Start with Elevance Health’s medical-cost trend and membership mix—not adjusted earnings per share alone. In the company’s July 15, 2026 release, second-quarter revenue was $49.8 billion and the benefit expense ratio rose to 89.7%; adjusted EPS was $7.45, including an approximately $0.80-per-share net below-the-line benefit. Those figures leave investors with two questions: whether premiums and operating changes can keep pace with claims costs, and whether the share price reflects the risks. The figures below are company-reported results for the quarter ended June 30, 2026, not a recommendation to buy or sell ELV.

What does Elevance Health do?

Elevance Health, Inc. (NYSE: ELV) is a U.S. health benefits and services company. Its two reporting segments are Health Benefits and Carelon. Health Benefits includes Individual, Employer Group risk-based and fee-based, BlueCard, Medicare, Medicaid, and Federal Employee Program businesses. Carelon comprises CarelonRx and Carelon Services. The company’s July 15, 2026 earnings release reports these segments separately; their revenue figures should not be added to infer consolidated revenue because segment definitions and eliminations matter.

What did the second-quarter 2026 results show?

Elevance’s July 15, 2026 earnings release reported the following for the quarter ended June 30, 2026. These are company-reported figures; adjusted EPS is a non-GAAP measure and is not interchangeable with GAAP EPS.

Measure Q2 2026 result How to read it
Operating revenue $49.8 billion, up 0.8% from Q2 2025 Revenue growth gives context, but does not by itself show whether underwriting margins improved.
Diluted EPS $6.71 GAAP; $7.45 adjusted The release said results included an approximately $0.80-per-share net below-the-line benefit. Consider that item when judging how repeatable adjusted earnings may be.
Benefit expense ratio 89.7%, up 80 basis points year over year Elevance attributed the increase to expected elevated medical-cost trends in government businesses, partly offset by improved Individual ACA performance.
Medical membership Approximately 44.9 million at June 30, 2026, down 469,000 sequentially The company cited a commercial fee-based customer transition and anticipated Individual ACA and Medicaid attrition.
Health Benefits segment $42.7 billion revenue; $0.9 billion operating gain Revenue grew 3% year over year while operating gain declined.
Carelon segment $19.2 billion revenue; $0.9 billion operating gain Revenue grew 6%; management cited Carelon Services risk-based solutions and CarelonRx product revenue.

The company raised 2026 diluted EPS guidance to at least $20.10, adjusted diluted EPS guidance to at least $27.00, and operating cash flow guidance to at least $6.0 billion. These are management estimates, not realized results. In its July 15 release, CEO Gail K. Boudreaux said the company was targeting at least 12% adjusted EPS growth in 2027 from its 2026 earnings baseline. That is management’s stated outlook, not an independently established forecast.

Why does the benefit expense ratio matter?

The benefit expense ratio compares benefit expenses with the applicable premium revenue; investors often use “medical loss ratio” to describe a similar measure. When the ratio rises, a larger share of premium revenue is going to benefits, which can pressure margins if premium rates and other income do not offset the increase. A single quarter is a signal to investigate, not proof that margins will continue to deteriorate.

Elevance’s second-quarter filing and release identify utilization and provider unit costs as important drivers. Other influences include service mix, member demographics and acuity, provider-contract inflation, labor costs, high-cost drugs, and regulation. The key investor question is whether the trend in claims costs can be priced into premiums—and when that pricing takes effect.

How should membership and product mix be assessed?

A total member count does not reveal how much premium risk the company carries or how profitable each business is. Fee-based membership can change reported enrollment without affecting risk-based premium exposure in the same way. Redeterminations and attrition can also change the health profile of members who remain, so fewer members do not automatically mean lower claims or better margins.

Track enrollment and operating performance by Individual ACA, Medicaid, Medicare Advantage, employer risk-based and fee-based products, and federal programs. Compare each category with its revenue, margin or operating gain where disclosed. For the June 2026 quarter, the company connected the sequential decline in total medical membership to a commercial fee-based customer transition and anticipated Individual ACA and Medicaid attrition; those explanations do not, by themselves, establish the financial effect of the mix changes.

