PHP mail() has two sender identities: the visible From: header and the SMTP envelope sender that can become Return-Path:. Set a server-controlled address in From:, put a validated visitor address in Reply-To:, and pass a controlled -f address when using a sendmail transport. Then verify the raw message and transport logs; a true return value means acceptance by the configured delivery system, not successful arrival.

Understand which “sender” is wrong
An email can display one address to the recipient while using another address for bounces and delivery errors. PHP hands your message to either a local mail transfer agent (MTA) or a direct SMTP connection. The transport then handles the SMTP envelope separately from the message headers.

From: |
Message header shown by most mail apps | The displayed author or sender | Set in additional_headers; inspect the raw message |
Reply-To: |
Message header used when the recipient clicks Reply | Where responses are directed | Set to a validated visitor address |
Sender: |
Optional message header | Identifies the agent that transmitted the message when different from From: |
Usually supplied by the sending system or framework |
| SMTP envelope reverse-path | SMTP transaction, not normally visible in the message body | Where delivery errors and bounces are sent | Set by the SMTP MAIL FROM command; with Unix sendmail, use the -f option |
Return-Path: |
Usually added to the received message by the final receiving system | Shows the envelope reverse-path used for that delivery | Inspect the raw received message; do not rely on a manually added header |
RFC 5321, published by the RFC Editor in 2008, defines MAIL FROM as the SMTP envelope reverse-path. The receiving system can construct Return-Path: from that envelope information. Consequently, changing From: does not necessarily change Return-Path:, and adding a Return-Path: header yourself is not a reliable fix.
Inspect the raw message before changing code
First establish whether the unexpected address is in the visible header, the envelope, or both. Looking only at the compact message view in a mail app can hide the distinction.

- Open the message’s “show original,” “view source,” or equivalent raw-message screen.
- Record the exact
From:,Reply-To:,Sender:, andReturn-Path:values. Also note anyReceived:lines that identify the relay. - If
From:is wrong, search the application, framework mail wrapper, environment variables, and PHP configuration for a default sender. - If only
Return-Path:is wrong, investigate the envelope sender, local MTA, SMTP relay, or hosting policy rather than changing the visible header. - Save the timestamp, recipient, and message identifier so the corresponding MTA or relay log entry can be located.
A message may contain a correct From: while the relay rewrites the envelope for authentication or policy reasons. Conversely, a local configuration can inject an unexpected visible header before the message leaves the server.
Set a controlled From address and a separate Reply-To
The safest basic pattern is to use a mailbox on a domain controlled by the site operator for From:. Put the person who submitted a form in Reply-To: after validating that address. This avoids claiming to send from a visitor’s domain when the server or relay is not authorized to do so.
<?php
$validatedVisitorEmail = filter_var($_POST['email'] ?? '', FILTER_VALIDATE_EMAIL);
if ($validatedVisitorEmail === false) {
throw new InvalidArgumentException('A valid email address is required.');
}
$headers = [
'From' => 'Website <[email protected]>',
'Reply-To' => $validatedVisitorEmail,
'Content-Type' => 'text/plain; charset=UTF-8',
];
$sent = mail($to, $subject, $body, $headers);
if (!$sent) {
error_log('PHP mail() could not hand the message to the configured transport.');
}
?>
Replace [email protected] with an address that exists, is monitored or intentionally unattended, and is permitted by your host or relay. Keep the visitor’s address out of From: unless your authenticated SMTP service explicitly authorizes that domain and address.
When using the string form of additional_headers, separate headers with CRLF as required by the PHP manual. The array form makes individual fields easier to audit, but it does not remove the need to validate values supplied by users.

