How to Tell When an AI Chatbot Is Making Things Up

AI chatbots can invent facts, quotes and citations while sounding certain. This practical guide shows how to break answers into claims, verify sources, raise the checking standard for high-stakes topics and recognize why detector scores are not fact-checks.

You usually cannot spot a fabricated chatbot answer by its tone. A model can sound certain, provide polished reasoning and even invent quotations or citations while being wrong. Treat every answer as a collection of claims: separate those claims, inspect the cited evidence, check dates and authority, and compare important points with independent reliable sources. The more current, obscure, ambiguous or consequential the claim, the more verification it needs.

What “making things up” means

NIST calls this behavior confabulation: a generative-AI system confidently presents erroneous or false content. “Hallucination” and “fabrication” are common informal terms for the same family of failures. The problem is broader than a wrong isolated fact. A response can diverge from the prompt, contradict itself, invent logic, attribute words to the wrong person or attach a nonexistent citation to a plausible-looking paragraph.

The output is generated text, not a guarantee that each sentence was retrieved from a verified database. A fluent answer can combine a real name with a false date, blend two studies, or fill an information gap with a likely-sounding detail. The model may not signal which parts are remembered, inferred or uncertain unless the system and prompt encourage it to do so.

Why confidence and fluency are unreliable signals

OpenAI’s Help Center guidance says ChatGPT can sound confident even when it is wrong, including when it invents quotes, studies, citations, dates and facts. Grammar, detail and a decisive tone show that the model produced a coherent response; they do not show that the underlying claims are true.

A confident answer to a vague question is particularly risky. If a prompt leaves out a jurisdiction, date, product version or definition, the model may silently choose one and continue as if the assumption were established. A long explanation can make that hidden assumption harder to notice.

No single visual clue proves fabrication. The useful question is not “Does this sound like AI?” but “What evidence establishes each important claim?”

  • Unusually precise unsupported detail: a verbatim quotation, page number, statistic, legal citation, article title or person-specific fact that has no traceable primary source.
  • A citation that does not support the sentence: the linked page discusses a related topic but lacks the quoted wording, number or conclusion.
  • Out-of-date evidence for a current claim: an old policy, price, software version, regulation or event is presented as current without a date.
  • Certainty despite ambiguity: the question has several plausible meanings, but the answer never states which interpretation it used or asks for clarification.
  • Internal contradictions: dates, names, totals or recommendations change within the same response.
  • Drift from the supplied facts: the answer adds assumptions or conclusions that were not in the prompt and are not supported by evidence.
  • Over-specific causal stories: the response explains exactly why something happened when the available source only reports that it happened.
  • Perfectly convenient evidence: every source appears to confirm the requested conclusion, while contrary evidence or uncertainty is never mentioned.

Use these signals as a triage list, not as a detector. A concise answer can be wrong, and a detailed answer can be correct; only evidence can resolve the claim.

A practical workflow for checking an AI answer

1. Define the decision and its risk

First decide what you might do with the answer. A casual explanation of a familiar concept needs less checking than advice that could affect health, safety, money, employment, legal rights or security. NIST identifies consequential settings, including health-related decisions, as contexts where confabulation risk matters especially. A higher consequence means you should require stronger, more authoritative evidence and human review.

2. Break the response into atomic claims

Do not fact-check a whole paragraph as if it were one statement. Mark each name, date, number, quotation, definition, causal explanation and recommendation. For example, a sentence saying that a regulation took effect on a certain date and applies to a particular group contains at least three claims. Checking only the regulation’s existence leaves the date and scope unverified.

3. Classify each claim

Label claims as factual, interpretive, predictive or advisory. A factual claim can be checked against a document or dataset. An interpretation may be reasonable but should be identified as analysis. A prediction is uncertain by nature. Advice should be evaluated against the goal, constraints and consequences rather than accepted merely because it is presented as a fact.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

4. Open every important citation

Do not stop at a citation list or a formatted footnote. Open the original page or document and confirm:

  • the exact title and the author, institution or publisher;
  • the publication date and, for a changing page, the latest update date;
  • the passage, table or data point relevant to the attached sentence;
  • whether the source actually supports the same wording, scope and level of certainty;
  • whether the publisher has appropriate authority for this type of claim.

A citation can be real yet still be misleading if it supports a narrower statement, discusses a different population or is too old for the question. OpenAI cautions that search results and citations can be incomplete, outdated or incorrect, so a formal-looking reference is only a starting point.

5. Prefer the primary, authoritative source

For a regulation, check the regulator or official legal text. For a court decision, use the court’s document. For a company policy, use the current policy page. For research, inspect the paper or the named researcher’s institution rather than relying on a blog summarizing it. A second article that repeats the chatbot’s wording is not independent confirmation; it may have copied the same error.

6. Compare consequential claims independently

Find another reputable source that did not simply reproduce the first one. Independent agreement is most useful when the sources use different evidence or methods. If sources disagree, record the disagreement and determine whether one is newer, more authoritative or answering a different question. Do not force a single definitive answer when the evidence is genuinely mixed.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

7. Check the date and version

For changing information, verify against the current authoritative source and write down the date you checked. A model’s stored knowledge may not include later events unless it uses a search tool, and search output still needs inspection. Software behavior, prices, policies, laws and public-office holders can change after the model’s training data was collected.

8. Recalculate numbers and inspect quotations

Recompute totals, percentages, unit conversions and dates yourself or with a trusted source. For a quotation, search the original speech, transcript, paper or recording and confirm the wording, speaker and context. If you cannot locate a specific quote, treat it as unverified rather than “probably accurate.”

9. Record what is established and what is not

Keep a short audit trail: claim, source, supporting passage, date checked and remaining uncertainty. This prevents a verified detail from lending false credibility to neighboring claims that were never checked.

How to evaluate citations that look convincing

Fabricated citations often use realistic journal names, plausible URLs, genuine authors or a real paper paired with a conclusion it never made. Verify the source independently instead of copying the citation into your notes as proof.

  1. Search for the title or document in the publisher’s own archive, an official repository or a library catalogue.
  2. Confirm that the author, institution, publication date and document type match the chatbot’s description.
  3. Use the document’s search function to find the quoted phrase, number or technical term.
  4. Read enough surrounding context to determine whether the passage is being applied to the right population, time period and question.
  5. Check whether the source reports a result, proposes a hypothesis or merely discusses a possibility. Those are not interchangeable.

Google’s guidance for Gemini users similarly warns that a matching search result is not necessarily the source the model used. A search result can look related while failing to establish the exact claim. Treat the result as a route to evidence, not evidence by itself.

Raising the verification bar by claim type

Claim or situation Minimum sensible check When to escalate
Stable, familiar fact Check a reputable reference if the detail matters to your work. Escalate when the answer supplies an unusual number, quote or attribution.
Current event, policy or software behavior Use the current official source and note its publication or update date. Escalate when the model cannot identify a current source or the change affects a decision.
Technical, obscure or specialized claim Read the primary documentation, paper or standards text. Ask a qualified practitioner to review consequences and assumptions.
Medical, legal, financial or safety advice Use an official source or qualified professional; treat the chatbot as an aid to questions. Do not act until the responsible professional or authority confirms the advice.
Prediction or recommendation Separate evidence from the model’s inference and state the assumptions. Compare alternatives, downside risks and uncertainty before acting.

Ask for uncertainty without mistaking formatting for proof

You can improve an answer’s usefulness by asking the chatbot to:

  • separate sourced facts, calculations, assumptions and inferences;
  • mark statements it cannot verify;
  • provide a source for each material factual claim, not one citation for an entire paragraph;
  • state the date, jurisdiction, version or population to which a claim applies;
  • ask clarifying questions before answering an ambiguous prompt;
  • show the calculation behind a number and identify any missing inputs.

