up is how the treating team can assess the implanted generator and adjust its programming when appropriate. Mayo Clinic notes that activation takes place during a medical visit a few weeks after implantation, and that follow-up helps check whether the generator is working and has not moved. The schedule and checks for you should come from your own care team.

Contact that team if symptoms have not improved, have worsened, or you are concerned that the implanted device has moved or is not operating. “VNS” can refer to different approaches. An implanted system is placed surgically; transcutaneous VNS (tVNS) delivers stimulation through the skin with a portable device. The National Institute of Mental Health says findings for depression are mixed and research on tVNS for depression, PTSD, and other mental disorders continues. Do not assume evidence, approved uses, or monitoring practices for one type apply to another. Compare the specific device, condition, evidence, clinician oversight, and outcome used to judge response.