Prevent header injection from form input
Never concatenate unchecked form fields into From:, Reply-To:, Cc:, or Bcc:. An attacker who inserts carriage-return or line-feed characters can add new headers or alter recipients.
- Validate an email value with an email parser such as
filter_var($value, FILTER_VALIDATE_EMAIL); treat a failed result as invalid. - Reject any value containing
rornbefore placing it in a header, even if another validation step is present. - Do not accept arbitrary header names or complete header blocks from a browser.
- Keep recipient lists, subjects, and body text separate from header construction, and apply the same newline checks to any user-controlled subject or address list.
- Use a fixed server-owned
From:value whenever possible.
function validatedReplyTo(string $value): string
{
if (str_contains($value, "r") || str_contains($value, "n")) {
throw new InvalidArgumentException('Newlines are not allowed in an email address.');
}
$email = filter_var($value, FILTER_VALIDATE_EMAIL);
if ($email === false) {
throw new InvalidArgumentException('Invalid email address.');
}
return $email;
}
Validation is a security requirement, not merely a deliverability preference. PHP’s mail documentation specifically warns that data used in headers must be sanitized to prevent header injection.
Set the envelope sender when PHP uses sendmail
On Unix-like installations, PHP commonly delegates to the executable configured by sendmail_path. In that arrangement, the fifth argument to mail() can pass sendmail’s -f envelope-sender option.
$headers = [
'From' => 'Website <[email protected]>',
'Reply-To' => $validatedVisitorEmail,
'Content-Type' => 'text/plain; charset=UTF-8',
];
$sent = mail(
$to,
$subject,
$body,
$headers,
'[email protected]'
);
The -f value should be a server-controlled address accepted by the MTA and the receiving relay. PHP escapes the additional parameter internally, but the PHP manual still recommends sanitizing it. Do not build it directly from a visitor’s form field.

Some sendmail configurations require the web-server account to be listed in /etc/mail/trusted-users. If it is not trusted, sendmail may add an X-Warning: header or reject the requested envelope sender. Check the MTA log and its local policy rather than repeatedly changing the PHP header.
Do not confuse the fifth argument with a message header. It affects the transport command and therefore the envelope reverse-path; it does not replace the visible From: header.

| Windows PHP direct SMTP | SMTP, smtp_port, and sendmail_from in php.ini |
The configured Windows sender appears despite application headers, or delivery goes to an unexpected host | Confirm the SMTP host and port, set an authorized sendmail_from, then restart the relevant PHP process |
| Unix-like PHP with local MTA | sendmail_path, the selected MTA’s configuration, and local sender policy |
Return-Path: differs from the requested envelope address, or the MTA adds warnings |
Review the MTA log, trusted-user rules, and the -f argument |
| Framework or hosting wrapper | Mail-driver settings, environment variables, cached configuration, and deployment secrets | Direct PHP test works but application mail uses a default or staging address | Trace the wrapper’s final headers and transport call; clear its configuration cache after changing values |
PHP’s runtime-configuration documentation distinguishes Windows direct SMTP settings from Unix-like use of sendmail_path. Edit the configuration file actually loaded by the web worker, not only the one used by the command-line binary. A quick diagnostic page can display php_ini_loaded_file() and the relevant ini values in a protected administrative context; remove that diagnostic after use.
Configuration changes normally require restarting the web server, PHP-FPM pool, FastCGI worker, or other process that caches PHP settings. A command-line test can therefore show a different result from a browser request until both environments use the same configuration.

