Build the fallback around the business function, not the AI product. Define the work that must continue, the harm that an incorrect or missing output could cause, the observable conditions that trigger a bypass, the people authorized to activate it, and a tested alternate workflow. That workflow may be manual, human-reviewed, non-AI, redundant, reduced-capacity, or a controlled suspension. Document how work is reconciled and how normal service is restored; a promise to “use human judgment” is not an operating plan.
Start with the work and the consequences of failure
An AI outage is an operational problem only because it interrupts a business process. Begin by naming that process and deciding what “good enough to continue” means. The National Institute of Standards and Technology (NIST) AI Risk Management Framework (AI RMF) calls for mapping business context, impacts, system components, third-party risks, and the system’s validity, reliability, safety, and resilience. The framework is voluntary guidance, not a universal legal requirement.

For each AI-supported function, record the following before choosing a backup:
- Function and service level: What task does the system perform, for whom, and by when?
- Owner: Which role is accountable for the process, even when another team operates the model?
- Inputs and dependencies: Which data feeds, identity systems, applications, model versions, vendors, networks, and staff are required?
- Downstream decisions: Does the output trigger a payment, safety action, customer communication, access decision, compliance record, or another consequential event?
- Unacceptable outcomes: Which errors, omissions, delays, privacy exposures, or unsafe recommendations require the AI path to stop?
- Continuity priority: What must continue immediately, what can run at reduced volume, and what may be paused?
- Constraints: What staffing, training, legal, contractual, geographic, accessibility, or data-retention limits apply to the alternate process?
Separate availability from reliability. A service can respond normally while producing answers outside its validated quality range, contradicting itself, using stale data, or operating beyond its intended use. The fallback must cover both a hard outage and a working-looking system whose outputs are unsafe to trust.

Map the system boundary and impact
Make a one-page system map that a responder can understand during an incident. Include the AI component, the application that calls it, data sources, storage, authentication, human reviewers, external providers, and every process that consumes the output. Mark which components are shared with the proposed fallback. A second AI service that depends on the same identity provider, data feed, network, or vendor is not independent redundancy.
Classify the impact of interruption and error separately. A low-risk drafting assistant may tolerate a queue and later review. A system that recommends a medical, financial, employment, security, or safety action may require immediate human control or suspension. Do not assume that a human path is automatically safer: people need the right information, time, training, supervision, and workload limits.
| Planning field | Question to answer | Evidence to keep |
|---|---|---|
| Essential output | What result must still be produced during an incident? | Required fields, acceptance criteria, and deadline |
| Maximum tolerable delay | How long can the function wait before harm or material loss occurs? | Business impact analysis and decision owner |
| Quality boundary | What error, confidence, freshness, or consistency condition is unacceptable? | Validated ranges, test cases, and escalation rule |
| Data exposure | What information may be copied, viewed, stored, or transmitted in the alternate path? | Access rules, retention period, and approved tools |
| Capacity | How many cases can the fallback handle per hour or shift? | Staffing model, queue limit, and prioritization method |
Set observable activation triggers and authority
Replace “switch when the AI seems wrong” with conditions someone can detect and record. Suitable triggers depend on the system; examples include:
- the service is unavailable or authentication fails;
- requests repeatedly fail, time out, or take too long for the business deadline;
- output quality falls outside a validated range;
- monitoring detects drift, stale inputs, data corruption, or a broken dependency;
- two outputs conflict in a way the process cannot safely resolve;
- a harmful, prohibited, or clearly out-of-scope result is produced; or
- the system is being used for a task, population, geography, or data type outside its approved purpose.
These examples are implementation choices, not universal NIST thresholds. Set values from your own impact analysis and testing. For instance, “three failed requests in ten minutes” is meaningful only if the owner has established that condition as a safe activation point for that service.
Write the authority chain in plain language:
- Detect: Identify the role or automated monitor that notices the condition and opens an incident record.
- Decide: Name who may pause AI use or activate the fallback, including an on-call substitute outside business hours.
- Protect: State which actions must stop immediately, such as automatic approvals, outbound messages, or irreversible transactions.
- Escalate: Specify when security, privacy, legal, safety, executive, or vendor contacts must be informed.
- Record: Capture the trigger, time, affected version or dependency, cases in progress, and the person who authorized the change.
NIST’s AI RMF calls for human oversight and mechanisms to supersede, disengage, or deactivate systems whose performance or outcomes conflict with intended use. The control should be usable under pressure: a documented switch, permission, queue status, or feature flag is better than an informal request in a chat channel.
Choose and document the alternate workflow
Select the least risky path that preserves the essential objective. State exactly what information the alternate operator receives, which steps change, what capacity is available, and how work in progress is handled.

