A low-stakes clinical simulation debrief should be non-punitive, not unplanned. Before the exercise, decide what the discussion needs to help learners notice or practice; after it, guide a structured conversation that examines key decisions, teamwork, and relevant system conditions. INACSL’s standards, revised in 2025, call for every simulation-based educational activity to include a planned debriefing process, which may involve feedback, debriefing, guided reflection, or a combination. The process should fit the intended outcomes and support reflection while maintaining psychological safety and confidentiality.

What makes a debrief low-stakes?

“Low-stakes” describes the learning and evaluation context: the simulation is not being used as a punitive test or as a high-consequence judgment of a participant. It does not mean the conversation is casual, unstructured, or free of useful correction. Learners still need a clear purpose, a facilitator who can guide the discussion, and a connection between the scenario and the points they are asked to consider.

Explain what the exercise is for and how performance will be used. If it is formative, say so plainly. If any observations will be documented, shared, or used for another purpose under local policy, explain that accurately rather than promising more confidentiality than the institution can provide. Psychological safety depends in part on learners understanding the boundaries of the activity and being treated respectfully within them.

The debrief can examine individual reasoning, team interaction, and systems factors. These are different lenses, not competing explanations. A learner’s decision may be worth exploring while also asking what information, role expectations, communication patterns, or environmental conditions shaped the options available at the time. Avoid reducing a complex event to a verdict about someone’s character.

Plan the conversation before the simulation

Choose the intended outcomes

Start with a small number of learning or evaluation outcomes that are genuinely relevant to the scenario. For each one, decide what learners should be able to recognize, explain, or consider doing. Then identify the scenario moments that could make those outcomes visible. This keeps the discussion focused without requiring the facilitator to follow a rigid script.

For example, if an objective concerns escalation of a changing patient condition, prepare to explore what cues participants noticed, what they understood those cues to mean, and how the team communicated about them. Do not assume that a missed action has a single cause; the discussion should help uncover what learners perceived and what conditions influenced their response.

Prepare a short observation guide

Write down the objectives and a few neutral observations to help you remember where the scenario may offer useful learning. An observation should describe something that happened, rather than assign a motive or label. “The team did not state who would contact the next clinician” is more discussable than “the team was disorganized.” The guide is a prompt for inquiry, not a scorecard to read aloud.

  • List the scenario objectives in plain language.
  • Note a small number of moments that relate to those objectives.
  • Mark any safety-critical factual point that may need clarification.
  • Consider which team or system conditions could be relevant, not only individual actions.
  • Choose a structure or framework that suits the learners, objectives, and facilitator’s skill.

A structured approach can still be responsive. If learners surface an important issue that was not on the guide, decide whether it serves the objectives and the group’s learning needs before pursuing it. Conversely, do not let an engaging side discussion consume the time needed for the central learning points.

Orient participants and set expectations

Before the scenario, tell participants that a debrief will follow and describe its learning purpose. Explain the ground rules for respectful participation and confidentiality in line with local policy. Invite learners to reflect, ask questions, and discuss uncertainty without suggesting that everyone must feel or respond the same way. Preparation and briefing are part of prebriefing: they establish educational content and expectations before participants enter the activity.

Be specific about the evaluation context. If the activity is formative and not a punitive assessment, say what that means in practice. If there are limits to confidentiality or other institutional procedures, state those limits. A facilitator should not promise that nothing will be shared or recorded unless that is actually true under the applicable policy.

A practical run-of-show for the debrief

The sequence below is a usable starting point, not a universal script. Adapt the time and depth to the scenario, group size, learner experience, and objectives. A brief exercise may need a brief conversation; a complex scenario or a group that needs more reflection may call for additional time.

  1. Open the discussion. Invite a first reaction with a broad prompt such as, “What is on your mind after that scenario?” or “What moment stands out to you?” Allow a pause. Do not immediately ask a participant to defend a difficult decision.
  2. Identify moments worth examining. Ask what the group noticed and which parts of the scenario seemed important. Keep the conversation tied to the objectives, while remaining open to a relevant issue the learners identify.
  3. Explore reasoning and context. Ask what information was available at the time, how participants interpreted it, and how roles, communication, or team and system conditions affected the response. Prefer open questions before offering your own interpretation.
  4. Offer focused feedback when needed. Describe a specific observation, explain the concern or its relevance, and ask a genuine question about the learner’s reasoning. Listen to the answer before deciding what clarification or follow-up is useful.
  5. Connect discussion to practice. Ask learners what they would carry into a similar situation. Clarify safety-critical facts that remain misunderstood, and make room for a concise, evidence-based correction.
  6. Close deliberately. Invite a takeaway and a practical action from the group, summarize the key learning points, and end respectfully. If a participant raises a distress or support need, follow the institution’s local support and escalation procedures.

How to explore a difficult moment without turning it into blame

Use observation, concern, and inquiry

When an action raises a concern, make the discussion transparent: state what you observed, explain why it matters, and ask how the participant was making sense of the situation. This observation-plus-inquiry approach reflects the broad idea behind “Debriefing with Good Judgment,” a model described by Rudolph and colleagues as combining rigorous feedback with inquiry. It is not a formula for disguising criticism as a question; the facilitator should be candid about the concern and genuinely interested in the learner’s reasoning.

