You decide when to share an ALS diagnosis, whom to tell, and how much to say. Before posting, choose your audience, remove details you would not want copied, and state what responses or follow-up you welcome. Treat every online post as potentially public: account and group settings can limit visibility, but they cannot guarantee confidentiality.
Decide whether, when, and with whom to share
There is no obligation to announce a diagnosis online. The ALS Association says, “You’re in control of when you tell people about your ALS diagnosis.” Emotional readiness can be one factor; some people may want to tell close family early for support, while others prefer to wait or tell people one at a time. The Association also notes that what you share may differ by relationship. Read the ALS Association’s guidance on sharing a diagnosis.

Decide what you want the post to do: let friends know why you may be less available, ask for practical help, share an update, or simply prevent others from hearing the news secondhand. You can tell a few people directly, use a limited-audience post, or make a public announcement. None is inherently the right choice, and none guarantees privacy.

Choose the details and boundaries before writing
You can name ALS without sharing every medical detail. The ALS Association says it is okay not to have all the answers or to say you are not ready to discuss something. Consider whether you want to include any of the following:

- The diagnosis, if you want to name it.
- Only the context people need to understand the update or offer the kind of support you want.
- Whether you welcome messages, practical help, or questions—and whether you prefer people to wait before contacting you.
- Topics you are not ready to discuss, such as prognosis or treatment details.
- Whether people may share or repost your announcement. You can ask them not to, while recognizing that an online request cannot prevent copying.
For example: “I’m sharing this so you know what’s happening. I’m not ready to discuss prognosis or treatment details. Messages of support are welcome; please ask before sharing this post.” Adjust the wording to sound like you; you do not have to explain or defend your boundaries.
Match the channel to the audience you want
A private message, limited-audience post, and public post differ in reach and control, not in whether they can be copied. Choose based on how many people you want to tell, how much follow-up you can manage, and whether you are comfortable with the post circulating beyond its intended audience.

| Direct message | Tell one person or a small number of people individually, with room for a more personal exchange. | A recipient may copy, forward, or discuss the message with others. |
| Limited-audience post or group | Reach a selected set of contacts or a community at once. | Membership and visibility controls do not make the post confidential; members may capture or reshare it. |
| Public post | Make an announcement intended for a broad audience. | It may be visible beyond your followers and can be copied or recirculated. |
For a limited-audience post, review the platform’s current audience selector and group membership before publishing. Controls and labels can change, so check the actual audience shown for that post rather than relying on an old setting or assuming that “closed” means confidential.
Review what else may reveal health information
Privacy exposure is not limited to the words in an announcement. The FTC explains that browsing, location, and purchase information can convey health information. FTC and HHS have also warned that tracking technologies embedded in healthcare websites and apps can disclose sensitive information to third parties. FTC guidance on health information and mobile apps and the FTC and HHS warning on online tracking technologies describe these broader risks.

Know what HIPAA does—and does not—protect
Do not rely on HIPAA to protect a post you choose to publish. The Office of the National Coordinator for Health Information Technology explains that HIPAA applies to covered organizations and their handling of health information; it does not protect information a person posts about themselves online. Its consumer guidance says, “Never post anything online that you don’t want made public.” Read ONC’s HIPAA guidance for consumers (last updated April 1, 2026).

This is different from a provider’s legal responsibilities for records it holds. A person’s voluntary online disclosure is not made confidential by those provider obligations. The ALS Association’s own social-media pages are public, and its terms caution against posting sensitive personal information; that policy describes its pages and should not be assumed to define every platform or group. See the ALS Association’s social-media terms (updated January 2025).

Ask permission before someone else shares your diagnosis
If you are a relative or friend, ask the person with ALS before posting about their diagnosis or medical status. The ALS Association explicitly asks people to get the person’s agreement before sharing that information publicly. Let them decide the timing, audience, wording, and whether they want the news shared at all. The Association’s social-media terms explain its permission rule.

Plan separately before telling an employer online
A public post can reach an employer or coworkers even if they are not the intended audience. The ALS Association advises talking with a doctor or trusted person and understanding your plan and rights before bringing a diagnosis to an employer. Employment protections and obligations depend on the circumstances and jurisdiction; this general guidance is not an individual legal assessment. For a work-related disclosure decision, seek qualified local advice before sharing.
