Start by asking the person how they want to be supported—and what they want kept private. Offer a specific, manageable kind of help, follow their lead about communication, and do not share their diagnosis, updates, photos, or messages without permission.
Ask what kind of support they want
People differ in whether they want to talk about ALS, receive practical help, get information, or have a conversation about something else. Ask rather than guessing. For example: “Would you like me to listen, help with something practical, or talk about a distraction?”

Make it easy to choose a communication channel, too: “Would you like me to check in by text, or would another way work better?” The ALS Association’s My ALS Journey Navigation Toolkit addresses communication changes and tools that can help people share thoughts, needs, and choices. Preferences may change; silence does not necessarily mean someone is unwilling or unable to communicate.

The ALS Association says its social platforms are public and asks users to get a person’s agreement before publicly sharing their diagnosis or medical status. Apply that same care to photos, tags, screenshots, fundraising links, health updates, and forwarded private messages. Ask privately before posting or passing anything along, and accept a no without debate.

Make practical offers specific and bounded
A broad “Let me know if you need anything” leaves the other person to work out what to ask for. The ALS Association recommends being specific and honest about the help you can provide, with examples such as an errand, a meal, or a school or activity drive. Online, offer one clear task or a choice between two manageable options:

- “Would groceries on Thursday or help with school pickup be useful?”
Offer only what you can actually do, and leave the decision with the person. If they decline, change the subject, or do not reply, do not keep pressing.
Keep checking in without requiring a response
The ALS Association advises letting a family know you will continue to be there—and following through. A brief message can offer connection without creating another obligation: “No need to reply. I’m thinking of you.” You might add when you plan to check in again, but there is no universal schedule; follow the person’s preference and avoid asking for updates every time you reach out.

Let them set the terms of the conversation
Do not turn every interaction into a request for a medical update, unsolicited treatment advice, or pressure to stay positive. Ask whether they want to talk about ALS, discuss a practical need, or focus elsewhere. Respectful support means staying available while allowing the person to decide what they share and when.