These requests expose assumptions and make checking easier, but they do not create independent evidence. Asking the same chatbot whether its previous statement is true is a second expression of the same system, not external validation. You still need to inspect the sources and compare important claims independently.

“I don’t know” can be the responsible answer

OpenAI’s September 5, 2025 research on uncertainty argues that accuracy-only evaluations can reward guessing over appropriate uncertainty. A system that declines to answer, asks for missing context or gives a qualified range may be behaving more responsibly than one that supplies a crisp but unsupported detail.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Look for useful uncertainty rather than vague hedging. A good answer identifies the unknown, explains why it matters and states what evidence would resolve it. For example, “The result depends on your jurisdiction; provide the country and state, then check the current regulator’s rule” is actionable. “It depends” with no explanation is not.

Factual truth is different from AI authorship

A fact-check asks whether the claims are true. An AI-authorship detector asks whether the writing resembles text produced by an AI system. These are separate tasks. A human-written passage can contain errors, and an AI-generated passage can accurately summarize a well-sourced document.

NIST’s first text-summarization pilot reported that three generators produced summaries capable of fooling every detector in that evaluation. That result is specific to the pilot; it is not a universal failure rate for every detector or model. It does demonstrate why a detector score cannot be used as a truth test.

Question Evidence that answers it Evidence that does not answer it
Is this claim true? Primary documents, reliable data, expert review and independent corroboration. An AI-detector percentage or a confident tone.
Was this text generated by AI? Provenance records, version history or other evidence about how the text was produced. Whether the citations are accurate or the prose sounds polished.

Worked checks for common failure patterns

A precise quotation

The chatbot supplies a memorable sentence attributed to a public figure. Search the speaker’s official transcript, speech archive or recording. If the words cannot be found, do not repeat them as a quote. You may describe the idea as an unverified attribution only if that distinction is important, but never present an invented quotation as verbatim.

A current rule or product behavior

The answer states that a policy, law or software feature “currently” works a certain way. Identify the relevant country, state, edition and version, then open the current official documentation. Check the page’s date and any exception clauses. If the chatbot did not establish those variables, ask for them before relying on the answer.

An ambiguous technical question

The user asks how to “secure” a system without specifying the threat model, operating system or access level. A responsible response should ask clarifying questions or state its assumptions. Verify commands against the vendor’s current documentation in a test environment before applying them to production.

A statistic with a causal explanation

The response gives a percentage and claims a particular reason for it. Locate the original dataset, confirm the denominator, time period and sampling method, then check whether the source actually tested causation. A reported correlation does not establish the chatbot’s explanation.

Common mistakes when fact-checking

  • Checking only one sentence: a paragraph may contain several independent claims.
  • Trusting a citation’s appearance: verify the passage, not just the title or URL.
  • Using a search snippet as proof: read the underlying document and its context.
  • Confirming with another chatbot: different wording is not independent evidence.
  • Ignoring dates: an accurate source can be obsolete for a changing question.
  • Relying on an authorship detector: it evaluates style resemblance, not factual accuracy.
  • Turning uncertainty into a binary verdict: record what is supported, disputed or unknown.
  • Applying a low-risk checking habit to a high-risk decision: medical, legal, financial and safety matters require qualified review.

A compact checklist before you rely on an answer

  1. What exact decision or statement am I evaluating?
  2. Which names, dates, numbers, quotations and recommendations need evidence?
  3. Did I open the original source for each material claim?
  4. Does the cited passage support the exact wording, scope and certainty?
  5. Is the source authoritative and current for this jurisdiction, version or population?
  6. Did I compare consequential or surprising claims with an independent source?
  7. Did I separate fact from inference, prediction and advice?
  8. Would being wrong create a health, legal, financial or safety risk?
  9. If important uncertainty remains, have I asked a qualified person or official authority?

The reliable habit is simple: do not infer truth from confidence. Treat chatbot output as a draft of claims, verify the claims against appropriate evidence, and preserve uncertainty wherever the evidence does not justify certainty.
