Choose the correct troubleshooting path
PS5 and Windows PC problems can look identical—a black image, lost tracking or a controller that will not respond—but they do not have the same cause. Use this table to select the first section to follow.

The detailed image, tracking and connection sequence below is for Windows PC use with Sony’s PC adapter and associated software. PS5 guidance is limited to the console steps documented by Sony’s support material.
Before changing several things at once, write down the exact symptom and when it occurs. “The headset is not detected,” “the image is black,” “the controllers are not paired,” “the room boundary disappears,” and “the picture stutters” point to different checks. A controller reset will not restore a DisplayPort signal, and changing SteamVR quality will not repair a dirty tracking camera. Work from the platform and symptom first, then retest after each major change.

PS5: fix Sense-controller pairing and reset problems
Pair each controller for the first time
- Turn on the PS5.
- Connect one PS VR2 Sense controller to the console with the included USB cable.
- Press the controller’s PS button and wait for it to pair.
- Repeat the same process for the second Sense controller.
Sony states that a Sense controller pairs with one PS5 at a time. If a controller was last used with another console, pair it again with the PS5 you intend to use. Controllers charge individually. Controller-device-software updates are delivered with PS5 system-software updates, but charging or updating should not be treated as a guaranteed cure for every connection fault.
Pair the controllers one at a time rather than assuming both will be configured automatically. Use the included cable for the initial pairing step, and keep the console powered on while pressing the PS button. If only one controller works, repeat the same USB pairing process for the other before trying a reset.
Reset a Sense controller that will not connect
- Power off the PS5 completely.
- Find the small reset button on the back of the affected Sense controller.
- Press the button with a pin or similar narrow tool.
- Connect the controller to the PS5 with a USB cable.
- Press the controller’s PS button to pair it again.
Resetting is aimed at pairing or controller-state problems. It does not repair a damaged cable, failed charging circuit or headset tracking-camera fault.
Reset only the controller that is failing, then test it before resetting the other one. A reset clears the controller’s pairing state; it does not erase the PS5, alter your play-area settings or fix a headset that has no image. If the controller still does not respond after a USB reconnection and reset, use Sony’s console troubleshooting selector and support path instead of repeatedly pressing the reset button.
Reposition the cinematic-mode screen
If the virtual screen in cinematic mode is off-center or sitting at an inconvenient angle, hold the Options button on the Sense controller to reposition it. This recenters the view; it is not a fix for a completely blank panel, a damaged display or an optical fault.
Use this step only when the screen is visible but positioned incorrectly. A screen that remains black, flickers, or shows a persistent hardware warning needs the official symptom flow. Repositioning also does not change game tracking, controller pairing or the physical lens adjustment.
When a PS5 problem needs the official flow
For symptoms not covered by the steps above, open Sony’s official PS VR2 troubleshooting page and select the matching symptom. Keep the platform set to PS5 so that you do not apply Windows adapter procedures to a direct-console connection. Persistent hardware symptoms should be escalated to PlayStation Support.
Windows PC: rebuild the physical connection in the correct order
The PC adapter requires four separate connections. A missing power lead, an unsuitable USB port or the wrong video-conversion cable can prevent the headset from appearing in the PlayStation VR2 App. Treat the adapter as a complete signal chain: USB carries the computer connection, DisplayPort carries video, the AC adapter supplies power, and USB-C links the headset to the adapter.
Required connection chain
- Plug the adapter into a USB 3.0 Type-A port on the Windows PC.
- Connect the adapter to the PC’s DisplayPort using a separately sold DisplayPort 1.4 cable.
- Connect the supplied AC adapter and power cord to the PC adapter and a power outlet.
- Connect the headset to the adapter’s USB-C port.
- Turn on the headset and wait for the adapter and headset indicators to turn white.
Sony’s adapter manual corroborates this interface sequence. The DisplayPort cable is a separate requirement, not an optional substitute for USB. The adapter is the PC connection route; a direct HDMI or generic video lead does not replace it.
Check both ends of every cable, including the power connection, before launching a game. A connector can look inserted while remaining loose enough to interrupt detection. The white indicators are a useful basic check that the adapter and headset have power and are connected, but they do not by themselves prove that a game has a valid video signal.

