Prevent double-booking where a reservation is committed, not with an availability check alone. Two requests can see the same time as free before either writes; a database constraint or a correctly implemented transaction must decide which request wins. The right rule depends on whether appointments occupy fixed slots, arbitrary time ranges, or shared capacity.
Why an availability check cannot reserve a time
Consider two requests for the same provider and time:

- Request A checks availability and sees the time is free.
- Request B checks before A writes and also sees it as free.
- Both try to create an appointment.
Without protection around the write, both may succeed. The read is useful for displaying options, but it is only a snapshot; the committed write is authoritative. Do not report success until the database transaction commits.
Define the rule before choosing the mechanism
Write down exactly what must not happen. For one provider who can serve only one person at a time, the rule may be “no two active appointments overlap.” Decide which states consume availability: a temporary hold, a confirmed appointment, or both. Also specify what cancellation does and whether the resource can serve more than one person concurrently.

For time ranges, a half-open interval such as [start, end) treats the start as included and the end as excluded. An appointment ending at 10:30 and another starting at 10:30 can therefore coexist without overlapping.
Choose enforcement for the appointment shape

Fixed slots: let uniqueness choose the winner
When each bookable slot has one canonical row, enforce uniqueness across the resource and slot key. If two application instances insert the same key, only one can commit; translate the expected constraint conflict into a response such as “That time was just taken,” then refresh availability and offer alternatives. A cached or earlier availability result is not a guarantee.

A key that includes only a start time cannot protect a schedule with different appointment lengths: a 10:00–10:45 booking and a 10:30–11:00 booking have different starts but overlap. PostgreSQL exclusion constraints can encode the no-overlap rule for ranges belonging to the same resource and selected active states. An independent healthcare appointment project illustrates this pattern; treat it as an example, not proof of production behavior. Other database systems may require a different enforcement design because range exclusion is not a universal SQL feature.

When locking or serializable isolation is appropriate
A row lock can serialize work that conflicts on a known row, such as a resource or inventory record. Locks remain until the transaction ends, so keep the transaction short and do not hold it open while a user completes a form. Establish a consistent lock order where multiple rows are involved; PostgreSQL notes that deadlocks can abort a transaction.
Serializable isolation can protect correctness when a rule depends on a wider read/check/write set. It does not mean every transaction will simply wait and succeed: applications must handle serialization failures and constraint conflicts as expected outcomes, then retry safely where appropriate. A locking protocol is only effective if every writer follows it; a database constraint remains a useful final integrity guard when the model permits one.

Model holds, cancellations, and capacity in the same rule
If a user needs time to complete a form or payment, persist a hold rather than relying on a visual countdown. The hold should consume availability under the same concurrency rule as a confirmed appointment, with an expiration that releases it safely. Make expiration cleanup idempotent so repeated processing cannot corrupt availability. The Universal Scheduling Protocol describes holds as temporary reservations and emphasizes matching rules to the capacity model; check the current specification version before implementation.

Cancellation should change the appointment state in a way that releases the resource according to the same invariant. For group appointments, shared rooms, or other capacity greater than one, a simple exclusive uniqueness rule may be too strict. Define the permitted capacity and ensure concurrent updates cannot push active bookings beyond it.

Make retries and side effects safe
A client may time out after the first request commits and retry without knowing the outcome. Give each booking request an idempotency key or equivalent stable identity so a replay returns the original result rather than creating another appointment.
Keep email, calendar, and payment work out of the critical reservation transaction where possible. A durable event or outbox pattern and idempotent consumers can connect a committed appointment to later work without making external service delays part of the slot reservation. This is an implementation option, not a mandatory architecture; choose it based on the system’s failure and delivery requirements.
