How to Choose Between Robotic-Assisted and Conventional Knee Replacement

Robotic assistance can improve some alignment measures, but evidence has not shown a consistent patient-noticeable advantage over conventional knee replacement. Here’s what to ask your surgeon.

For most adults considering total knee replacement for osteoarthritis, current evidence does not show that robotic assistance reliably leads to less pain, better function, fewer complications, or fewer revisions than conventional surgery. Robotic systems can improve some measures of implant alignment, but that advantage has not consistently translated into a meaningful difference patients can feel. The decision is best made with your surgeon, weighing their experience with the specific approach, your circumstances, and any added imaging, risks, cost, or access issues.

What is different about the two approaches?

In conventional total knee replacement, the surgeon prepares the bone with cutting guides and standard instruments. In robotic-assisted surgery, planning and navigation or robotic tools help guide parts of the procedure; the details depend on the platform. Some systems use a preoperative CT scan to create a three-dimensional plan.

In either approach, the surgeon performs the operation and places the prosthetic components. A robot does not independently perform the replacement or make the decision about your care. The term “robotic” describes assistance used during planning or execution, not an autonomous operation. The AAOS patient guide to robotic-assisted joint replacement describes potential considerations including imaging, navigation-pin fracture risk, added planning or operating time, cost, and availability. These vary by system and setting.

What does the evidence say about outcomes?

Alignment can be more precise

A 2023 systematic review of 12 randomized trials involving 2,200 patients found that robotic assistance likely reduced mechanical-alignment outliers (risk ratio 0.43, 95% confidence interval 0.27 to 0.67) and reduced average deviation from neutral mechanical alignment by 0.94 degrees. A 2024 meta-analysis of seven randomized trials and 1,942 knees also reported better postoperative anatomical and mechanical alignment with robotic assistance. These are measurements of implant or limb alignment, not proof of better pain relief or everyday function.

Better alignment has not consistently meant better results patients notice

In the 2023 review, patient-reported outcomes showed little to no difference (WOMAC mean difference −0.35, 95% CI −0.78 to 0.07), as did range of motion (mean difference −0.73 degrees, 95% CI −7.5 to 6.0). The 2024 analysis similarly found clinical and functional outcomes to be comparable. The review authors cautioned that the alignment improvement may not be clinically meaningful.

The American Academy of Orthopaedic Surgeons (AAOS) guideline on knee osteoarthritis states: “Evidence suggests no significant difference in function, outcomes, or complications in the short term between robotic assisted and conventional total knee arthroplasty.” The guideline rates the evidence quality as high but the strength of the option as limited, and notes that studies involve different robotic systems. It also recognizes that some higher-quality studies show improved accuracy and fewer alignment outliers. Read the AAOS knee osteoarthritis clinical practice guideline for its scope and recommendations.

Complications and revisions

Comparative evidence has not established that robotic assistance generally reduces complications or revision surgery. The randomized-trial review detected no difference in revision rates or major adverse effects, but said evidence on these outcomes was insufficient to rule out meaningful differences. A 2025 systematic review, based on literature searched through December 19, 2024, concluded that conventional surgery was non-inferior for short- and long-term implant survival, complications, and postoperative pain; it reported subtle improvements in some functional measures with robotics. This is one synthesis, not proof that every platform or patient has the same results.

A registry analysis reported by AAOS Now covered 9,220 people aged 65 or older who had cementless primary total knee replacement for osteoarthritis. At two years, all-cause revision was 1.16% with robotic assistance and 1.3% with conventional surgery; the difference was not statistically significant. Those figures apply to that older, cementless cohort and follow-up period, not to every knee replacement patient.

What trade-offs should you discuss?

  • Surgeon and system experience: Ask which robotic system would be used, how often the surgeon uses it, and why it fits your anatomy and operative plan. Experience with a particular approach is more useful to your decision than the label “robotic” by itself.
  • Imaging and pin-related considerations: Ask whether the system requires CT or other preoperative imaging, whether navigation pins are used, and what the associated radiation exposure or pin-related risks mean in your case. AAOS notes that robotic surgery may require preoperative imaging that exposes the patient to radiation.
  • Time and setting: Ask whether the technique changes planning or operating time at the facility where your surgery would take place. The effect can depend on the system and local workflow.
  • Cost and access: Find out whether robotic assistance changes your out-of-pocket cost or the facility charge, and whether it is available locally. In announcing its guideline, AAOS noted that the technology can add cost. Dr. Srivastava’s comment that “the extra cost might not be necessary” was conditional on current evidence showing no difference in outcomes; it is not a statement about every patient or a guarantee about what a procedure will cost.
  • Your health, anatomy, and goals: Ask how your individual circumstances and priorities affect the recommended plan. AAOS emphasizes that treatment should reflect the clinician’s independent judgment and the patient’s specific circumstances.

Questions to take to your surgeon

AAOS’s patient question list includes: “Would you use robotics to perform my surgery? What are the pros and cons of robotic-assisted total joint replacement?” You can make the discussion more specific with these follow-ups:

  • Which approach do you recommend for me, and why?
  • Which robotic system would you use, and how much experience do you have with it?
  • What differences do you expect, if any, in my pain, function, recovery, complication risk, or chance of revision?
  • Would my plan involve a CT scan, navigation pins, or other additional steps?
  • Would choosing robotic assistance change my bill or access to surgery?
  • How many knee replacements do you and this facility perform, and will I need a walker, crutches, or cane afterward?

How to make the choice

Start with the surgeon’s recommendation for your knee and the reasons behind it. If robotic assistance is proposed, ask what specific benefit the surgeon expects for your situation and distinguish alignment goals from outcomes such as pain, function, or avoiding revision. Then weigh that expected benefit against the system-specific imaging and procedural considerations, cost, availability, and the surgeon’s experience.

Evidence cited here concerns total knee arthroplasty for osteoarthritis. It should not automatically be extended to partial knee replacement, other diagnoses, every robotic platform, or every patient. A newer AAOS review listing surfaced in 2026, but its available listing did not provide enough detail to assess its methods or findings, so it cannot be treated here as changing the conclusions above.
