Create a documented route that lets people report suspected AI-related harm, gets reports to an accountable team, and connects triage to investigation, response, recovery, communication, and improvement. NIST’s AI Risk Management Framework (AI RMF) 1.0 offers voluntary guidance for designing that process; it does not define a universal incident form, severity scale, response deadline, or legal reporting duty. Your process should fit the AI systems you use, the people they affect, your risk tolerance, and the requirements that apply to your organization.
What an AI incident reporting process does
An AI incident reporting process is the organizational path from noticing a suspected problem to deciding what happened, limiting harm, restoring safe operations, informing relevant people, and changing the system or its controls when needed. It covers more than a mailbox or web form: reports need an owner, an agreed way to assess urgency, access to people who can investigate, and authority to intervene.

Incidents may involve an AI system’s output or operation, the way it is used, or a connected service. The kinds of reports your organization chooses to handle could include a harmful or misleading output, an unreliable decision, privacy or security concerns, misuse, or a failure at a third-party provider. These are useful categories to consider, not a universal taxonomy or definition of an AI incident. State what your process covers and provide a route for uncertain cases rather than requiring reporters to classify them correctly.

Governance establishes context, policies, and accountability; the other functions help an organization understand its systems and risks, evaluate them, and manage prioritized risks. The framework and its Playbook are voluntary guidance, not a substitute for applicable law or a mandatory checklist. NIST AI RMF overview and the NIST AI RMF Playbook explain that context.

1. Set ownership, scope, and decision authority
Before inviting reports, decide which systems and uses are in scope and who is accountable at each stage. Start with an inventory of AI systems. For each one, record its purpose and deployment context, affected groups, internal operators, external providers, and important dependencies. This lets an intake owner route a report to someone who can recognize the system and its operating conditions. NIST emphasizes system inventories, documented responsibilities and communication paths, and attention to third-party risks in the AI RMF Core.
Assign named roles or teams, with a backup for each role. One person may hold several roles in a small organization, but the responsibilities still need to be explicit. The table is a practical allocation, not a structure prescribed by NIST.
| Role | What it owns | Decision or handoff to define |
|---|---|---|
| Intake owner | Receives reports, acknowledges them where possible, records the initial facts, and routes them. | Who can access the report and who takes over if the named owner is unavailable? |
| Triage lead | Assesses urgency, potential impact, uncertainty, and needed expertise. | Who can escalate a report immediately without waiting for routine review? |
| Investigation lead | Coordinates fact-finding, evidence preservation, and review of system behavior and context. | Which technical, operational, or subject-matter teams must participate? |
| Technical response owner | Implements containment, monitoring changes, rollback, override, or disengagement as appropriate. | Who can pause or change a system, and what approval is required in an urgent case? |
| Legal or compliance owner | Identifies potentially applicable reporting, privacy, contractual, or sector obligations. | Who determines whether an external notification is required and by when? |
| Communications owner | Coordinates accurate, audience-appropriate updates to affected people and other relevant actors. | Who approves communications, and who contacts affected groups? |
| Change approver | Reviews and authorizes system or process changes after an incident. | What evidence and testing are needed before a changed system returns to use? |
Document how these owners reach one another, how reports move between them, and who has authority to make urgent decisions. Include the process in relevant operating procedures so it does not depend on one employee remembering an informal escalation chain. NIST’s Governance function emphasizes documented roles and organizational practices that enable incident identification and information sharing. See the AI RMF Core.
2. Make reporting accessible to the people who may notice a problem
Offer a clear route for employees and other relevant AI actors, including users or people affected by an AI-supported service where appropriate. Depending on the organization, intake could use a form, help desk, safety contact, customer-support route, or escalation channel. Those are implementation options, not channels mandated by NIST. Make sure the route is visible where people encounter the system, and provide an alternative for someone who cannot use the primary channel.