Eliminate unsupported conversions
Do not use HDMI-to-DisplayPort or USB-C-to-DisplayPort conversion for the adapter. Sony identifies those conversions as unsupported, so an apparently correct connector does not establish a supported signal path. Replace the conversion chain with a direct DisplayPort 1.4 connection from the adapter to the PC.

Remove docks, video converters and other intermediate devices while diagnosing. A direct cable makes it possible to tell whether the adapter, port or cable is responsible for the failure.
Check USB and software status
- Unplug and firmly reconnect every adapter, power and headset cable.
- For a “Check USB Connection” message, connect the adapter directly to a USB 3.0 port rather than a hub or dock.
- Try another USB 3.0 port if the first one is not detected.
- Open the PlayStation VR2 App and inspect its connection status.
- Install current graphics drivers for the PC’s graphics hardware.
- Confirm that the PC meets Sony’s stated requirements.
- Turn off both the headset and PC, then restart them if the connection error remains.
Changing ports is useful only when the replacement port is also USB 3.0. A hub, front-panel extension or passive adapter can add another failure point, so test the adapter directly on the computer first.
Check the app before assuming that a game is at fault. If the PlayStation VR2 App cannot see the adapter, remain in connection troubleshooting rather than changing SteamVR image settings. If the app sees the hardware but a game will not start, verify drivers and the graphics-output arrangement described below.
Use the correct graphics output
On a desktop with a discrete graphics card, Sony notes that some games may fail to start when the monitor is connected to the motherboard’s onboard DisplayPort while the VR adapter is connected to a separate graphics card. Connect the monitor to the external graphics card in that configuration, then restart the game.

This arrangement matters only to a PC with both onboard and discrete graphics outputs. It is not a reason to buy a converter or to move the headset to an HDMI port. Keep the VR adapter on the supported DisplayPort path and make the monitor and adapter use the graphics hardware Sony’s troubleshooting flow expects.
PC headset tracking: restore the room map
Headset tracking uses the cameras on the visor and the play-area data stored by the PlayStation VR2 App. Tracking can degrade when the cameras are dirty, the room is too dark or bright, the view contains too few visual features, or the saved play area no longer matches the room.
Reset play-area data
- Open the PlayStation VR2 App.
- Delete the existing play-area data.
- Start play-area setup again from the app.
- Slowly scan the entire room, including the ceiling and floor, as prompted.
Scanning only the walls can leave the map incomplete. Give the cameras a view of stable features throughout the usable space. If the room has plain, featureless walls, add visible features such as posters or pictures so the tracking system has patterns to recognize.
Recreate the area whenever furniture, lighting or the usable boundary changes substantially. Move your head slowly enough for the app to capture all parts of the room, including the floor and ceiling, instead of completing the scan from one standing position. The goal is a consistent view of the space, not simply drawing a boundary quickly.

Wipe the headset’s tracking cameras with a soft, dry cloth intended for lens cleaning. Do not use a wet household cloth or abrasive material. Clean only the exterior camera surfaces and then repeat room setup if the play-area boundary was created while the cameras were obscured.

Hold the headset securely while cleaning and inspect the camera windows for smears or dust under the room light. The recommended cloth is a class of optical-cleaning cloth, not a promise that a particular accessory will solve tracking. If tracking remains unavailable after a clean surface, a complete scan and suitable lighting, escalate rather than opening the headset.
Correct lighting and sunlight
- Avoid direct sunlight entering the cameras’ view.
- Increase general room lighting when the scene is dim.
- Remove strong glare or backlighting that washes out room features.
- Keep lighting conditions similar each time you recreate the play area.
These changes address optical tracking conditions. They do not repair a camera that remains unusable after cleaning and a controlled lighting test.
Test with ordinary, even room lighting and keep reflective glare out of the camera view. If tracking works in one lighting condition but fails whenever direct sunlight returns, the environmental cause is more likely than a controller or cable fault. Do not confuse this headset-camera process with PC Bluetooth troubleshooting: the two systems can fail independently.