For example, a facilitator might say: “I noticed that the change in the patient’s condition was not escalated during that interval, and I’m concerned because it could affect how quickly the team responds. What cues were you attending to at that point?” This is an illustrative application, not a direct quotation from the model. It names an observable event, explains its significance, and leaves room to learn what the participant perceived.

Ask about the information available then

Hindsight can make a choice appear obvious after the scenario has ended. Bring the conversation back to the moment as it unfolded: What did the learner see or hear? What did they believe was happening? What else was competing for attention? What did they expect another team member to do? Questions like these help distinguish a knowledge gap from an information, coordination, role, or environment problem.

Do not interrogate one learner while the rest of the group becomes an audience for a public reprimand. Direct questions to the group when appropriate, invite more than one perspective, and make clear that exploring an action is not the same as judging the person. If an individual conversation is better suited to a sensitive issue, use local policy and appropriate support rather than forcing the matter into the group debrief.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Keep the person and the system in view

When relevant, ask what in the environment or team process made the desired action easier or harder. Consider role clarity, communication, information availability, and other scenario conditions. This does not erase individual responsibility or make every action acceptable. It helps the group understand the event accurately enough to identify what can be learned and what may need attention beyond an individual learner.

How to support psychological safety and confidentiality

Psychological safety is built through the way the activity is framed and facilitated; it is not achieved by avoiding hard questions. Learners should have a clear sense of the purpose, the ground rules, and the intended use of performance information. During the discussion, listen without ridicule, avoid personal labels, and make room for uncertainty and different interpretations. Do not assume a participant’s silence means agreement or that everyone experienced the scenario in the same way.

Confidentiality should be handled according to local policy. State what participants are expected to keep private and any applicable limits on what the facilitator or institution can keep confidential. Do not make an absolute promise if the activity’s documentation, reporting, or escalation rules do not permit one. A respectful conversation and an accurate explanation of boundaries are more useful than an assurance the facilitator cannot uphold.

  • Use neutral descriptions of actions and events rather than judgments about character.
  • Invite reflection without demanding that a learner disclose personal feelings.
  • Do not single out a participant for immediate defense after a difficult moment.
  • Give participants space to consider a question before moving on.
  • Follow institution-specific procedures if a support or escalation need arises.

Choose a structure that fits the learners and objectives

INACSL’s current debriefing-process standard emphasizes a planned, purposeful process, capable facilitation, reflection and analysis, psychological safety and confidentiality, and an appropriate structure. A structure gives the facilitator a route through the conversation; it should not prevent adaptation to learner needs. No single approach is established here as best for every scenario.

When selecting or adapting a framework, consider how well it fits the intended outcomes and audience, what facilitator skill or training it requires, how it balances structure with flexibility, how it handles safety and confidentiality, and whether it supports individual, team, or systems reflection. “Debriefing with Good Judgment” is one named approach for combining feedback with inquiry, but the cited paper does not establish comparative superiority over other approaches.

Facilitation is a capability, not merely a role on the schedule. A capable facilitator keeps the discussion purposeful, listens for learner reasoning, offers feedback when appropriate, and responds to the group without abandoning the objectives. INACSL lists an online Cornerstones of Best Practice: Debriefing module covering its standard and foundational principles as an optional professional-development resource.

Common problems and how to respond

The discussion becomes an unstructured recap

If participants are recounting the entire scenario without examining it, return to a key moment and ask what they noticed, how they interpreted it, or how it connects to an objective. A debrief is not just a replay of events. The goal is to help learners reflect and analyze, not to narrate every action in order.

The facilitator talks more than the learners

If you are supplying explanations before hearing learner perspectives, pause and ask an open question. Learner reasoning is part of what the conversation needs to surface. Offer a correction when necessary, but do not let a lecture replace inquiry into what happened and why.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A participant becomes defensive

Return to a specific observation and its relevance rather than escalating into a judgment about the person. Ask what information they had and what they were trying to accomplish. If the exchange is no longer productive for the group, slow down and consider whether the issue should be handled through a separate, policy-consistent conversation.

A safety-critical misconception remains

Do not leave a material factual error unaddressed in the name of keeping the discussion comfortable. Clarify the point plainly, connect it to the scenario’s learning objective, and check that the group understands the correction. A respectful tone and clear feedback can coexist.

The group raises an issue outside the planned objectives

Decide whether it is important to the scenario and learner needs, and whether there is time to explore it adequately. If it is relevant but cannot be addressed well in the available discussion, name the boundary and use the appropriate local route for follow-up. Do not imply that a debrief itself replaces institutional support or escalation processes.

Close with learning that can transfer

Reserve time for participants to identify what they will take from the exercise. Ask each person or the group to name one learning point and one action they would carry into practice or another simulation. These prompts turn reflection toward application without asking learners to make a grand commitment or claim certainty they do not have.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Summarize only the central learning points the group actually discussed. Correct any remaining safety-critical factual issue, identify any appropriate next step, and close in a respectful way. The debrief should end with clarity about what was learned, not with a verdict on who performed well or poorly.
