Compare the care, not the “longevity” label: find out what a service includes, who is clinically responsible, what its tests can establish, and what happens when a result is concerning. A biological-age score is not a diagnosis or a reliable prediction of how long you will live, and a bigger test menu does not prove that a program improves health.
Start by identifying what the service actually does
“Longevity clinic” is not a uniform clinical standard. A provider using that label might offer preventive primary care, focused risk management, testing without ongoing care, or experimental interventions. A biological-age program may be a stand-alone consumer test or one component of a broader clinical service. Compare the specific services and their limits rather than the brand or label.

Ask for a written description of what is included, what costs extra, and what falls outside the program’s scope. A polished personalized plan or a long list of tests does not, by itself, establish clinical quality or benefit.
Use these questions to compare providers
| What to compare | Questions to ask | What a useful answer establishes |
|---|---|---|
| Service model | Is this preventive clinical care, specialty risk management, testing only, or an intervention program? What is not included? | Whether the service matches your needs and whether you will need a separate clinician or referral. |
| Clinical accountability | Who is the named clinical lead? Who can change or stop a service if it appears unsafe? | Who is responsible for protocols, care decisions and adverse-event response. |
| Credentials and scope | Who orders tests, interprets results, prescribes treatment and follows up? What requires referral? | Whether each task is handled by a qualified professional within their scope. |
| Test validity and utility | What does each test measure? In which population was it validated? How reproducible is it, and what decision would its result change? | Whether the measurement is dependable for its intended use and can inform a meaningful next step. |
| Evidence and intervention status | Is a proposed service standard care, guideline-supported, off-label, under formal study or unsupported for the advertised claim? | How established the intervention is for the outcome being promised. |
| Safety and follow-up | Who contacts me about an abnormal result? How are referrals, adverse events and ongoing care handled? | Whether there is a clear route from finding to appropriate clinical care. |
| Transparency | What is the total cost, including recurring charges and follow-up? Are conflicts of interest and limitations disclosed? Are outcomes reported with follow-up and adverse events? | Whether you can assess the service’s financial terms and claims; transparency alone does not prove effectiveness. |
Understand what a biological-age test measures
Biological age is an attempt to describe how aspects of the body are changing relative to chronological age. Methods differ: some examine DNA methylation patterns, while others use blood proteins, metabolic markers or wearable metrics. These methods do not measure one standardized quantity. Stanford Medicine notes that there is no single agreed-upon way to measure biological age, and different tests can produce conflicting results.
#1 Best Overall
- Aeternum DNA Biological Age Test
- Advanced Epigenetic Analysis: Uses state-of-the-art DNA methylation profiling to examine hundreds of thousands of sites on your genome, yielding an exceptionally precise measure of your biological age . This science was pioneered by top researchers and provides a deep look at how your cells are aging internally.
- Unmatched Accuracy & Reliability: Aeternum’s testing method is unmatched in its accuracy, built on next-generation epigenetic “clocks” for aging. Results are highly reproducible – you can test now and retest later, confident that any change reflects real progress and not lab error. This precision gives you a trustworthy baseline to measure the impact of lifestyle changes
- Third-Party Certified Labs: Your sample is analyzed in CLIA-certified laboratories (the same level of certification as medical labs), ensuring clinical-grade quality, objectivity, and data privacy. Our test kits and processes meet rigorous international standards, so you can trust the authenticity and security of your results.
- Comprehensive Results & Insights: Receive a detailed digital report that not only reveals your biological age but also provides context and guidance. We explain what was tested and what it means for your aging journey , including science-backed recommendations on how to lower your biological age through nutrition, exercise, sleep, and other lifestyle changes. It’s like getting a personalized roadmap to improving your healthspan.
For any specific test, ask which markers or biological processes it measures, how the measurement was validated, what can affect the result, and whether the test is intended for wellness information, research, diagnosis or a defined clinical decision. Also ask what action would follow a concerning result and who is qualified to interpret it.
FDA oversight depends on a test’s intended purpose and risk. FDA consumer guidance says some tests are not reviewed before marketing and advises discussing health-related results with a health care provider rather than using them alone to make medical decisions. The agency describes analytical validity, clinical validity and claims as relevant considerations; these are distinct questions:

- TARGETED INSIGHTS: Tailor your diet, supplements, and lifestyle to your genetic makeup for optimal health and performance.
- Analytical validity: Does the test measure the stated marker reliably?
- Clinical validity: Does that marker relate to the health state or condition the provider claims it reflects?
- Clinical utility: Does acting on the result improve a decision or health outcome?
A dependable measurement does not automatically make it a useful clinical tool. If a score would not change a decision, ask whether repeating the test adds useful information or mainly produces another number.
Do not read a score as a diagnosis or lifespan forecast
A biological-age score is not, by itself, a diagnosis, a definitive prediction of lifespan or proof that a program extends life. Stanford’s experts describe the relationship between biological age, lifespan and healthspan as an area still being worked out. A changed score or a plausible biological mechanism does not establish that a treatment improves a meaningful health outcome.

Stanford professor of genetics Anne Brunet cautioned: “People need to understand that it’s more complex than this one number, and that they should not freak out if a clock says they’re older than their calendar age,” as quoted in Stanford Report on September 23, 2026. If a result worries you, discuss it with a qualified health professional who can consider your overall health and the test’s limitations.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Separate established prevention from uncertain interventions
For each proposed treatment, ask what outcome the provider claims it improves and what evidence supports that claim in people like you. Get the intervention’s status in plain language: routine standard care, guideline-supported, off-label, being studied formally, or not supported for the marketed claim. Ask about known risks, alternatives, consent, monitoring and what would lead the clinician to stop it.

- Used Book in Good Condition
Do not treat a testimonial, an association, a proposed mechanism or a better biological-age score as proof of clinical benefit. A provider should be able to explain the difference between evidence that a marker changes and evidence that patients benefit.

Before paying, find out who reviews results, how quickly concerning findings are communicated, who arranges referral or further evaluation, and whether the provider remains involved afterward. Ask how urgent findings are handled, how adverse events are reported, and whether follow-up is included or billed separately. If a test identifies a problem outside the program’s scope, there should be a clear handoff to appropriate care.

Make the decision using the whole service
Compare the full service rather than the headline test or advertised treatment. A useful program should make its scope, responsible clinicians, evidence, limitations, risks, total costs and follow-up process understandable before you enroll. If the provider cannot explain what a test is meant to change or who will act on the result, treat that as a substantive gap—not something a longer test menu can fix.