Tell reporters what to flag, what details could help, what happens after submission, and how they can provide additional information. Invite reports of suspected problems and near misses, not only confirmed harm. Do not make the reporter prove that AI caused an event before the organization will review it; initial reports are leads for assessment. If anonymity is supported, explain its limits, including that investigators may be unable to ask follow-up questions or verify some details.
Design intake so that a lack of technical vocabulary does not block a report. A person should be able to describe what they saw, where they encountered it, and why it seemed wrong. NIST calls for mechanisms to capture and evaluate input from users and other relevant AI actors, including feedback and appeal processes, as part of evaluation and monitoring. The AI RMF Core describes these practices.
3. Capture useful facts without making the form a barrier
NIST does not prescribe one universal AI incident form or field list. A practical intake form should gather enough context to route a report and begin an investigation, while allowing an incomplete report to be submitted. Mark which details are essential for follow-up and which are optional; do not require information that a reporter is unlikely to know.

Restrict access to reports according to their contents and the work required to investigate them. Set a method for preserving relevant evidence and recording decisions. These are operational design choices derived from the need to capture and evaluate input and to track, respond to, recover from, and document incidents and errors; they are not a NIST-mandated form specification. 4. Triage reports using documented risk and urgency criteria
Define a triage method before the first serious report arrives. It should account for potential impact, likelihood or evidence of recurrence, urgency, your organization’s risk tolerance, and the response resources available. A report can be urgent even when the cause is unclear—for example, when potential harm could continue while the team waits for confirmation. Record the reasoning and route uncertain, high-impact cases to an appropriate decision-maker.
Organizations can use working categories to help select expertise and response paths. They are not categories required by NIST, and one event may fall into more than one:
- Safety or rights impact: possible physical, financial, or other significant harm, or an effect on access, treatment, or opportunities.
- Privacy or security: suspected exposure, unauthorized access, or misuse of data or system access.
- Reliability or performance: unexpected behavior, degraded performance, or a failure to operate as intended.
- Harmful or misleading output: an output that could cause harm or mislead a user or decision-maker.
- Misuse or unexpected use: behavior that indicates the system is being used in a way the organization did not anticipate or authorize.
- Third-party dependency: a provider, model, data, or connected service failure that affects an AI-enabled workflow.
Set escalation triggers that match the system and context rather than borrowing a universal severity scale. Examples of criteria to consider include credible potential for continuing harm, multiple affected people, an important service becoming unreliable, sensitive information being implicated, or an event involving a system for which the organization has little operational control. For each trigger, specify who must be alerted, what immediate protective action they may authorize, and how to handle after-hours reports. NIST directs organizations to prioritize and respond to assessed risks and to manage risks associated with third parties; it does not establish these example thresholds. AI RMF Core.
5. Investigate, contain, and recover
After triage, appoint an investigation lead and set the investigation’s scope. Establish what is known, what remains uncertain, which system version and deployment context are involved, and whether the event can recur or is ongoing. Preserve relevant logs, outputs, configuration details, and report materials under your organization’s applicable information-handling rules. Keep a record of findings and decisions, including when evidence is incomplete.

Containment should be proportionate to the potential risk and the consequences of interrupting the service. Depending on the incident, options may include closer monitoring, limiting a feature or use, routing a decision to a human, overriding an output, rolling back a change, or disengaging the system. Decide in advance who can authorize each type of intervention and who assesses whether harm is continuing. A system pause or shutdown is not automatically the right response in every context; weigh the risk of continued operation against the effects of interruption and follow applicable operational and safety procedures.
Recovery is more than turning the system back on. Identify what must be resolved or controlled, what evidence supports resuming use, and who approves the decision. If a system or process changes, use change management to record what changed and assess the risk again. If continued use is not appropriate, consider decommissioning or disengagement. NIST Manage 4.1 covers post-deployment monitoring, appeal and override, decommissioning, incident response, recovery, and change management. See Manage 4.1 in the AI RMF Core.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.6. Communicate with affected people and relevant AI actors
Decide who needs information, who should provide it, and how updates will be approved. Relevant audiences may include affected people or communities, users, operators, internal decision-makers, and external providers involved in the system. The audience depends on the incident and applicable requirements; a report to an internal team does not by itself answer whether others need to be informed.

