How to Support Bone and Brain Health as You Age: An Evidence-Based Guide

A practical, evidence-based plan for aging well: combine aerobic, weight-bearing, strength and balance activity with calcium-rich foods, appropriate vitamin D, chronic-condition management and individualized bone screening.

The most useful approach to aging bones and brain is a coordinated one: move regularly, include weight-bearing, strengthening and balance work, obtain calcium and vitamin D mainly from food, control conditions that damage vascular and bone health, and ask a clinician when screening or exercise needs to be individualized. These habits can support strength, mobility, mood and some aspects of cognitive function, but no single exercise, food or supplement is proven to prevent dementia or guarantee freedom from fractures.

This is general U.S. guidance for older adults, not a personal treatment plan. Your age, sex, fracture history, medical conditions, medications and current fitness determine what is safe and how much you need.

Why bone and brain health belong in the same plan

Bones, muscles, balance and cognition interact in everyday life. Stronger legs and better balance make it easier to stay mobile and reduce situations in which a fall could cause a fracture. Physical activity can also improve strength, balance and mood, and may benefit some aspects of cognitive function, according to the National Institute on Aging (NIA). The evidence is encouraging, not a promise that exercise prevents Alzheimer’s disease or other dementias.

Bone loss is common with age. The NIH Office of Dietary Supplements (ODS) reports that bone mineral density falls by about 1% per year after menopause. ODS also cites estimates that about 30% of postmenopausal women in the United States and Europe have osteoporosis and that at least 40% of those affected experience a fragility fracture. Those are population figures, not a prediction for any one person.

A practical plan therefore has four parts:

  • regular aerobic movement plus weight-bearing activity;
  • strength training and balance practice;
  • adequate calcium, vitamin D and overall nutrition;
  • medical care that addresses chronic disease, fracture risk and safe exercise.

What can I do to keep my bones strong as I get older?

Use several kinds of activity rather than relying on one favorite exercise. Federal guidance cited by NIA uses at least 150 minutes of physical activity per week as a general adult benchmark. NIA presents that amount as broad guidance, not a personalized prescription. If you are inactive, begin with manageable sessions and build duration and frequency gradually.

Combine four training effects

Activity type Examples supported by NIA guidance What it contributes Important qualification
Moderate aerobic movement Walking, dancing or another activity that raises your breathing rate Cardiovascular fitness, endurance, mood and the ability to stay active Choose intensity and duration that fit your health and fitness; the 150-minute figure is a general benchmark.
Weight-bearing loading Walking, climbing stairs and dancing Places useful load through the skeleton while you move High-impact loading is not automatically safe for someone with osteoporosis or a fracture history.
Strength work Resistance exercises using body weight, household items or bands when appropriate Builds muscle force for transfers, carrying, stairs and joint support Learn technique and progression from a qualified professional if pain, weakness or a medical condition affects movement.
Balance practice Balance exercises selected for your ability, often with a stable support nearby Improves control needed for walking and changing direction Have a clinician or physical therapist adapt drills if you have fallen, feel unsteady or use an assistive device.

A simple way to schedule the week

  1. Spread aerobic sessions over several days instead of doing all activity at once.
  2. Add weight-bearing movement during some of those sessions, such as a walk that includes stairs if stairs are safe for you.
  3. Schedule strengthening on at least two nonconsecutive days, leaving recovery time for muscles and joints.
  4. Practice balance regularly in a secure area; hold a counter or sturdy rail when advised.
  5. Increase one variable at a time—minutes, frequency, resistance or complexity—so you can identify what causes soreness or fatigue.

Stop and seek medical advice for chest pain, fainting, severe shortness of breath, a new neurologic symptom or sudden severe bone or joint pain. Ordinary muscle effort is different from sharp pain or pain that worsens with each session.

What exercises are safe if I have osteoporosis?

Safety depends on bone density, the location and severity of osteoporosis, prior fractures, posture, pain, medications and other conditions. NIA states: “If you have osteoporosis, talk with your health care provider about physical activity that is safe for you and good for your bone health.” The recommendation appears in its guidance on exercising with chronic conditions, reviewed January 14, 2025.

Before changing your routine

  • Tell your clinician about vertebral, hip, wrist or other fragility fractures, even if they happened during an ordinary activity.
  • Ask whether you need a bone-density test or a referral to physical therapy for technique and fall-risk assessment.
  • Bring a list of medicines and supplements; some affect balance, bone turnover or exercise tolerance.
  • Ask which ranges of motion, impact levels and lifting loads to avoid or modify.

