The right hydration advice depends on the kind of fast: some plans allow water, while Ramadan and other dry fasts restrict drinks during fasting hours, and medical instructions may prohibit all oral fluids. If your fast allows fluids, drink water regularly during the hours you can; if it does not, plan ahead and take dehydration symptoms seriously. Anyone with diabetes, a condition affecting fluid balance, or medicines that affect hydration should ask their healthcare team about fasting before starting.
First, check whether your fast allows fluids
Intermittent fasting does not always mean avoiding drinks. NIDDK says dehydration generally should not be an issue when a fasting plan does not restrict fluid intake; when fluids are restricted, dehydration becomes a more significant risk. Ramadan fasting commonly prohibits both food and drink from dawn to sunset. Medical preparation may impose a complete oral-fluid restriction, so follow the care team’s specific instructions rather than assuming water is allowed.

For a water-permitting fast, drink water as usual during the fasting period. For a dry fast, use the permitted hours to hydrate and do not drink during the restricted period unless your religious guidance or medical team advises otherwise. If you develop concerning symptoms, seek medical advice rather than trying to push through them.

Drink regularly, not all at once
For Ramadan or another fast with a defined drinking window, drink water regularly while fluids are permitted, including at suhoor and after iftar. NHS England recommends plenty of water during Ramadan, and Mayo Clinic advises plenty of liquids when not fasting. You can spread your drinking across the hours between sunset and dawn instead of treating one large drink as a requirement. There is no single volume established here that suits everyone; needs vary with the person, weather, activity, and health.

Choose drinks with the guidance in mind
NHS England lists sugar-free and decaffeinated drinks as options. Mayo Clinic advises limiting caffeinated or highly sugared beverages. These are source-specific recommendations, not a claim that any caffeine inevitably causes dehydration.
Adjust activity for heat and long fasts
Heat and exertion can make hydration harder to manage. Ramadan daily fasts may last about 10 hours in winter or more than 17 hours in summer, depending on geography and season, according to Mayo Clinic in 2023. Mayo Clinic pediatrician Dr. Nusheen Ameenuddin offered a rule of thumb for Ramadan: reduce usual exercise intensity or duration to about half to avoid pushing too hard. That is her practical suggestion, not a universal medical standard; adjust activity to your circumstances and follow clinical advice where relevant.

Signs of dehydration to watch for
Thirst may be an early signal. Also pay attention to dark yellow urine or urinating less than usual. NHS guidance identifies unusual tiredness or drowsiness and dizziness that persists when standing as warning signs warranting urgent advice.

What to do if illness causes fluid loss
If vomiting or diarrhea is causing fluid loss, small, frequent sips may be easier to tolerate. Water is not always enough to replace the salts and sugars lost in these illnesses: NHS advises asking a pharmacist whether an oral rehydration solution is suitable. It is a targeted option for fluid and salt losses, not a routine requirement for fasting hydration. If you cannot keep fluids down or symptoms are severe, seek urgent medical help.

Fasting can pose additional risks for people with diabetes, particularly with poor glucose control, fever, a recent cardiovascular event, or medicines such as diuretics and SGLT-2 inhibitors, which NIDDK discusses in its fasting guidance. Talk with your healthcare team before fasting so they can advise on your individual risks and medication plan. Do not stop, start, or change prescribed medicines based on general hydration advice.

People with other conditions or treatments that affect fluid balance should also ask their clinician whether fasting is appropriate and how to follow any fluid restrictions safely. If a clinician has instructed you to take nothing by mouth for a procedure, do not override that instruction; contact the care team if symptoms arise or you are unsure what is permitted.
