Why AI can return plausible but unsupported details
A polished answer is not proof that the document contains the information. Generative AI can produce statistically informed guesses rather than facts, and extraction can also be inaccurate, incomplete, or stripped of important context. UK government guidance recommends documenting the data sources and how a conclusion was generated: Generative AI Framework for HMG. The Information Commissioner’s Office also advises developers to assess and communicate the risks and impacts of generative AI inaccuracies: ICO guidance on generative AI.

The practical goal is therefore not to trust a confident response, but to make each extracted value checkable and give uncertainty a defined path through the workflow.

1. Define the fields and evidence rules
Specify the fields you need and their expected types—for example, a date, a party name, or a contract term. Tell the system to use only the supplied document for evidence. Require an explicit unknown value when information is absent, unreadable, or ambiguous rather than allowing a plausible completion. This is a workflow design recommendation, not a guarantee offered by any particular model or product.

2. Keep source locations alongside values
For each extracted field, retain the page number and, where available, a section, text span, or coordinates that a reviewer can inspect. Microsoft’s Content Understanding documentation describes source references and spans for checking extracted output against a PDF: Microsoft: Document elements in Content Understanding. A value without a usable reference is harder to verify and should not be treated as self-validating.
3. Verify support and surrounding context
Open the cited location in the original document and confirm it actually supports the extracted field. Check nearby text as well: a passage may qualify a number, identify an exception, or clarify which person or date it refers to. NIST distinguishes among faithfulness to the source, completeness of the response, and sufficiency of evidence; a workflow should consider all three rather than checking only whether a value appears somewhere in the file: NIST AI Risk Management Framework.

Microsoft Support puts the traceability principle plainly: “If a statement can’t be traced back to a source, treat it as unconfirmed until verified.” See Microsoft Support’s guidance on validating responses in Copilot.
4. Route uncertain or consequential fields to review
Show the reviewer the original passage next to the extracted value. Prioritize names, dates, amounts, quotations, citations, and other details where a mistake could cause harm. For high-impact uses, consider expert review. Human review is a safeguard, not a substitute for clear evidence: the reviewer still needs to judge whether the cited text supports the field and whether relevant context is missing. 5. Test the whole workflow on representative documents
Evaluate files that reflect the formats, layouts, languages, and image quality you actually handle. Record different failure types separately, including invented values, omissions, transcription errors, and lost context. AWS recommends identifying the veracity issues relevant to a use case and applying appropriate filters and verification: AWS guidance on accuracy in responsible AI. Keep monitoring after changes to the model, prompts, document mix, or extraction process.
Choose tools by how well they support verification
Compare document-extraction options against the checks your workflow needs. A feature that returns structured output is not enough if reviewers cannot trace values to the source or find uncertainty.

No directly applicable published statistic establishes a hallucination rate for document extraction or quantifies how much a particular safeguard reduces errors. Avoid treating a single score or an untested prompt as proof that a workflow is reliable.