Do not assume that an exercise is safe merely because it is labeled “low impact.” A professional can show you how to keep your spine and joints aligned, use support and progress resistance without excessive bending, twisting or falling risk. A resistance band can be useful for home strengthening, but it does not replace weight-bearing or balance training and is not suitable for every person.

How much calcium and vitamin D do older adults need?

NIA lists the following general reference amounts. They describe total daily needs from food and supplements combined, not a recommendation to take a pill. Requirements can differ with medical conditions, medications and laboratory findings.

Group Calcium per day Vitamin D per day
Men ages 51–70 1,000 mg 15 mcg (600 IU)
Men age 71 and older 1,200 mg 20 mcg (800 IU)
Women age 51 and older 1,200 mg 15 mcg (600 IU) at ages 51–70; 20 mcg (800 IU) after age 70

NIA lists an adult upper limit of 100 mcg (4,000 IU) of vitamin D per day. That ceiling is not a target. Taking more can be harmful, particularly when kidney disease, high blood calcium, certain cancers or interacting medicines are present. Ask a clinician or pharmacist to interpret your personal need.

What foods have calcium if I do not eat dairy?

Food is the first way to close a nutritional gap. NIA lists milk and other dairy products, some tofu, dark-green leafy vegetables, soybeans, canned sardines and salmon with bones, and fortified foods as calcium sources. The ODS calcium fact sheet notes that lactose-free dairy retains calcium. People who avoid dairy can use fortified foods or, when a professional identifies a shortfall, a supplement.

Compare practical food choices

Choice Who may find it useful What to check
Dairy or lactose-free dairy People who tolerate milk products or need a convenient, familiar source Serving size and the calcium amount on the nutrition label; lactose-free products still provide calcium.
Fortified plant beverages or other fortified foods People avoiding dairy for dietary, cultural or lactose-related reasons Fortification varies by product. Read the label rather than assuming every plant beverage contains calcium.
Tofu, soybeans and leafy greens People building calcium into meals from plant foods Amounts vary by preparation and brand; some tofu is calcium-set and some is not.
Canned sardines or salmon with bones People who eat fish and want a food that naturally includes edible bones Choose products that fit your sodium, allergy and dietary needs.

Keep a short food record for several days if you are unsure whether you are meeting the target. Share it with a registered dietitian, clinician or pharmacist before starting a high-dose product. More calcium is not automatically better: the goal is to correct an identified gap, not to maximize intake.

Do I need calcium or vitamin D supplements?

Only if food and an identified medical need leave a gap. Calcium-plus-vitamin-D studies do not agree. ODS describes reviews that found improved bone mineral density or fewer fractures and other systematic reviews that found no meaningful fracture reduction.

One 2016 meta-analysis of eight randomized trials involving 30,970 adults older than 50 associated calcium and vitamin D supplementation with a 15% lower risk of total fractures and a 30% lower risk of hip fractures. Those are pooled study results, not a guarantee for an individual, and ODS also reports null findings from other analyses. Differences in baseline intake, adherence, dose, age, sex, living situation and study design can change results.

When professional advice is especially important

  • You cannot meet the food target because of an elimination diet, malabsorption or poor appetite.
  • You have osteoporosis, a prior fragility fracture or a bone-density result that changes treatment.
  • You have kidney disease, kidney stones, high blood calcium or a condition affecting vitamin-D metabolism.
  • You take thyroid medicine, certain antibiotics, iron or other products whose absorption can be altered by calcium.
  • You are considering doses near the vitamin-D upper limit of 4,000 IU (100 mcg) per day.

Read the Supplement Facts label for the amount of elemental calcium and vitamin D, and ask a pharmacist how to separate calcium from medicines when necessary. Do not use a supplement as a brain-health product. NIA states: “At this time, no vitamin or supplement is recommended for preventing Alzheimer’s or other forms of cognitive decline.” See NIA’s cognitive-health guidance.

Can exercise help memory as I age?

It may help some aspects of cognitive function while also improving mood, strength and balance. NIA describes cognitive health as the ability to think, learn and remember clearly, and brain health more broadly as including motor, emotional, tactile and sensory function. Its evidence includes observational studies and trials, but NIA says more research is needed to establish whether exercise prevents cognitive decline.

