To get a new Verizon Fios TV set-top box, sign in to My Verizon and choose Manage home internet, or contact Verizon Customer Service. Verizon may require a plan or equipment update, and additional fees may apply. Confirm your eligibility, total cost, and whether the request is for an extra box or a replacement before placing an order.
Request a box through Verizon
Fios TV set-top boxes are managed through your Verizon service account. Verizon’s Fios TV Home App FAQ gives two ways to request one:

This is a request route, not a guarantee that every account or plan is eligible. Before agreeing to an order, ask whether the box is available for your account, whether your service package or other equipment must change, and what one-time or recurring charges will apply. Verizon’s public guidance does not establish an individual account’s price.

Be clear about what you need: an additional box, a replacement for a defective device, or an upgrade from older equipment. Those situations can have different eligibility and return instructions. If you are asking how to use your own device instead of an existing additional set-top box, Verizon says a package or equipment update may be needed and that its box must be returned within 30 days in that specific situation. Do not assume that deadline applies to every replacement; follow the instructions for your order.

If the box is defective
If your set-top box is failing, contact Verizon before ordering equipment elsewhere. For qualifying purchased equipment, Verizon’s published policy says a defective item may be eligible for repair, replacement with new or refurbished equipment, or a refund. The policy says the return must be within one year of purchase or activation, whichever is earlier, and is subject to its limitations. It does not promise that a rented or Verizon-owned box will be handled under the same terms.
Keep the old box and its accessories until Verizon tells you what to return and how. Ask whether the replacement request is being treated as a repair, a warranty or policy claim, or a new equipment order, and get the applicable return deadline.

Check whether an older box has reached its cutoff
Verizon’s set-top-box FAQ says Cisco set-top boxes will not transmit Fios TV video content on or after August 23, 2024, and Motorola set-top boxes will not transmit it on or after April 15, 2025. Both dates have passed. Check the label on the device rather than relying on its appearance, then contact Verizon about the upgrade path for that model. These statements refer to Cisco and Motorola set-top boxes; they do not establish that every older-looking device has the same cutoff.

Connect and activate the new box
Verizon’s Fios TV activation instructions describe this basic connection:
- Connect a coaxial cable from the VMS or set-top box to a coax outlet.
- Connect the set-top box to the TV with an HDMI cable.
- Connect the box’s power cord.
- Turn on the box and TV, then follow the on-screen steps. Verizon also offers online and My Fios app activation.
The activation instructions say the Fios router should already be installed. They establish that HDMI is needed for the described connection, but not whether your particular order includes a cable. Check the package contents or ask Verizon before buying one. Do not assume a used retail set-top box will work as a substitute for equipment provisioned to your account.

Return old equipment and keep proof
If Verizon instructs you to return equipment, contact the company to start the Fios return process. Verizon’s Fios Equipment Return FAQ says it will identify nearby UPS Store locations that can accept the equipment. At the store, staff scan the serial numbers, pack and ship the equipment; keep the return receipt. Verizon says it emails a confirmation after processing.
Follow the deadline and return instructions tied to your specific order or account. The FAQ lists these current unreturned or damaged equipment amounts:
