How to choose a nerve-stimulation treatment for chronic pain

Nerve stimulation is not one treatment. This guide explains how diagnosis, pain pattern, evidence, trial results, surgical suitability and daily device demands shape the choice between TENS, peripheral nerve stimulation and spinal cord stimulation.
Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

There is no single “best” nerve-stimulation treatment for chronic pain. The right choice starts with the diagnosis, where the pain is felt, how it affects function, and what you want treatment to change. A surface TENS unit, an implanted peripheral-nerve device and spinal cord stimulation (SCS) are different interventions with different evidence, procedures and responsibilities.

Use stimulation as one part of an individualized pain plan. Confirm that the proposed treatment fits your pain condition and the device’s current indication, understand whether a temporary trial is required, and decide in partnership with a clinician who can discuss benefits, risks, uncertainty and alternatives.

Start with the pain diagnosis, not the device

Chronic pain is generally pain that persists or recurs for more than three months. NICE distinguishes chronic primary pain, in which the pain itself is the main problem, from chronic secondary pain, which is caused by an underlying disease or injury; both can occur in the same person. That distinction matters because evidence for a stimulation method in one condition cannot automatically be transferred to another.

Your clinician should first establish the likely cause, whether the pain is neuropathic, nociceptive or mixed, and whether it is confined to a small territory or spread across a limb, trunk or back. The assessment should also cover sleep, movement, mood, work, medicines, previous procedures, other illnesses and your access to follow-up care. The National Institutes of Health notes that treatment depends on pain type, duration and likely cause, as well as age, other health conditions and access to care.

Write down what a worthwhile result would look like before discussing a device. Examples include walking farther, sleeping through the night, returning to work, reducing rescue medicines or tolerating a specific activity. A lower pain score can be useful, but function and quality of life should be measured as well.

What “nerve stimulation” can mean

The term covers treatments that differ in where electrical pulses are delivered and how much medical intervention they require.

Approach Where stimulation is delivered Typical care setting What must be established
TENS Electrodes on the skin Often used outside a hospital after instruction Whether the diagnosis has evidence for benefit and whether you can use the unit safely
Peripheral nerve stimulation (PNS) A lead under the skin near a named peripheral nerve Specialist procedural care That the pain distribution matches a targetable nerve and that implantation is appropriate
Peripheral nerve-field stimulation (PNFS) Implanted leads in tissue around a painful field rather than directly on one nerve Specialist procedural care, sometimes staged That the indication and local guidance cover the particular pain pattern
Spinal cord stimulation (SCS) Leads positioned near the spinal cord and connected to an implanted generator Multidisciplinary specialist service with a trial and, if successful, implantation A supported indication, surgical suitability, meaningful trial response and realistic long-term expectations

An implanted system is not an advanced version of a consumer TENS unit. It involves a procedure, follow-up, device management and possible future surgery. The fact that a product can be purchased does not establish that it is suitable for a particular chronic-pain diagnosis.

TENS: a surface option with diagnosis-specific limits

How it works

Transcutaneous electrical nerve stimulation sends mild electrical pulses through adhesive electrodes placed on the skin. The unit is external and can be removed, so there is no implanted lead or generator. The NIH describes TENS as something that may provide short-term relief for some people; that broad statement is not a recommendation for every diagnosis.

What the chronic-primary-pain evidence means

NICE guideline NG193 (2021) says not to offer TENS to people aged 16 and over to manage chronic primary pain. Its rationale reported no clinically important difference between TENS, sham TENS and usual care across several outcomes assessed at less than three months, and no longer-term evidence was identified in the reviewed evidence. This conclusion is specific to chronic primary pain. It should not be stretched into a claim that TENS is ineffective for every form of chronic secondary pain or for every condition-specific use.

When a discussion may still be reasonable

If your pain is secondary to a diagnosed condition, ask what evidence exists for that condition and what outcome you should track. Clarify electrode placement, session length, skin care and when to stop. A TENS unit should not delay investigation of new neurological symptoms, progressive weakness, infection, a wound or another condition requiring medical attention.

Because TENS is external, it is generally the least procedurally intensive option in this group. That does not make it automatically appropriate: the diagnosis, contraindications, skin tolerance and your ability to operate the controls still matter.

Peripheral nerve and nerve-field stimulation

How PNS differs from TENS

Peripheral nerve stimulation places an electrode under the skin near a peripheral nerve. The target is selected to match the nerve territory producing the pain. The procedure and device follow-up make it a specialist treatment, even though the target is outside the spinal canal.