Manual processing
A trained person performs the task using an approved procedure, source data, and decision record. Manual processing is appropriate when volume is manageable and the work can be specified clearly. Define segregation of duties for sensitive actions, a second check for high-impact cases, and a queue limit so staff do not silently work beyond safe capacity. NIST’s Generative AI Profile, published July 26, 2024, recognizes manual processing as a possible rollover or fallback.
Human review of every AI output
Keep the AI available for low-risk assistance but require a qualified reviewer to verify every output before it affects a person, account, payment, record, or external communication. The reviewer needs the original inputs, relevant source material, a way to reject or edit the result, and a rule for escalating uncertainty. This option fails if review is nominal, rushed, or performed by people without the required subject knowledge.
A simpler non-AI procedure
Use a deterministic rule, form, checklist, lookup table, or established business process. It may be slower and less personalized, but its behavior can be easier to explain and audit. Document any reduced service level and the conditions under which a case must wait for specialist handling. The AI RMF recommends considering viable non-AI alternatives rather than treating automation as the only path.
An independent redundant service
A second service can preserve capacity, but independence must be demonstrated. Assess its model or rules, data sources, infrastructure, security, privacy, contract, monitoring, and failure modes. Confirm that it is suitable for the same population and task. Do not label another AI model “safe redundancy” merely because it has a different brand or endpoint.
Controlled suspension
Stopping the affected function is the correct fallback when every available alternative creates unacceptable risk. Provide a safe holding state: preserve the request, tell the affected person what will happen next, prevent duplicate submissions, and define the owner and deadline for reassessment. A plan should never promise uninterrupted service when safe operation is impossible.
Compare options against the same decision criteria
When several alternatives are viable, score them with the process owner and the people who will operate them. The following criteria keep the comparison focused on risk and recovery rather than convenience:

- [Dont Hesite to Order] 24 hours customer services and professional technology team are standing by
| Criterion | Questions |
|---|---|
| Safety and residual risk | Does the option avoid the harm that made the AI path unacceptable? What new human or technical errors can it introduce? |
| Continuity and capacity | Which essential functions continue, at what volume, speed, and prioritization? |
| Activation and recovery time | How quickly can staff switch, and how long will it take to return to normal? |
| Human workload and skill | Are enough trained people available, including outside normal hours? What supervision and fatigue controls are needed? |
| Dependencies | Does the option rely on the same vendor, data, identity system, network, facility, or failure-prone component? |
| Data handling and security | Will copying, exporting, printing, or sending data create new privacy or access risks? |
| Cost and maintainability | Can the organization keep the procedure, access, training, licenses, and contact information current and rehearse them? |
Record why the selected option is proportionate to the possible harm. If no option meets the risk tolerance, choose suspension and escalation instead of accepting an undocumented exception.
Assign operational roles, not just departments
List a named role and a backup for each action. NIST SP 800-34 Rev. 1 provides general contingency-planning guidance and business-impact-analysis templates; it is federal information-system guidance, not an AI-specific operating standard, so adapt it to your environment.
- Detection lead: Confirms the trigger, gathers initial evidence, and opens the incident.
- Fallback authority: Authorizes bypass, deactivation, or suspension and records the decision.
- Process operator: Runs the alternate workflow and applies its quality and capacity limits.
- Case or queue owner: Prioritizes work, prevents duplicates, and tracks every item through completion.
- Safety, privacy, security, or legal adviser: Handles regulated data, harmful outcomes, suspected compromise, or mandatory notices.
- Communications lead: Tells users, customers, partners, and affected parties what changed and what action they should take.
- Vendor coordinator: Manages the provider incident, status information, contractual contacts, and evidence without making restoration claims the provider has not confirmed.
- Restoration approver: Confirms that return-to-service checks passed and authorizes re-enabling AI.
Put phone or paging routes, permission requirements, time-zone coverage, and substitutes in the runbook itself. A role that exists only on an organizational chart is not available during an incident.