Use exercise for the benefits that are established for you—better endurance, muscle function, balance and mood—while treating any cognitive benefit as possible rather than promised. A varied routine also creates regular opportunities for social or mentally engaging activity if you choose to walk with others, attend a class or learn a new movement pattern. No game, supplement or single lifestyle change should be marketed as preventing Alzheimer’s disease.

Which chronic conditions should I manage?

NIA specifically highlights high blood pressure, diabetes, depression and high cholesterol as conditions to manage when supporting cognitive health. The same medical review can identify factors that increase fall or fracture risk. Keep regular appointments, take prescribed medicines as directed and report side effects such as dizziness, unusual sleepiness or weakness that could make activity unsafe.

Bring useful information to an appointment

  • A list of diagnoses, medicines, over-the-counter products and supplements.
  • Recent falls, near-falls, fractures or changes in walking confidence.
  • Home blood-pressure or glucose readings if your clinician has asked you to track them.
  • Your typical weekly activity, including what causes pain, breathlessness or fatigue.
  • A record of calcium-rich foods and fortified products you regularly eat.

Do not stop blood-pressure, diabetes, cholesterol or depression treatment on your own. Ask the prescriber whether timing, dose or an alternative could reduce a side effect without sacrificing disease control.

When should I ask about a bone-density test?

Discuss testing when you have risk factors for osteoporosis or a history of fracture. NIA’s osteoporosis information identifies bone-density testing as a clinical tool to evaluate bone strength. The appropriate timing depends on age, sex, menopause status, previous fracture, family history, medications and other risks; a clinician can apply the relevant U.S. screening guidance to you.

A test is not a substitute for movement or nutrition. Its value is that the result can change the conversation about fall prevention, exercise limits, medication and follow-up. Tell the testing service about a hip or spine replacement, recent contrast imaging or inability to lie still, because these details can affect scheduling or interpretation.

How to make a safe, realistic weekly plan

The following example shows how the activity types can fit together. It is a planning model, not an individualized prescription.

Day Example session Adjustment rule
Monday Comfortable-paced walk; include stairs only if your clinician says they are safe. Start with a duration you can repeat without next-day exhaustion.
Tuesday Strength session using controlled resistance exercises. Use light resistance first; stop for sharp pain and seek coaching if technique is uncertain.
Wednesday Walk or dance at a moderate, sustainable effort. Use a stable route or nearby support if balance is limited.
Thursday Balance practice plus gentle mobility recommended for your condition. Perform drills near a counter or rail and do not practice alone if you have a serious fall risk.
Friday Second strength session. Allow recovery between sessions and increase resistance only when form remains steady.
Saturday Longer walk, dancing or another weight-bearing activity you enjoy. Break the session into shorter bouts if continuous activity is difficult.
Sunday Easy movement or rest, depending on recovery. Use symptoms and professional advice to decide whether to repeat, shorten or skip activity.

Add the minutes from your aerobic sessions to see how close you are to the general 150-minute benchmark. If you have been sedentary, several short bouts can be a safer starting point than one long workout. Consistency matters more than a perfect calendar.

How to reduce fall and fracture risk during daily life

Exercise is only one part of prevention. Ask your healthcare team to review dizziness, vision or hearing concerns, foot problems, assistive-device fit and medicines that may impair alertness or blood pressure. Keep frequently used items within easy reach, improve lighting on stairs and pathways, and remove loose objects that catch your feet. These practical changes do not strengthen bone directly, but they reduce opportunities for a fall while strength and balance improve.

If you fall, seek urgent care for head injury, severe pain, inability to bear weight, deformity, confusion or loss of consciousness. Even when pain seems minor, tell your clinician about a fall if you have osteoporosis or take medicines that increase bleeding risk.

How to track progress without overpromising

Choose measures that reflect function rather than a promise of disease prevention:

  • minutes of activity completed each week;
  • strength exercises and resistance used with safe form;
  • confidence walking, climbing stairs or rising from a chair;
  • number of stumbles or falls;
  • calcium-rich servings and fortified foods in your usual diet;
  • appointments completed for blood pressure, diabetes, cholesterol, depression and bone assessment.

Review the record monthly with a clinician or physical therapist if you have osteoporosis, recurrent falls or a chronic condition. A plateau is a reason to adjust the plan, not to double the dose of a supplement or force painful exercise.
