When an AI intake, transcription, summarization, or client-management tool gets a detail wrong, correct more than the visible note: verify what the client said, identify every record derived from it, prevent the disputed information from driving consequential action while you check it, and update affected copies and decisions. A client may have stated something accurately while the AI transcript or summary distorted it; an inference or opinion must not be presented as an established fact.
How do I correct an AI-generated client record?
Use a documented workflow that follows the information from its source to every place it was stored or used. Assign a person to own the case, especially if the disputed detail could affect eligibility, service, payment, safety, or another important outcome.
- Receive and log the challenge. Under UK Information Commissioner’s Office (ICO) guidance, a rectification request can be verbal or written and need not cite a legal provision. It can reach any part of an organisation, so train client-facing staff to recognize and route it. Record when it arrived, the exact field or statement challenged, the client’s explanation, and the correction they seek. See the ICO’s right to rectification guidance.
- Trace the claim through the system. Locate the original client input, any source document or recording, the AI transcript or summary, structured fields, later inferences, CRM copies, exports, and decisions or communications that relied on the information. For each item, record its source and date, and whether it is a current fact, a historical fact, or an opinion. The ICO accuracy guidance says source and status should be clear and that accuracy depends on the purpose for which data is used.
- Pause consequential use while checking. Compare the disputed entry with suitable source evidence and ask the client for clarification if needed. Where continued processing could affect the client, do not let the challenged item drive a new consequential action while its accuracy is under review. The ICO says restricting processing during verification is good practice; how much checking is reasonable depends on the importance and consequences of the information.
- Decide what kind of correction is needed. Correct information that is wrong or misleading when stated as fact. Add missing information when the record needs it for its purpose. If the entry is an opinion, identify it as an opinion and, where appropriate, say whose view it records. A client’s disagreement alone does not necessarily make an opinion inaccurate. Keep a past entry only when it accurately records what happened, and make any later correction or finding clear.
- Fix the authoritative record and follow the error outward. Update the source-of-truth field, then check summaries, classifications, caches, exports, connected systems, and decisions that used the error. Notify recipients of corrected information when the applicable law requires it. If you decline a requested change, explain why and identify the relevant complaint or remedy route.
- Close the loop and prevent repeats. Tell the client what changed or why you consider the existing information accurate. Keep a record of the challenge, evidence, reviewer, decision, affected copies, and notifications. Check for recurring causes such as a confusing intake prompt, transcription error, field-mapping problem, unsuitable reference data, or inadequate review. The ICO’s AI individual-rights guidance recommends recording challenges and whether they changed decisions, monitoring them, and considering system changes when they reveal repeated errors.
What if the AI got the client’s statement wrong?
Separate what the client actually supplied from what the AI produced. For example, a recording may contain the correct address while speech recognition transcribes a different street; the error is in the derived transcript, not necessarily in the client’s statement. Correct the transcript and every downstream field or decision that inherited the mistake. Preserve the original source where appropriate, with access and retention handled under your organisation’s rules.
Do not silently overwrite history if an audit record is needed. A record of the original mistake can remain when it truthfully documents what happened and clearly shows the correction or later finding. That distinction matters: “the client gave address A” and “the system recorded address B” are different claims.

How should I handle missing details, historical facts, and opinions?
- Incomplete fact: Add the missing detail if it is needed for the stated purpose, after checking it with the client or an appropriate source. Avoid filling gaps with an AI-generated guess.
- Historical fact: Keep the time context explicit. A former address may be accurate as a past address but misleading if shown as the client’s current address.
- Opinion or inference: Label it as such, attribute it where appropriate, and distinguish the underlying observations from the conclusion. A system-generated risk label is not an independently established fact merely because it appears in a structured field.
- Disputed factual claim: Record the client’s challenge and assess evidence suited to the claim and its consequences. If it remains unresolved, avoid presenting it as settled fact and follow applicable rules for restricting or annotating the record.
How much human review is needed?
For a consequential decision, review must be more than clicking approval on an AI recommendation. The reviewer needs to understand the relevant evidence and output, have the competence and authority to change the result, and consider what the client says. The ICO warns that automation bias can lead staff to accept outputs uncritically, while poor interpretability can leave reviewers unable to assess them meaningfully.
Where processing is solely automated and has legal or similarly significant effects, UK ICO guidance describes safeguards that include human intervention, an opportunity for the person to express a view and contest the decision, and an explanation of the logic involved. The exact safeguards depend on the law and the particular processing context.

Which correction deadline and legal rights apply?
Rights and deadlines depend on the applicable privacy law, the organisation’s role, and the processing context; UK and EU guidance should not be treated as a universal rule for every country or sector. Under the UK GDPR guidance described by the ICO, an organisation ordinarily has one calendar month to respond to a rectification request. The European Commission says the GDPR response period is in principle one month. These are response periods, not a reason to leave a harmful error in active use while it is being checked.
The UK accuracy principle requires reasonable steps to keep personal data accurate for its purpose, clear about source and status, and corrected or erased without delay when inaccurate. The ICO reproduces the UK GDPR wording as: “accurate and, where necessary, kept up to date; every reasonable step must be taken to ensure that personal data that are inaccurate, having regard to the purposes for which they are processed, are erased or rectified without delay (‘accuracy’)” (Article 5(1)(d), ICO accuracy guidance).

For EU GDPR, Article 16 provides a right to obtain rectification without undue delay. The text is available at GDPR Article 16. The ICO’s published guidance notes that it is under review following changes made by the UK Data (Use and Access) Act. Check current law, commencement provisions, regulator guidance, exemptions, and applicable deadlines for the jurisdiction and processing at issue rather than assuming these summaries settle every case.

Using a vendor does not, by itself, remove the organisation responsible for the client-facing processing from its rights obligations. The ICO says outsourcing does not remove the controller’s responsibility to handle individual rights. Map where the provider stores inputs and outputs, how corrections are transmitted, and whether derived records or retained copies can be updated. When selecting services, confirm that their processes and technical controls support rights handling; the ICO discusses this in its guidance on individual rights in AI systems.