Write the incident runbook as executable steps
The operator should be able to follow this sequence without interpreting policy.
- Confirm the condition. Check the service status, recent errors, latency, input freshness, output samples, and affected scope. Preserve logs and representative cases.
- Contain unsafe effects. Disable automatic actions, outbound publication, or irreversible transactions that depend on the suspect output. Keep already completed actions identifiable for later review.
- Declare the mode. The authorized person records whether the organization is in outage fallback, unreliable-output review, reduced capacity, or suspension.
- Protect data. Use only approved storage and transfer methods. Minimize copied information and set an explicit retention and deletion action for temporary files.
- Open the alternate queue. Give each case a unique identifier, arrival time, priority, required inputs, assigned operator, and next deadline.
- Apply triage rules. Process safety-critical, legally time-bound, vulnerable-person, or financially material cases according to the documented priority. Route cases lacking required information to the stated escalation path.
- Perform the alternate task. Follow the manual, non-AI, review, or redundant-service procedure. Require the specified second check and record the decision basis.
- Communicate the change. Tell internal users what is unavailable, what they must do differently, expected delays, and where to report a harmful or incorrect result. Give external parties a truthful status and a safe next step.
- Monitor capacity and harm. Track queue size, age, error reports, staff workload, and any near miss. Escalate before the fallback exceeds its tested limits.
- Prepare reconciliation. Mark each case as completed, awaiting information, duplicated, rejected, or needing comparison with a later AI result. Do not allow automatic replay to create duplicate actions.
Define restoration and reconciliation before the outage
Returning the AI to service is a change-management decision, not simply noticing that an endpoint responds. Specify the checks required for each trigger type:
- the provider or internal team has identified and addressed the original failure;
- dependencies, authentication, data freshness, model version, configuration, and security controls are verified;
- representative test cases meet the approved quality and safety boundaries;
- any harmful, inconsistent, or out-of-scope behavior has an owner and disposition;
- the restoration approver has reviewed incident evidence and authorizes the change; and
- users are told when the operating mode changes and which backlog or temporary rules remain.
Reconcile work completed during fallback in a controlled order. Match records by unique identifier, compare timestamps and inputs, resolve conflicts with the designated owner, and preserve the manual decision where it is safer or legally required. If the AI output is only advisory, do not overwrite a human-approved result without an explicit review. If a case was suspended, notify the responsible person before resuming it.