7. Review the incident and turn lessons into changes
Once immediate response work is stable, review how the event occurred and what conditions contributed to it. Include operational context, human decisions, system behavior, monitoring, and relevant third-party dependencies. Identify groups that were affected or may have been affected, and distinguish evidence from assumptions.
Assign corrective actions with an owner and follow-up date. Actions may involve revising controls, monitoring, documentation, training, appeal routes, provider oversight, or the system itself. Update relevant risk assessments and check that changes do not introduce new risks. Track unresolved actions to completion and feed lessons into ongoing monitoring. NIST frames AI risk management as continuous and calls for continual improvement; incident learning is useful only when it changes the organization’s understanding or practice where warranted. NIST AI RMF overview and AI RMF Core.
8. Map legal, regulatory, and contractual duties separately
Do not treat alignment with NIST AI RMF as proof that your organization has met every reporting duty. The framework is voluntary guidance; applicable notification requirements, recipients, and timing depend on the organization’s jurisdictions, sector, contracts, and the facts of an event. Assign counsel or a compliance owner to map those requirements for the systems in scope and make the relevant escalation route part of the incident process.
NIST SP 800-53 control IR-6 says incident reporting authorities, content, and timeliness should reflect applicable laws, directives, regulations, policies, standards, and guidelines. That is a reason to maintain a tailored obligations map—not a universal deadline or list of recipients for every AI incident. NIST SP 800-53 IR-6: Incident Reporting. The AI RMF itself does not settle those legal questions. NIST AI RMF overview.
9. Choose an intake and triage design that fits your organization
There is no single right architecture. Pick an arrangement that makes reporting reachable, keeps accountability clear, and gets specialized issues to the people who can act. The options below are design choices, not a NIST comparison or ranking.
| Design choice | Useful when | Trade-off to address |
|---|---|---|
| Internal intake | Employees or operators need a direct route to the people responsible for systems. | People outside the organization may not know how to report, so provide an appropriate external route where relevant. |
| External-facing intake | Users, customers, affected people, or partners may be the first to observe an issue. | Reports need a clear internal owner and routing rules to avoid landing in an unmonitored channel. |
| Identified reporting | Follow-up questions and case updates are important and reporters are comfortable providing contact details. | Some people may hesitate to report; explain how contact information will be handled. |
| Anonymous option where feasible | Barriers to reporting could prevent useful concerns from reaching the organization. | Investigators may not be able to clarify facts or tell the reporter what happened next. |
| Centralized triage | A consistent intake and escalation point is valuable across several systems or teams. | The central team needs enough system knowledge and fast access to technical and operational owners. |
| System-level triage | Specialized teams can assess reports quickly within a bounded system or service. | Define common escalation rules and oversight so similar risks are not handled inconsistently. |
| Provider-assisted investigation | A third party controls logs, model components, or service operations needed to investigate. | Clarify contacts, evidence access, and contractual escalation paths; retain organizational ownership of decisions affecting your users. |
10. Put the process into operation and keep it usable
A written procedure only helps if people can find it and the organization can exercise it. Validate the process using realistic scenarios for the systems in your inventory. Check whether a reporter can locate the route, whether the intake owner can identify the system, and whether urgent reports reach someone empowered to act. These are practical readiness checks, not a prescribed NIST test or guarantee of outcomes.
- Publish the reporting route to the audiences expected to use it, with a clear description of what belongs there.
- Confirm that every in-scope system has a current owner, provider contact where relevant, and escalation path.
- Check that after-hours coverage and backup decision authority match the risks of the deployment.
- Confirm that records can be tracked from intake through response, recovery, communications, and corrective actions.
- Review the procedure when systems, providers, uses, affected groups, or applicable obligations change.
- Use feedback, appeals, incidents, and near misses to improve monitoring and the process itself.
Keep a short operational procedure for reporters and responders, backed by more detailed internal instructions where needed. The core should make it obvious how to report, who receives the report, how urgent concerns are escalated, and how the organization will track action. NIST’s Playbook offers suggested actions to help apply the framework, but it is not a mandatory checklist. NIST AI RMF Playbook.