PC Sense-controller tracking: fix the Bluetooth link separately
Headset tracking and Sense-controller tracking are different systems on PC. The controllers communicate over Bluetooth, so a headset that tracks correctly can coexist with controllers that drift, disappear or fail to respond.
Improve the Bluetooth connection
- Install current Bluetooth drivers for the PC.
- If you use an external Bluetooth adapter, place it in a USB 2.0 port away from USB 3.0 devices and cables.
- If the computer’s case blocks the adapter, use a USB 2.0 extension cable to place it where the controllers have a clearer path.
- Charge the affected controller.
- Turn off other Bluetooth devices temporarily while testing.
- Remove furniture or other obstructions between the controller and Bluetooth adapter.
- When the PC is on Wi-Fi, use the 5 GHz band if available.
Bluetooth-adapter compatibility varies. Sony does not guarantee that every third-party adapter will work, so test with a current, directly connected adapter before concluding that the controller is defective.
Keep the external adapter away from USB 3.0 interference even if both ports are electrically functional. The extension cable is for positioning and visibility, not a replacement for a compliant Bluetooth adapter. Test with other Bluetooth devices temporarily disconnected so you can isolate the Sense-controller link.
Prevent a PS5 from reclaiming the controllers
If the Sense controllers were previously paired with a PS5, turn that console off while using them on the PC. A powered-on nearby PS5 can interfere with the intended Bluetooth session or cause the controllers to reconnect to the console instead.

PC display problems: black image, blur and poor quality
Start with signal integrity
For a black or missing PC image, repeat the physical-connection checklist before changing SteamVR settings: direct USB 3.0 Type-A, direct DisplayPort 1.4, supplied power, USB-C from adapter to headset, white status indicators, current graphics drivers and a confirmed PlayStation VR2 App connection. A display setting cannot compensate for a missing video or power signal.
Do not treat a black image as proof that the lenses or panel are damaged until the app and adapter chain have been checked. Reconnect the cable at both ends, try another supported USB 3.0 port, verify the monitor is connected to the appropriate graphics card, and restart the PC and headset if the error persists.

Reduce image-quality load in SteamVR
If the picture is unstable or performance drops after the connection is confirmed, lower SteamVR’s image-quality settings. Sony also recommends disabling Motion Smoothing as a test. These are PC software adjustments; they do not recenter a PS5 cinematic screen or correct a damaged headset panel.
Change one SteamVR option at a time and retest the same scene. Lowering quality can help distinguish a performance-related blur or instability from a missing signal. If the image remains completely absent, return to cables, power, drivers and app status rather than continually lowering quality.
Address frame stutter in Windows
For persistent frame stutter, turn off Windows hardware-accelerated GPU scheduling in the system’s advanced graphics settings, then restart the game and reassess. Change one setting at a time so you can identify whether the scheduling option affected the symptom.
Frame stutter is a PC software symptom and should be evaluated after the headset is detected correctly. It is not evidence that the room map needs to be deleted, and it does not justify installing generic PC-cleanup software. Sony’s supported path is to adjust SteamVR image quality or Motion Smoothing and test the Windows scheduling setting.

Sony warns that resuming a SteamVR game after the PC sleeps can produce an error. The most reliable prevention is not to put the PC to sleep during a VR session.
If the error has already appeared, use this recovery order:
- Turn off the headset.
- Turn off the monitor.
- Turn the monitor on.
- Turn the headset on.
Then relaunch the SteamVR game. This sequence re-establishes the monitor and headset startup order without changing the saved play area. If the same error returns after avoiding sleep and repeating the sequence, continue with the app connection and driver checks rather than treating sleep recovery as a permanent hardware repair.

Stop changing settings once the headset, controllers and image work reliably in the same conditions. Keep the play area consistent, avoid direct sunlight, and reconnect the adapter directly if a future session reports a USB error. If a camera, display, adapter or controller still fails after the platform-specific checks, contact PlayStation Support for PS VR2 hardware assistance. Sony’s guidance does not promise a user-serviceable repair, and generic PC-cleanup software is not a substitute for checking the physical connection, room conditions or supported VR settings.
