If fasting is required, an early appointment can make the fasting period easier to schedule. Do not skip or change medication based on general advice.
Do you need to fast before this blood test?
It depends on the test. MedlinePlus distinguishes fasting glucose and oral glucose tolerance tests, which require fasting, from glucose challenge, random glucose, and HbA1c tests, which do not. Other tests may have different preparation requirements. As MedlinePlus puts it, “You should not fast if your provider doesn’t tell you to.” Check the instructions for the tests actually ordered rather than assuming every blood test requires fasting.

How long should you fast, and when should you book?
If your clinician or lab tells you to fast, follow the exact duration they give you. MedlinePlus describes 8 to 12 hours as the usual range and notes that these tests can often be scheduled early in the morning; that range is guidance, not a universal instruction for every test.
- When the appointment is made, check the test order and any patient instructions for fasting requirements.
- Ask the ordering clinician or laboratory how many hours to fast and whether any additional preparation applies.
- If fasting is required and an early appointment works for you, request one so much of the fasting period can fall overnight.
What can you have during a fast?
In its fasting guidance, MedlinePlus permits plain water. It advises avoiding other drinks, including juice, coffee, and soda, as well as gum, smoking, and exercise during the fast. Follow any additional directions given for your particular test.

Can you take medication before a fasting blood test?
There is no universal medication schedule for fasting blood tests. Ask the ordering clinician or lab whether to take each usual prescription or over-the-counter medicine and when. Mention vitamins and supplements, too, because medicines and supplements can affect some test results, including blood glucose.
If a medicine normally needs to be taken with food, do not skip it, move its timing, or take it without food on the basis of generic advice. Contact the clinician or lab before the test to get instructions that account for both the medicine and the test.

If you have diabetes
Tell the clinician or lab that you have diabetes and ask specifically how to coordinate the test with your usual meal and medicines. MedlinePlus notes that some medicines affect glucose and that not eating enough after diabetes medicine can contribute to low blood glucose. The cited patient guidance does not provide an individualized dose-adjustment plan, so get one from your care team rather than changing a dose yourself.

What if you accidentally eat or drink before the test?
If fasting was required and you ate or drank something besides water, tell the lab or ordering clinician what happened. They can decide whether the test can proceed or should be rescheduled. Do not conceal the lapse or assume the result will still be valid.
