In a case reported by TonyDzi of PaloAlto Ai Research Lab, a routine ran for 19 days, but its progress journal had lost updates and the agent kept selecting already-processed items. The lesson is practical: count verified outputs, not invocations.
TonyDzi described the incident in a DEV Community post published October 5, 2026. The routine selected an item from a queue, processed it, recorded progress in a journal, and exited. The author says they later found 453 runs but only 39 surviving progress lines—an 11-to-1 loss rate—and that the routine spent 19 days re-processing items. Why did the agent keep doing the same work?
The author identified two failures that reinforced each other:
- Progress state was stored in a synced folder. The routine ran across machines, and competing writes could be reconciled by the sync process in a way that lost an append. Without a durable record of completed items, earlier work could look unfinished.
- Success meant that a run exited. Each invocation recorded success at exit, so the counter reflected that the routine had run—not that its output had landed or that the queue had advanced.
With stale progress, previously processed items could be selected again. The author says the driver was about 300 lines of Python and made no LLM calls; the workload was a larger task split into queued pieces and scheduled several times a day. This was a state and verification failure, not evidence that an AI model itself caused the repetition.

What should a progress monitor verify?
Separate three events that are easy to conflate: the scheduler fired, the worker exited, and the intended output was successfully produced. Only the last establishes task completion. As the post puts it, “The task fired” and “the work landed” are different claims.
- Check the artifact. Verify completion from the output itself—for example, by checking its timestamp or checksum with a reader other than the process that wrote it. A self-authored success message or exit code alone is not proof.
- Track queue movement. Record which item was selected and whether it reached a verified completed state. Make it possible to distinguish a new result from another attempt at the same item.
- Alert on no progress. Repeated runs without new output should be treated as a stall, not a healthy success streak. The post gives five consecutive runs as an example threshold, not a universal standard.
- Stop at completion. When the queue is empty, the scheduler should stop and report that there is no work left.
How can you make routine state safer?
Keep the writable record local and single-writer
Store writable state on local disk, outside synced or shared-transit paths, and arrange for one writer. If another machine needs visibility, send it a read-only copy rather than allowing multiple machines to update the same journal.

Bound retries and expose repeated failures
Set a retry limit for each item and report items that exceed it instead of letting them cycle indefinitely. The post uses three failures as an example ejection threshold; teams should choose a limit appropriate to their workload rather than treating that number as a standard.
Make no-progress behavior explicit
Define what counts as progress—such as a verified output or a completed queue item—and what the scheduler does when that signal does not change. A routine that runs on schedule but produces no new result should stop or escalate according to that policy.

When is a custom scheduled routine the wrong fit?
- If the task fits in one sitting, adding a queue, driver, and scheduler may add unnecessary moving parts.
- If each piece requires human judgment before the next can begin, an unattended routine is not a substitute for that review.
- If an existing job queue already provides acknowledgments and dead-letter handling, duplicating those functions in a custom driver can create a second, conflicting source of state.
Before adding a routine, check whether its state has one writer and a durable location, whether completion is verified from output, whether stalled work can be detected and stopped, and whether a queue system already handles acknowledgments and failed items. A useful monitoring question from the post is: “if it had done nothing for the last two weeks, would anything you monitor actually turn red?”