Exercise design
- Run a announced tabletop to confirm that staff can find the runbook, recognize the trigger, and name the decision authority.
- Run a controlled technical test of access, queue creation, permissions, notifications, and data handling.
- Use a scenario in which the service is available but produces conflicting or harmful results; verify that people can disengage it rather than waiting for an outage.
- Measure whether essential work continues at the capacity promised in the plan, without hiding backlog or unsafe workload.
- Include nights, weekends, vendor handoffs, staff absence, and a second simultaneous dependency failure where the impact analysis says they are plausible.
The NIST Generative AI Profile recommends regularly rehearsing third-party incident-response plans and improving them through retrospective learning. After each exercise or real event, record what happened, which trigger was reached, how long activation took, where work accumulated, and which control prevented or failed to prevent harm.
Ongoing monitoring
Monitor deployed-system performance and user feedback, not only uptime. Track failures, latency, quality indicators, drift, override and appeal activity, harmful outputs, and cases sent to the fallback. Review whether thresholds still match the risk and whether staff can operate the alternate path at the promised volume.
NIST’s AI RMF Playbook advises regular review of bypass actions, incident thresholds, and the consequences of bypass or deactivation. Update the plan after model, prompt, data, process, vendor, infrastructure, staffing, regulation, or risk-profile changes. Retire obsolete procedures and access rights; an old fallback can become a new security or safety hazard.
Illustrative designs for different risk levels
Low-risk document classification
If an internal classification tool is unavailable, staff can use a published taxonomy and place uncertain documents in a review queue. The trigger may be service failure or an error rate outside the validated sample range. The fallback owner should cap the queue and defer nonessential classification rather than forcing uncertain labels.
Customer communication drafting
If a drafting assistant produces contradictory or unsupported text, stop automatic publication. A trained employee uses approved factual sources and a communication checklist, with a second reviewer for regulated or high-impact messages. Preserve the draft and source record so the incident can be analyzed without sending the suspect text.
Best Value
High-impact recommendation
If an AI system contributes to a decision that could cause significant personal, financial, health, or safety harm, the fallback may require a qualified human process or a pause. The operator should not substitute a different AI system until its suitability, dependencies, security, and review controls have been assessed. If trained capacity is unavailable, suspend the function and communicate the next safe step.
Common plan failures and their fixes
- “Use human judgment” with no procedure: Specify the operator, inputs, decision criteria, second check, queue limit, and escalation route.
- Only testing a total outage: Add scenarios for plausible but unreliable answers, stale data, drift, conflicting results, and out-of-scope use.
- Using uptime as the sole health signal: Monitor quality, safety, latency, data freshness, overrides, and harmful outcomes.
- Assuming a second AI is independent: Map shared vendors, data, identity, infrastructure, security controls, and failure modes.
- Ignoring capacity and fatigue: Test real staffing, set a queue ceiling, rotate reviewers where needed, and define when to suspend work.
- Leaving restoration vague: Require evidence, test cases, an approver, reconciliation, and a user communication before re-enabling AI.
- Failing to preserve incident evidence: Keep timestamps, versions, inputs, outputs, decisions, overrides, and communications under the approved retention rules.
- Promising uninterrupted service: State clearly when the safe outcome is delay or suspension.
Use NIST guidance without treating it as a universal rulebook
The NIST AI RMF Core is an excerpt from AI RMF 1.0 (2023). Its Measure 2.6 says: “The AI system to be deployed is demonstrated to be safe, its residual negative risk does not exceed the risk tolerance, and it can fail safely, particularly if made to operate beyond its knowledge limits.” It also says: “Safety metrics reflect system reliability and robustness, real-time monitoring, and response times for AI system failures.” These are framework statements, not a single legally mandated checklist. NIST’s AI Risk Management Framework overview identifies the framework as voluntary and notes that it is being revised; verify the applicable version and obligations for your organization.
For generative systems, use the NIST Artificial Intelligence Risk Management Framework: Generative Artificial Intelligence Profile (July 26, 2024) for guidance on rollover, manual processing, third-party incident response, monitoring, and learning from incidents. For broader continuity planning, consult NIST SP 800-34 Rev. 1 (November 11, 2010), adapting its federal information-system context rather than presenting it as an AI operating standard. The NIST AI Risk Management Framework Playbook (March 30, 2023) provides practical prompts for reviewing bypass actions, thresholds, incident response, recovery, and change management.

Before approval, verify that the plan contains all of these items in one accessible location:
- the AI system, supported process, owner, scope, dependencies, and affected parties;
- essential work, acceptable delay, capacity limits, quality boundaries, and unacceptable harms;
- observable outage and unreliability triggers with a documented activation authority;
- the selected alternate workflow, its data and security controls, and its limits;
- roles, backups, contact routes, vendor responsibilities, and escalation criteria;
- step-by-step activation, queue, communication, evidence, and workload instructions;
- restoration tests, approval authority, reconciliation rules, and user notification;
- exercise results, monitoring measures, review dates, and change triggers; and
- a clear suspension path when no alternative can operate within risk tolerance.
The practical test is simple: during a real failure, can the authorized person identify the trigger, stop unsafe automation, keep essential work moving at a tested capacity, protect data, and know exactly what evidence is required before restoration? If not, the organization has a policy statement, not a fallback plan.
