First, check the video’s shape: Telegram documents animated profile pictures as square MPEG4 videos, with a maximum resolution of 1080×1080 pixels and 800×800 recommended. If your iPhone’s HEVC clip is missing from Telegram’s picker or fails to upload, try exporting a square MP4 and testing that copy. H.264 is one compatibility-oriented option to try—not a format Telegram says is required.
What Telegram documents for profile videos
Telegram’s Core API file-upload documentation describes animated profile pictures as square MPEG4 videos. It gives 1080×1080 pixels as the maximum dimensions and recommends 800×800. The reference does not specify a required codec, an iPhone conversion method, or whether every Telegram client accepts HEVC.

Try the original first
If the clip is square, or can be cropped to a square without cutting off the subject, try selecting it in Telegram before converting. This avoids an unnecessary export and lets you see whether your current app accepts the file.

Export a square MP4 as a troubleshooting step
If Telegram does not show the HEVC clip or the upload fails, keep the original and make a separate square MP4 export. Aim for no more than 1080×1080 pixels; 800×800 is Telegram’s recommended resolution. Check the crop so the subject remains visible in the square frame.
If your export options include H.264, you can try it as a compatibility-oriented alternative. A Telegram issue report describes one user’s H.264 file and a file-specific upload problem; it does not establish H.264 as a universal requirement or guarantee that conversion will fix an upload. Telegram’s official reference does not publish a codec recipe.

Compare the original and converted copy by whether each appears in the picker, whether its upload completes, and whether the crop and still preview look right. The available official guidance does not establish a profile-video duration or file-size limit, audio requirement, bitrate, or exact iPhone export setting, so avoid treating any particular value as a Telegram rule.
Choose the still frame for chats
Before saving the profile video, select the frame that should represent you when the animation is not playing. Telegram documents a video-start timestamp for the static preview, and its feature announcement describes choosing a frame for the still image shown in chats. Check both the animated crop and the still frame; a good-looking clip can have an unhelpful preview if the chosen frame catches a blur or an expression you do not want.

If the video is missing from Telegram’s iPhone picker
Check whether Telegram can access the video in Photos. In iOS Settings, review Telegram’s Photos permission and make sure the clip is available under the access you have granted. Labels and menus can vary by iOS version.
A Telegram issue report documented videos disappearing from the profile picker with limited Photos access. Full Photos Access was listed as a workaround for that reported issue, which the page says was fixed in Telegram iOS 12.7.0. That history does not mean every current installation requires Full Access; use the least access that lets Telegram see the clip, and check that both iOS and Telegram are up to date.

If the clip appears but upload still fails, try the square MP4 copy and compare the results. A file-specific failure may have causes beyond its codec; Telegram’s reported troubleshooting discussion mentions encoding or bitrate as possibilities, but does not define a universal setting.