What the PNFS guidance actually covers

NICE health technology guidance HTG309 addresses peripheral nerve-field stimulation for severe chronic low-back pain that has not responded to conservative interventions. It describes the procedure as usually staged and less invasive than SCS or spinal fusion. The guidance dates from 2013 and is specific to that procedure and the evidence available at the time; it does not make PNFS routine for all back pain.

Questions that determine suitability

  • Can the clinician identify a consistent painful nerve or field that the lead can cover?
  • Have appropriate non-procedural treatments been tried, and what happened?
  • Will a temporary stage or trial be used before a permanent generator is placed?
  • What movement, sleep or activity outcome will define success?
  • What happens if the lead moves, the skin does not heal or the stimulation stops helping?

Ask the service to explain the expected procedure, restrictions during healing, charging or battery arrangements and the plan for removal or revision. These details vary by system and local practice, so they should come from the treating team and the current device information.

Spinal cord stimulation: a specialist, trial-based choice

What is implanted

SCS uses one or more leads positioned near the spinal cord. The leads connect to a generator implanted under the skin. Programming determines how pulses are delivered, and the patient normally receives a controller. The system is considered for selected difficult-to-manage pain, not for every person with back or limb pain.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Assessment and the trial

NICE technology appraisal TA159 says SCS should be offered only after assessment by a multidisciplinary team skilled in chronic pain. In that 2008 appraisal, trial success was defined as tolerating the device and its sensation, covering at least 80% of the painful areas and obtaining at least 50% pain relief in the covered area. Those figures belong to that appraisal’s trial definition; they are not a universal current threshold for every indication or device. A contemporary team may use additional measures of function, sleep, medicine use and quality of life.

A trial is intended to answer a practical question: does stimulation produce a meaningful benefit for this person before a permanent generator is implanted? Agree in advance how the trial will be judged, who will review the result and what happens if benefit is partial or absent.

Why the device label matters

Indications differ by system and regulator. For example, an FDA SCS supplement approved on May 30, 2024 lists uses including intractable pain associated with failed back surgery syndrome, certain low-back pain when surgery has not been performed and the patient is not a surgical candidate, and diabetic peripheral neuropathy in the legs and feet. The same FDA information says the system should not be used when a patient cannot operate it or did not obtain effective relief during trial stimulation.

Another FDA supplement, approved January 18, 2022, describes an implanted rechargeable system for selected chronic intractable trunk or limb pain. Its listed indications include failed back surgery syndrome, intractable low-back and leg pain, diabetic neuropathy and a specified nonsurgical low-back-pain population. It also cautions against use when the patient cannot operate the system, did not receive effective trial relief or is a poor surgical candidate. These are device- and label-specific descriptions, not evidence that one system is superior to another.

Expectations are part of eligibility

A multisociety consensus guideline frames selection around expected efficacy, tolerability of induced sensations and the patient’s understanding of long-term therapy. Ask what improvement is realistic, what maintenance the system requires and which symptoms it is unlikely to change. A good trial cannot compensate for an indication that does not match the device or for expectations that the treatment will cure every aspect of chronic pain.

Compare options using the same decision questions

Decision axis What to ask Why it changes the choice
Pain condition and pattern Is this chronic primary or secondary pain? Is it neuropathic, mixed or another type? Is the target focal or widespread? Evidence and device indications are diagnosis- and distribution-specific.
Evidence and indication What does the current local guideline say, and does the exact device label include this diagnosis? A procedure supported for one condition may be unproven or unsuitable for another.
Invasiveness and reversibility Is the treatment external, temporarily implanted or permanently implanted? What procedures and follow-up are involved? Procedure burden, recovery and future revision differ substantially.
Trial response Is a trial recommended or required? What counts as meaningful benefit for me? A defined test helps prevent permanent implantation without a clear personal benefit.
Risks and practical demands Can I undergo the procedure, operate the controller and manage charging, batteries and appointments? Surgical candidacy and day-to-day use affect safety and durability.
Goals and preferences Which matters most: function, sleep, activity, medicine reduction, tolerability, access or avoiding surgery? Shared decisions should reflect the outcome you value, not pain intensity alone.