Trace framework and application defaults
If a minimal mail() script sends the expected address but the production application does not, the PHP runtime is probably not the source of the rewrite. Inspect the code path that ultimately calls the mail transport.
- Search for every
mail(call and every mailer configuration key containing “from,” “sender,” “return,” or “envelope.” - Check environment variables loaded at deployment time; staging values often survive in a production container or worker.
- Look for a framework’s global default sender that is applied after your message-specific headers.
- Confirm that configuration caches were rebuilt after editing environment files.
- Log the final recipient, visible sender, reply address, and chosen transport without logging passwords or full message bodies.
- Compare the message generated by a form request with one generated by a command-line or queue worker; they may run under different users and settings.
Inspect the final raw message rather than assuming that the array passed to your application’s mail API is the array passed to PHP. Wrappers may normalize, overwrite, or add Sender: and envelope values.
Understand why mail() can return true while no email arrives
The PHP Documentation Group states that a true result means the configured delivery system accepted the message. It does not prove that the recipient’s server accepted it, that the message passed filtering, or that it reached the inbox.
- Check the application log for the exact time and the boolean result from
mail(). - Check the local MTA log or SMTP relay log for a queue ID, SMTP response, rejection, deferment, or rewrite.
- Inspect the recipient’s spam, quarantine, and filtered folders.
- Verify that the relay authorizes the
From:domain and envelope sender. - Review domain authentication and policy records configured for the sending domain with the responsible mail administrator.
- Use the raw message to compare
From:andReturn-Path:with the values recorded in the transport log.
A queued message can produce a successful handoff result and still fail later because of a DNS problem, recipient rejection, reputation decision, mailbox quota, or policy violation. Treat acceptance and final delivery as separate checkpoints.

Use an authenticated SMTP library or relay when you need control
PHP’s manual notes that mail() is inefficient for larger volumes and points readers toward PEAR Mail packages. An SMTP-capable library or managed transactional email service can make authentication, envelope selection, connection errors, and message IDs more explicit than a local sendmail handoff.
| Approach | Visible-header control | Envelope control | Authentication | Diagnostics and operational effort |
|---|---|---|---|---|
Local mail() with sendmail |
High, subject to MTA rewriting | Use the fifth argument and MTA policy | Handled by the local MTA or upstream relay | Requires server access, queue inspection, and MTA knowledge |
| Windows direct SMTP through PHP settings | Uses application headers plus runtime defaults | Influenced by sendmail_from and the SMTP server |
Depends on the configured SMTP service and PHP capability | Simple for small deployments, but runtime configuration can be opaque |
| SMTP-capable library | Explicit message and reply headers | Explicit envelope sender in the SMTP transaction | Supports authenticated SMTP credentials when configured | Usually clearer errors and connection logs; adds dependency and secret management |
| Managed transactional relay | Restricted to verified sender identities | Provider controls or permits the envelope sender according to account policy | Account credentials or API authentication | Centralized logs, bounces, and quotas; requires domain verification and provider setup |
Move to an SMTP library or authenticated relay when you need reliable authentication, multiple sending identities, bounce processing, queue visibility, or more than occasional low-volume notifications. Verify the current library and provider documentation before implementation, because authentication methods, sender-verification rules, limits, and configuration screens change.
Even with a relay, keep the same separation: a verified site address in From:, the visitor in Reply-To:, and an envelope sender authorized by the relay. A provider may rewrite or reject unauthorized combinations.
Apply a repeatable production checklist
- Use a fixed, domain-controlled
From:address. - Validate and sanitize the visitor address before assigning
Reply-To:. - Reject carriage returns and line feeds in every user-controlled header value.
- On sendmail transports, pass a controlled envelope address with
-f. - Confirm the correct
php.iniis loaded by the web worker. - On Windows, verify
SMTP,smtp_port, andsendmail_from. - On Unix-like systems, verify
sendmail_path, MTA permissions, and trusted-user policy. - Restart the process that caches configuration after changes.
- Capture MTA or relay logs and correlate them with the application timestamp.
- Inspect the raw received message for
From:,Reply-To:,Sender:, andReturn-Path:. - Test both successful handoff and downstream receipt; do not treat
trueas proof of delivery. - For sustained volume or strict deliverability requirements, use authenticated SMTP through a maintained library or transactional relay.
What to change first
For most sites, the immediate fix is a server-owned From: header, a validated visitor Reply-To:, and—when PHP invokes sendmail—a matching server-owned -f envelope address. If the raw message still shows a mismatch, the remaining cause is outside that header array: inspect the loaded PHP configuration, MTA or relay policy, framework defaults, and transport logs. Keep those layers separate and you can identify whether the problem is display, reply routing, bounce routing, or final delivery.
