Many routine lipid panels can be done without fasting; fasting is used when a clinician needs it for your circumstances or prior results. If you are told to fast, confirm the exact duration and what to do about water and medicines rather than guessing.
What is a cholesterol test?
A cholesterol test is a blood draw, usually reported as a lipid panel. A standard panel measures total cholesterol, LDL cholesterol, HDL cholesterol, and triglycerides. The Centers for Disease Control and Prevention (CDC) says the only way to know whether you have high cholesterol is to get tested; the result still needs to be interpreted in context.

Some clinicians may use an advanced lipid panel to measure apolipoprotein B (apoB) or count cholesterol particles. That is different from the standard panel and may be useful for some people; a healthcare professional can advise whether it is appropriate.
Do you need to fast before a cholesterol test?
Not necessarily. The American Heart Association (AHA) says a lipid panel may be fasting or nonfasting depending on the person’s situation. The 2026 ACC/AHA dyslipidemia guideline recommends either a fasting or nonfasting standard lipid profile for baseline levels and risk assessment in adults and children. The clinician’s instructions for your specific test take precedence.

A fasting sample can be useful when a clinician needs to assess triglycerides under fasting conditions or when certain history or previous results warrant it. The 2026 guideline identifies situations such as a known or suspected triglyceride metabolism disorder, a nonfasting triglyceride result of 400 mg/dL or higher, and relevant family history. Do not arrange a fasting repeat on your own; ask the clinician who ordered the test whether one is needed.
How long should you fast if instructed?
Official patient guidance gives slightly different ranges for tests that require fasting: CDC and the National Heart, Lung, and Blood Institute (NHLBI) say 8 to 12 hours, while the National Institutes of Health (NIH) consumer page says 9 to 12 hours. These ranges are not a universal requirement for every cholesterol test. Confirm the duration with the ordering clinician or laboratory for your appointment.

- IMPORTANT CODE CHIP SETUP: To ensure the meter reads the correct code, always INSERT the code chip while the meter is turned OFF. Ensure the side with the code number (e.g., 602) is facing UP when inserting. Inserting the chip with the "CODE" text side facing up is incorrect and will cause an error code!
- Advanced Electrochemical Sensing Technology: This total cholesterol meter utilizes state-of-the-art electrochemical sensor technology to provide professional-grade accuracy for monitoring cholesterol levels at home. It is designed to quantitatively measure total cholesterol using fresh capillary or venous whole blood, offering a reliable aid for managing your cardiovascular health.
- Fast Results with Tiny Blood Sample: Get your total cholesterol readings in just 25 seconds. The system requires only a tiny 1μL blood sample, minimizing discomfort during testing. The pre-suction siphon design of the test strips ensures blood is applied easily and effectively every time.
- Comprehensive Data Tracking: Stay on top of your health trends with a large internal memory that stores up to 500 test results. The meter automatically calculates 7, 14, and 28-day averages, allowing you and your healthcare provider to easily monitor the effectiveness of your cholesterol control over time. Perfect for proactive lipid management, helping you track the key component of your lipid profile alongside a heart-healthy diet.
- User-Friendly Design & Large LCD: Features an easy-to-read, large LCD screen that guides you through the testing process with clear symbols and reminders. For added convenience, the device includes an automatic shut-off feature after 3 minutes of inactivity and a low-battery warning to ensure you are never caught off guard.
Ask the clinic what to do about water, coffee, medicines, exercise, alcohol, and any change to your usual diet. The sources cited here do not establish one comprehensive set of rules for those details. Do not stop a prescribed medicine or alter your routine unless your clinician tells you to.
Fasting and nonfasting tests: what is the difference?
| Approach | When it may be used | Practical point |
|---|---|---|
| Nonfasting lipid profile | Suitable for many baseline assessments and routine risk evaluations, according to the 2026 ACC/AHA guideline. | Does not require scheduling around a fasting interval; follow the instructions on your order. |
| Fasting lipid profile | May be requested for a known or suspected triglyceride metabolism disorder, a nonfasting triglyceride result of at least 400 mg/dL, or certain family-history contexts described by the 2026 guideline. | Requires the fasting interval specified by the clinician or laboratory; check the instructions rather than assuming a duration. |
The choice is about what information your clinician needs, not about one approach being universally more accurate. If fasting is inconvenient or unclear, contact the ordering office before the blood draw so it can confirm the plan.

How should you understand the results?
A lipid panel provides numbers, not a diagnosis or treatment plan by itself. CDC says clinicians consider cholesterol results alongside factors such as age, family history, sex, lifestyle, smoking, and other health conditions. Review the report with a healthcare professional, who can explain what the results mean for you and whether follow-up is appropriate.
For population context, CDC reported that 86.4 million U.S. adults age 20 or older had high or borderline-high cholesterol—defined in that report as total cholesterol of at least 200 mg/dL—during 2017–2020. This is a dated population statistic, not a threshold that independently determines an individual’s care.
