How to Set Up Human Approval for High-Risk AI Agent Actions

A practical guide to gating high-risk AI agent actions before execution, with informed human review, auditability, failure handling, and legal context.

To require approval before an AI agent takes a risky action, intercept the action before its connected tool executes, show an authorized reviewer exactly what will happen, and let that reviewer approve, reject, revise, or stop it. Define which actions need a gate, keep the gate outside the agent’s ability to bypass, and plan for timeouts and service failures. Approval is a risk-control pattern—not a reason to pause every agent action.

Which AI agent actions should require approval?

Classify actions by their possible harm, reversibility, affected rights or assets, and operating context. An action that is routine in one setting may be high risk in another: changing a test file differs from changing a production database, and drafting a message differs from sending it to customers.

Start by inventorying every action the agent can take through connected tools. Record which system, account, data, or people each action can affect; whether it communicates outside the organization; and whether its effects can be reversed. The Singapore government’s agentic AI security addendum points to business transactions, database, table or file changes, and code execution with elevated privileges as examples that may warrant controls. These are useful prompts, not an exhaustive legal list.

Set a policy that distinguishes permitted, approval-required, and prohibited actions. Consider potential effects on health, safety, fundamental rights, finances, sensitive data, and service availability. A practical starting point is to allow low-impact, reversible actions under constrained rules; hold high-impact or hard-to-reverse actions for review; and block actions outside the agent’s authorized scope. Adjust the thresholds for the actual deployment rather than treating an action label as a universal risk rating.

How to put an approval gate in the agent workflow

1. Intercept the action before execution

Place the control at the boundary between the agent and the tool or system it would change. When a protected call is proposed, hold it and send it to the approval process instead of executing it. The reviewer must decide before the operation can affect a person, account, dataset, or external system; reviewing it afterward cannot prevent its initial consequences.

Keep enforcement outside the agent’s discretion. The agent should not be able to remove the hold, substitute a materially different action after approval, or execute the same operation through another unprotected route. The EU AI Act describes, where appropriate, operational constraints that the system itself cannot override; the specific workflow architecture is an implementation choice, not a prescribed product.

2. Give the reviewer enough context to decide

Present the exact proposed operation and the information needed to understand its consequences. A useful approval view includes:

Yubico - Security Key NFC - Basic Compatibility - Multi-Factor Authentication (MFA) Key, Connect via USB-A or NFC, FIDO Certified
  • POWERFUL SECURITY KEY: The Security Key NFC is the essential physical passkey for protecting your digital life from phishing attacks. It ensures only you can access your accounts.
  • WORKS WITH 1000+ ACCOUNTS: Compatible with Google, Microsoft, and Apple. A single Security Key NFC secures 100 of your favorite accounts, including email, password managers, and more.

  • The operation, including relevant parameters or proposed changes.
  • The affected system, account, file, data, or recipients.
  • The likely consequence and whether the action can be undone.
  • Relevant context or evidence from the agent, along with the applicable policy or risk category.

Offer distinct controls to approve, reject, revise, or stop. If a reviewer revises the proposal, treat the changed operation as a new proposal and require approval for that version. Do not imply that a single confirmation is meaningful oversight if the reviewer cannot understand what will happen.

3. Assign authority and keep review workable

Specify who may approve each risk tier, how a decision is escalated, and who responds when a time-sensitive action is held. Reviewers need appropriate competence, training, authority, and capacity—not merely access to an approval button. Train them to recognize system limitations and automation bias, and give them a safe way to intervene or halt the operation.

Keep the queue manageable by gating actions that matter rather than every low-impact step. Batch review only when each action and its consequences remain independently clear. Overloading reviewers or manipulating them into approval undermines the control; Singapore’s guidance expressly flags both risks.

4. Record decisions and verify the gate

As an operational practice, retain the proposed action, relevant context, risk classification, reviewer identity, decision and time, plus the eventual execution outcome. Review patterns such as rejection, revision, escalation, and timeout to find weak policy thresholds or confusing approval screens. Test that a rejected or revised action cannot execute in its original form. These records and checks support accountability; the cited guidance does not prescribe one universal event schema or metric set.

5. Define safe behavior when approval fails

Decide in advance what happens if the approval service is unavailable, a reviewer does not respond, or the pending action changes. For high-risk actions, a sensible default is to fail closed—do not execute—or enter another explicitly defined safe state. Provide a way for a human to halt the operation. NIST calls for contingency processes for high-risk third-party AI failures; choosing a fail-closed default is a design recommendation, not a quoted requirement.

What standards and laws say about human oversight

EU AI Act: duties apply to covered high-risk AI systems

Article 14 of Regulation (EU) 2024/1689 addresses high-risk AI systems within the Act’s scope; it does not make every consequential agent action a legally defined high-risk AI system. The consolidated EUR-Lex text dated 27 July 2026 says oversight measures must be proportionate to risk, autonomy, and context of use. It describes oversight capabilities such as understanding system limits, monitoring for anomalies, accounting for automation bias, interpreting outputs, disregarding or reversing outputs, and intervening or stopping the system safely. Measures may be built in by providers, implemented by deployers, or both. Recital 73 discusses competent, trained, authorized overseers and, where appropriate, system constraints that the system cannot override. See the consolidated regulation.

The European Commission’s AI Act overview gives examples of high-risk use cases, including certain critical infrastructure, education, employment, essential private and public services, and biometric systems. Whether a particular system qualifies depends on the Act’s scope and conditions. Deployers also have oversight and monitoring responsibilities for covered high-risk systems. Check current legal text and Commission guidance when assessing a real deployment.

NIST AI RMF: voluntary lifecycle risk management

NIST AI RMF 1.0 is voluntary guidance, released on 26 January 2023, rather than a legal mandate or a ready-made agent approval architecture. Its functions—Govern, Map, Measure, and Manage—frame risk management as continuous across the system lifecycle, with governance as a cross-cutting activity. The Core supports documenting human-AI roles and responsibilities, training personnel and partners, ongoing review, and contingency planning for high-risk third-party AI failures. NIST says the framework is being revised. See the AI RMF page and AI RMF Core.

Singapore guidance: agent-specific security recommendations

The Singapore government’s agentic AI security addendum recommends human approval for high-risk or irreversible actions. It also discusses defining agent boundaries and human roles, validating high-risk transactions, confirming database, table, or file changes, and manually reviewing AI-generated code with elevated privileges. Its practical resource list names LangGraph interrupts and Amazon Bedrock Agents as implementation references; that mention is not an endorsement or confirmation of current feature details.

How to compare approval implementations

Evaluate an orchestration framework or internal workflow against the control you need, rather than assuming a named tool guarantees effective oversight. Compare:

  • Enforcement point: Does the gate run before tool execution, or only log or review actions afterward?
  • Scope: Can policy target individual actions, action classes, or defined escalation thresholds?
  • Authority: Can an authorized human reject, revise, or stop the operation independently of the agent?
  • Visibility: Does the approval surface show the proposed action and its material context?
  • Failure behavior: What happens on timeout, reviewer absence, or approval-service outage?
  • Audit and testing: Can decisions and outcomes be reviewed, and can bypasses or stale approvals be tested?
  • Integration burden: How much work is needed to cover each agent framework and connected system?

Official sources describe oversight objectives and security recommendations, not comparative product benchmarks. Confirm the current behavior of any implementation you choose against its own documentation and test it in your environment.
