A citation that opens can still be wrong. To fix citation failures in an AI research agent, check three things separately: whether the reference resolves, whether the source is relevant, and whether the cited passage supports the exact claim. Then constrain citations to retrieved evidence, repair or remove unsupported claims, and test the system for attribution errors—not just plausible answers.
Why an AI citation can fail in three different ways
“The link works” is only a link-health check. A reliable citation must also point to relevant evidence and support the full statement attached to it.

- Broken: The URL does not resolve, leads to an error, or no longer identifies the intended page.
- Irrelevant: The page opens but does not address the claim, or the cited passage is about a different point.
- Unsupported: The source is related, but its text does not establish the claim as written. A citation can also support only one part of a compound sentence.
These failures need separate checks. A URL validator cannot determine whether a passage proves a claim, and a semantic checker cannot establish that the model was actually given the source it cites.
1. Keep a registry of evidence the agent actually saw
At retrieval time, create a record for each source and passage supplied to generation. A useful record includes a canonical source ID, final URL, title, retrieval time, source type, and the exact passage or chunk placed in context. For cached or multi-turn systems, record whether the evidence was freshly fetched or served from cache.

Keep this registry separate from citation text generated by the model. NVIDIA’s deep-researcher architecture describes a per-session source registry for retrieval URLs and citation keys, followed by a check that report references appear in that registry.
2. Make citations reference IDs, not invented metadata
Ask the model to attach an internal source ID to each claim. Render that ID into a reader-facing citation using trusted registry metadata. The model should not author the URL, title, publication date, or identifier: those fields should come from the retrieval record.
If an ID cannot be matched to retrieved evidence, do not display the citation. Retrieve and verify the source, or present the point as unverified or omit it. Anthropic’s search-result content format illustrates how source URL and title metadata can accompany result text so citations can link to supplied material.
3. Validate links and citation records mechanically
Run deterministic checks before evaluating meaning. Confirm that each citation ID exists, the URL is well formed, and the page resolves. Match against the registry conservatively: overly permissive matching can attach a reference to the wrong page. NVIDIA documents exact and normalized URL matches, as well as constrained prefix, child-path, and query-subset matching; unmatched citations are removed and an audit reason is recorded in its implementation.
When a URL fails, distinguish a source that was retrieved and later moved from a URL with no evidence of having existed. The 2026 urlhealth preprint describes using URL-liveness checks and Wayback Machine information to classify stale versus likely fabricated URLs. That is a classification approach, not proof that every dead link is fabricated or every archived page supports the claim.
4. Check each claim against its cited passage
Break long answers into atomic claims that can be checked individually. Evaluate each claim against the cited passage or chunk—not just the page title or abstract—and require evidence for the whole claim. If a sentence combines multiple facts, split it or give each fact its own support.

NIST’s evaluation probes distinguish three useful dimensions:
Quick wins for a faster PC:
Clear out junk files and repair common Windows errorsFree Scan →Scan for outdated or missing drivers - takes under a minuteDriver Scan →- Faithfulness: Does the cited source support the claim?
- Completeness: Does the claim preserve the source’s full message rather than cherry-picking a fragment?
- Sufficiency: Is the evidence strong enough for the claim being made?
These dimensions are described on NIST’s Building Evaluation Probes into Agentic AI page. Google Cloud’s grounding check documentation describes linking claims to cited chunks and assigning support scores; its guidance says perfect grounding requires each claim to be supported by one or more supplied facts. A score or automated verdict is a check to review, not proof that the answer is true.
5. Repair the claim as well as the citation
Choose the repair based on the failure. Replacing a bad link without rechecking the claim can leave the underlying error untouched.
Rank #4
| Failure | Repair |
|---|---|
| Retrieved URL is now stale or broken | Find a current authoritative source, verify its relevant passage, and rerun the claim check. Do not substitute a search snippet as if it were the source. |
| URL resolves but source is irrelevant | Retrieve a more direct source and check that its passage supports the claim. |
| Source supports only part of the claim | Narrow or split the claim, add evidence for the missing part, or remove the unsupported detail. |
| Evidence is weak or sources conflict | Qualify the claim, explain the disagreement where it matters, or abstain. |
| No retrieved source supports the point | Retrieve evidence before asserting it, or leave the point unverified or out of the answer. |
Record the result explicitly as supported, revised, replaced, or removed, with a reason. Avoid arbitrary confidence labels unless they have been calibrated against the system’s actual performance.

A regression set should test whether the agent cites the right evidence, not only whether its response sounds correct. Include:
- Facts that appear in only one source, so a test can verify source attribution.
- Similar facts in competing documents, to catch incorrect source selection.
- Updated and outdated versions of a source, to test whether the agent uses the appropriate evidence.
- Questions for which none of the available sources contains the answer, to check that the agent abstains instead of inventing support.
Microsoft’s knowledge-grounding and accuracy scenario library recommends unique markers and source-attribution checks. An answer that happens to be right but cites the wrong source is still a grounding failure.
Best Value
Track link validity, relevance, entailment, completeness, and sufficiency separately over a fixed evaluation set. NIST says probes can run during the active workflow or after generation; NVIDIA documents post-processing verification. Teams can use either point in the pipeline or both, but should keep URL/provenance checks distinct from semantic checks.
7. Preserve an audit trail that explains the disposition
For each claim, retain its text, source ID, exact supporting span, URL-check result, semantic verdict and rationale, model or evaluator version, and final disposition. This lets an engineer trace a bad citation from the response back to the passage and see whether it was rejected, revised, replaced, or allowed through.
NIST describes structured audit trails that map agent decisions to evidence, while NVIDIA documents logging citation-verification decisions. Use deterministic validation for provenance and URL syntax; use rubric-based semantic evaluation, with human review for consequential claims.

Reported error rates vary by benchmark and evaluation method; they are not universal rates for AI research agents.
- Authors of the 2026 urlhealth study report that 3–13% of citation URLs were hallucinated and that 5–18% did not resolve in their evaluated DRBench and ExpertQA data. In that study’s agentic self-correction experiments, urlhealth reduced non-resolving URLs by 6–79× to under 1%; the outcome depended on the model’s tool-use ability. These are study-specific results, not a guarantee for another system.
- Authors of the 2026 Cited but Not Verified preprint report factual accuracy of 39–77% for evaluated systems, even where link validity exceeded 94% and relevance exceeded 80%, under that paper’s benchmark and evaluation method. The paper also reports an approximately 42% average drop in fact-check accuracy as tool calls increased from 2 to 150 for two tested frontier models. This experiment does not establish that additional retrieval generally makes citations worse.
Google Cloud documents its grounding check as designed for latency below 500 ms. That is a vendor-specific documented characteristic, not a general benchmark for grounding systems.
