How to Compare Hospital Bills and Spot Duplicate or Unexplained Charges

Get an itemized bill, match charges to your records, and reconcile your balance with the insurer's EOB before deciding whether a line is a duplicate or error.
Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Start by getting an itemized bill, then compare each line with your medical records and, if you used insurance, the Explanation of Benefits (EOB). A repeated-looking charge or a code you do not recognize is a reason to ask questions—not proof of an error. Hospitals and clinicians may send separate bills for distinct services.

Gather the documents for the visit

Collect the itemized bill from each provider, every EOB tied to the care, relevant medical records, receipts or proof of payments, and—if you were uninsured or chose not to use insurance—the good faith estimate. A single hospital visit can generate separate bills from a facility and individual clinicians, and insurers may send separate EOBs by provider, facility, or date. Sort the paperwork by provider and date of service so you compare the right items.

Ask the provider’s billing office for a detailed list of services or items and their costs if your statement only shows a balance. CMS recommends an itemized bill as the starting point for checking accuracy: Check your medical bill for errors.

Read the bill before judging the balance

Check the billing date and date of service, item or service descriptions, charges, adjustments, payments already made, and remaining amount due. Adjustments can reflect a discount or lower charge agreed to by the provider; patient payments may include amounts paid at the visit, such as a copayment. Do not compare only the original gross charge with your EOB. Compare the final patient balance after adjustments and payments. CMS explains these bill fields in How to read your medical bill.

Check whether a charge matches the care

Review each line against what happened during the visit and what is documented. Ask yourself:

  • Does this item or service correspond to something I received?
  • Can I find it in the relevant medical record?
  • Does the billing-code description seem consistent with the service?
  • Does the same service appear elsewhere on this bill or on another provider’s bill?

Look up an unfamiliar code description and ask the provider to explain it if it does not seem to fit. A code mismatch by itself does not establish that a service was wrongly billed or unnecessary; the provider can clarify how the code relates to the care. If a billed item is missing from the record, ask the billing department to identify its documentation or review the charge. CMS says patients should not be billed for services that are not documented in their records.

How to spot a possible duplicate

Look for the same service listed more than once, including across statements from different clinicians or facilities. Compare the provider, date, description, and code—not just the amount. Different providers can legitimately bill for different parts of the same encounter, so two similar entries are not automatically duplicates. If the entries appear to describe the same service, contact each billing office and ask whether they are separate services or a repeated charge. CMS includes duplicate checks in its medical-bill error checklist.

Reconcile the bill with your insurance EOB

If you used insurance, compare the EOB’s services and patient-responsibility amount with the provider bill. The provider bill should agree with the amount the insurer says you owe for those services. If they differ, ask the insurer whether the claim was reprocessed or an adjustment is pending, and ask the provider how it calculated the balance. Save each EOB; a separate notice may cover another provider, facility, or treatment date. CMS advises paying only after you receive a provider bill and check that the services and amount owed agree with the EOB.

Ask the billing office specific questions

Call or write the billing office with the date of service, line description, and amount. Specific questions make it easier to get a useful answer:

Do not assume an ordinary billing question automatically pauses collections; protections depend on the issue and applicable rules. Record the name of the person you spoke with, the date, and any reference number. CMS offers guidance on talking to your provider about a medical bill. If the balance is correct but unaffordable, ask about financial assistance, a reduction, or a payment plan.

Know when federal billing protections may apply

The No Surprises Act took effect on January 1, 2022. It protects people with most types of health insurance from certain unexpected out-of-network bills, including specified emergency services, some non-emergency services connected with care at an in-network hospital, hospital outpatient department, or ambulatory surgical center, and air ambulance services. Whether a protection applies depends on the care and coverage circumstances. A deductible that has not been met, by itself, does not establish a violation. If you think a protection was violated, CMS directs insured patients to the plan’s appeal process or a complaint, depending on the issue. See CMS’s overview of medical bill rights.

If you were uninsured or chose not to use insurance

Providers generally must give a good faith estimate when care is scheduled at least three business days in advance or when you ask. A federal patient-provider dispute process may be available if the provider or facility charged at least $400 more than its estimate, but the amount alone does not guarantee eligibility. CMS lists additional requirements, including that you did not have or use insurance and told the provider in advance, the care occurred on or after January 1, 2022, you have the written estimate, and the initial bill is dated within the last 120 calendar days. The process has a $25 non-refundable administrative fee; CMS says it is deducted from the amount owed if the decision favors you. Check the current criteria and start the process through CMS’s dispute guidance.

While an eligible federal patient-provider dispute is underway, the provider or facility cannot move the bill into collections, threaten to do so, collect existing late fees, or take action against you because you are disputing the bill through that process. This protection is specific to that process; it should not be assumed to apply to every billing inquiry or insurance appeal.

If the issue is still unresolved

Ask your state’s Consumer Assistance Program or a patient advocate for help. CMS says advocates can handle billing issues on a patient’s behalf; its error-checking guide points to these resources. You can also ask the hospital about financial assistance and payment options. Hospital standard-charge information and nearby CPT-code prices may help with price comparisons, but they do not by themselves show whether a particular charge was billed correctly or what an insurer will ultimately allow.