How a specialist evaluation usually proceeds

  1. Document the pain. Record onset, duration, location, triggers, sensory symptoms, previous operations, medicines and functional limits. Bring imaging and treatment records if available.
  2. Confirm the diagnosis and target. The team should explain whether the pain is primary or secondary, whether a nerve or spinal pathway is a plausible target and whether another cause needs treatment first.
  3. Review the wider plan. Discuss physical rehabilitation, psychological support where appropriate, sleep, medicines and management of the underlying disease. Stimulation is one component, not a replacement for this plan.
  4. Check the indication. Ask for the current guideline or regulatory indication for the proposed system in your jurisdiction. Confirm whether your diagnosis, pain distribution and prior treatments match it.
  5. Assess procedure and device suitability. Review surgical risks, infection risk, healing, other illnesses, ability to use the controller, charging or battery needs and access to follow-up.
  6. Set measurable goals. Choose one or more functional outcomes and a time frame. Decide how pain, sleep, activity and medication use will be recorded.
  7. Complete a trial when required. Follow the service’s restrictions and keep the agreed diary. Report benefit, unpleasant sensations, loss of coverage or new symptoms promptly.
  8. Make the permanent-implant decision. Proceed only if the trial produced meaningful benefit, the expected long-term workload is acceptable and the team still considers the indication and surgical risk appropriate.
  9. Plan long-term follow-up. Establish who reprograms the device, how urgent problems are handled, what happens when the battery reaches end of life and how loss of benefit will be evaluated.

Risks and practical issues to clarify

The sources summarized here do not provide a complete head-to-head comparison of adverse events, costs or quality-of-life outcomes. Do not infer that one category is globally safer or more effective. Instead, ask the treating service for the risks of the exact procedure and system being proposed.

  • Procedure-related problems: Ask about infection, bleeding, anaesthetic risk, wound healing, lead movement, hardware failure and the possibility of revision or removal.
  • Stimulation effects: Clarify what sensations may occur, whether they are acceptable, how coverage can change with posture and what happens if stimulation becomes uncomfortable.
  • Device management: Find out how to use the controller, how often charging is needed, what battery replacement involves and whether travel or other equipment requires special precautions.
  • Ability to operate the system: FDA device information specifically cautions against use when a patient cannot operate the system. Arrange practical help only if the clinical team confirms that the device can still be used safely.
  • Trial failure: A poor or intolerable trial is useful information. Ask whether the lead will be removed, whether another target is reasonable and which non-stimulation options remain.
  • Changing pain: Chronic pain can evolve. Agree how a new pain pattern, neurological deficit or recurrence of the underlying disease will be assessed rather than assuming that reprogramming is always the answer.

Fit stimulation into the broader chronic-pain plan

NICE recommends person-centred assessment and shared decision making for chronic primary pain, chronic secondary pain or both. That means knowing you as an individual, enabling active participation and discussing benefits, risks and uncertainty. A stimulation decision should therefore sit alongside treatment of the underlying condition, movement or rehabilitation goals, sleep, emotional wellbeing and medication review.

Ask how success will be reviewed after the trial or implantation and what would trigger a change in the plan. A device may help one component of pain while leaving fatigue, muscle weakness, inflammation or sleep disturbance unchanged. Naming those limits early prevents an otherwise useful treatment from being judged against an impossible promise.

A checklist for your appointment

  • What is the confirmed pain diagnosis, and is it primary, secondary or both?
  • Which nerve, field or spinal pathway is being targeted?
  • What evidence supports this option for my exact condition and pain distribution?
  • Is the proposed use included in the current device label and local guidance?
  • Will I have a temporary trial? What result will count as meaningful?
  • Which functional goals will we measure besides pain intensity?
  • What are the procedure-specific risks, recovery limits and possible future surgeries?
  • Can I operate, charge and maintain the system, and who provides technical support?
  • What alternatives remain if the trial fails or benefit fades?
  • How will stimulation fit with rehabilitation, medicines and treatment of the underlying cause?

Bottom line

Choose a nerve-stimulation treatment by matching the intervention to the diagnosis and distribution of pain, then weighing evidence, invasiveness, trial response, surgical suitability, practical demands and personal goals. TENS is a removable surface treatment and is not recommended by NICE for chronic primary pain in adults. Peripheral nerve and nerve-field stimulation are implanted specialist procedures used for selected patterns, including guidance specific to severe chronic low-back pain. SCS is a multidisciplinary, trial-based option whose indications depend on the exact system and regulator label. A careful assessment and shared decision are more reliable than choosing a device by name or assuming that a consumer unit and an implanted stimulator are interchangeable.