Which business and policy risks deserve the closest attention?

Medical costs versus premium rates

Claims utilization, provider prices, acuity, labor, and expensive pharmaceuticals may rise faster than premiums. Elevance says health-plan premiums are set in advance relative to expected cost trends. If actual claims exceed assumptions, or competitive and regulatory conditions limit rate increases, profitability can weaken. Government program rate-setting can also lag changes in medical costs.

Medicaid, ACA, and Medicare Advantage

Medicaid and ACA enrollment changes can affect both revenue and the risk profile of the remaining membership. Medicare Advantage results are exposed to government rules, risk adjustment, quality ratings, and payment methodology. Elevance’s 2026 Form 10-Q reported that approximately 59% of its Medicare Advantage members were in plans rated at least 4.0 Stars or equivalent under 2026 Star Ratings used to determine 2027 bonus payments. This is Elevance’s reported membership figure, not a market-wide statistic.

CMS notice and separate DOJ litigation

In its Form 10-Q for the quarter ended June 30, 2026, Elevance said CMS closed the enforcement process on July 13, 2026, concerning certain diagnosis-code submissions for dates of service from 2015 through April 2023. The company said sanctions would not be imposed after required steps were completed. It reported a $342 million payment and a $593 million accrual as of June 30, 2026, and cautioned that the ultimate estimate could differ.

That closure does not resolve a separate Department of Justice lawsuit concerning Medicare risk-adjustment allegations. Elevance said the litigation was ongoing and its outcome could not then be determined. Investors should assess the two matters separately.

Other execution and regulatory exposures

The same filing identifies risks involving changes in health and insurance laws, Medicare and Medicaid compliance, litigation and government investigations, cyber and privacy incidents, information systems and technology, provider contracting, competition, debt, and capital requirements. Elevance reported no material changes to the risk factors in its 2025 Form 10-K as of June 30, 2026.

How can investors evaluate ELV’s valuation?

A valuation needs a share price tied to a specific date and an earnings measure with a clearly stated period and basis. No share price or peer multiple is established by the company results summarized here, so a current price-to-earnings multiple cannot be responsibly supplied. To calculate one, divide the date-stamped share price by the chosen per-share earnings estimate; label whether the denominator is trailing or forward and GAAP or adjusted. A multiple using the company’s at-least-$27.00 adjusted EPS guidance would be a guidance-based scenario, not a realized-earnings valuation, and should account for the approximately $0.80-per-share net below-the-line benefit in Q2.

Do not let a single quarter set the earnings baseline. Test whether a valuation still makes sense under different assumptions for benefit expense ratio trends, premium rate adequacy, membership and product mix, Medicare Advantage performance, Medicaid margin recovery, and Carelon’s revenue and operating gain. Compare earnings with operating cash flow, and consider debt, capital needs, and capital returns. If comparing insurers, use the same periods and definitions for GAAP versus adjusted EPS, medical-cost ratios, risk-based membership, government exposure, cash conversion, debt, capital returns, and valuation.

A practical research sequence

  1. Read the latest results and filing. Start with Elevance’s earnings release and SEC Form 10-Q, checking for disclosures after the quarter ended June 30, 2026. The figures in this article are from the July 15, 2026 release and the filing for that quarter; they should not be assumed to include later announcements.
  2. Separate reported performance from outlook. Record GAAP EPS, adjusted EPS, and any material items affecting comparability, then keep those results distinct from management’s 2026 guidance and 2027 outlook.
  3. Follow claims and enrollment together. Review the benefit expense ratio and its stated drivers alongside membership by business, rather than treating either the ratio or total enrollment as a complete picture.
  4. Check whether rates can catch up. Assess how the timing of premium setting and government payments compares with claims inflation, especially for Medicaid and Medicare Advantage.
  5. Build a valuation with explicit assumptions. Use a dated share price, identify the earnings period and GAAP or adjusted basis, and test the assumptions that could move normalized earnings and cash generation.
