Use gates around stairs or other unsafe areas, clear places where your dog could get stuck, improve traction where needed, and add nightlights along routes used after dark. If your dog develops new confusion, sleep changes, anxiety, or house soiling, arrange a veterinary assessment rather than assuming dementia is the only cause.
Start by checking the routes your dog uses
Walk through the home at your dog’s pace and look for places where disorientation or unsteadiness could lead to injury or leave the dog unable to get out. Make the safest route between resting, eating, drinking, and toileting areas as clear and familiar as possible.

Block stairs, drop-offs, and unsafe areas
Use a secure gate or other suitable barrier to prevent access to stairs or drop-offs if your dog is disoriented or unsteady. Check exits and the yard, too; fences or barriers may be needed to limit access to unsafe areas and reduce the chance of escape. Merck Veterinary Manual recommends barriers such as gates as part of environmental management for dogs with cognitive changes (Merck Veterinary Manual: Behavior Problems of Dogs).

Improve footing where it matters
Slippery steps can be difficult for a dog with mobility problems. Nonskid flooring on those steps may help; a ramp may be an option where the dog needs to manage a change in height. Choose only an arrangement your dog can use comfortably and safely in that specific space. Merck lists ramps and nonskid flooring among possible accommodations, not as a one-size-fits-all solution (Merck Veterinary Manual: Behavior Problems of Dogs).
Make nighttime movement easier to navigate
If your dog roams after dark, place nightlights along the routes it uses so familiar areas are easier to see. Keep the lights from adding cords or other obstacles to the path. Cornell University College of Veterinary Medicine recommends nightlights to improve visibility and help reduce the chance that a dog will get stuck (Cornell: Senior dog dementia).

Make toileting easier, but investigate accidents
For a dog that needs to go more often, offer more frequent outdoor trips. If you use an indoor toilet, make sure the dog can enter and leave it easily and that it is conveniently located.
New house soiling should prompt a veterinary visit, not just a change in cleaning routine. Cornell specifically recommends ruling out a bladder infection; other medical problems can also resemble cognitive changes. If a veterinarian agrees that a diaper is appropriate, change it frequently and keep the dog’s skin and coat clean and dry to reduce the risk of skin sores (Cornell: Senior dog dementia).

Keep the day predictable and comfortable
A consistent routine can make daily life easier to anticipate. Keep familiar cues—such as touch, sounds, and odors—where they help your dog orient, and avoid unnecessary changes to the layout. Provide exercise and mental enrichment appropriate to your dog’s comfort and mobility. Some dogs seek company; others need a quiet place to rest, so make room for both preferences rather than pushing activity beyond what the dog can manage. Merck and Cornell include routine and suitable environmental enrichment among management approaches (Merck Veterinary Manual; Cornell: Cognitive dysfunction syndrome).

Canine cognitive dysfunction syndrome can involve disorientation, changed interactions, altered sleep and wake patterns, house soiling, anxiety, and changes in activity. These signs may develop gradually, but they do not establish dementia on their own. Pain, arthritis, seizures, systemic illness, and sensory loss can produce similar changes.

Contact your veterinarian if you notice new or worsening behavior, confusion, sleep changes, or accidents indoors. Cornell notes that an examination and blood and urine testing may help check for other causes. A veterinarian can also help you track changes; Cornell identifies Katherine Houpt, James Law Professor Emeritus of Behavior Medicine, as recommending the DISHAA assessment guide for monitoring signs, and identifies veterinary behaviorist Gary Landsberg as the questionnaire’s developer (Cornell: Cognitive dysfunction syndrome; Cornell: Senior dog dementia).
