Choose a gut microbiome test only if you know what question you want it to answer—and whether the result can reliably guide that decision. A test may measure microbial material without diagnosing a condition or showing that a recommended diet or treatment will help. Compare its method, sample handling, quality controls, and clinical evidence; for symptoms or a medical decision, ask a healthcare professional whether a targeted stool test is more appropriate.
Are gut microbiome tests accurate?
“Accurate” can mean three different things. A test can perform well at measuring what it claims to measure and still fail to predict a health condition or improve care. The U.S. Food and Drug Administration (FDA) treats analytical validity and clinical validity as separate considerations in its review framework for applicable direct-to-consumer (DTC) tests. For microbiome tests, the international expert consensus describes clinical applicability as limited.
- Analytical validity: Does the assay reliably detect or quantify the microbial material it claims to measure?
- Clinical validity: Does the measured result reliably indicate the health state the provider says it predicts, in the population and context claimed?
- Clinical utility: Does acting on the result lead to a better healthcare decision or outcome?
These are separate questions, not one marketing score. A detailed organism list or a precise-looking percentage cannot by itself establish clinical validity or utility.
What commercial testing variability shows
In 2024, the National Institute of Standards and Technology (NIST) reported an evaluation of seven DTC gut microbiome services using standardized human fecal material. The researchers found major discrepancies within and between services; differences between providers were on the same scale as biological variability between donors. NIST’s summary attributes the inconsistencies to method variability and insufficient quality control. The result is evidence of variability in that evaluation—not a universal error rate, a ranking of every current provider, or proof that every result from every service is wrong. The authors of the NIST publication summary, Stephanie Servetas, Scott Jackson, Diane Hoffmann, and Jacques Ravel, wrote: “Our results reveal major discrepancies, both within and across the different service providers.”

Which test method should you choose?
For broad gut-community profiling, the international consensus recognizes amplicon sequencing and whole-genome sequencing, including shotgun metagenomic approaches. The method tells you something about how the sample is analyzed; it does not, on its own, validate a disease claim or a food or supplement recommendation.

A targeted stool test may still be useful for a particular suspected problem; it answers a different question from broad community profiling. A clinician can help determine which kind of test, if any, fits your concern.

How to compare tests before buying
Look for evidence and practical detail—not just a long report, a wellness score, or the number of organisms listed. The international consensus calls for transparent claims, quality standards, and appropriate expertise across collection, sequencing, computation, and reporting. NIST’s evaluation makes it especially important to ask how a provider controls variability.
- Intended use: What specific question does the provider say the test answers, and what evidence supports that use?
- Assay and interpretation: Which method is used, what can it detect, and how are organisms identified? Is the reference database explained?
- Collection and preservation: Are there clear directions to avoid contamination and preserve the sample’s microbial genome?
- Shipping: Does the provider specify the required shipping time and conditions for its kit?
- Laboratory quality: Is the laboratory identified, and are its quality standards and assay controls explained?
- Comparisons and uncertainty: Does the report explain the population used for comparison, the limits of those comparisons, and what is not known?
- Clinical interpretation: Can a qualified healthcare professional explain whether any action follows from the result?
If these details are missing or unclear, a polished report does not fill the gap. The international consensus says providers should make customers clearly aware of the currently limited evidence for clinical use.

Can a stool microbiome test tell you what to eat?
A broad consumer profile should not be treated as a validated personalized diet plan. A sequencing result may describe measured microbial features, but that does not establish that a particular food recommendation will improve your health. The evidence summarized by the international consensus does not support assuming that a consumer microbiome report can prescribe an effective diet or treatment.
Do not start, stop, or materially change medication, supplements, or diet solely because a consumer report recommends it. The consensus advises that changes in patient management based on microbiome testing be made by the referring healthcare professional; FDA guidance likewise says a DTC result should not be the sole basis for medical decisions or treatment.

When to seek clinical testing instead
If you have persistent or concerning symptoms, seek clinical evaluation rather than using an exploratory microbiome profile as a diagnostic substitute. A clinician may recommend a stool test aimed at a defined suspected problem, which is not the same thing as a broad microbiome community profile. The international consensus also discourages patients from directly requesting broad microbiome testing without a clinical recommendation.
What FDA status does—and does not—mean in the United States
FDA says DTC tests have varying levels of evidence behind their claims, and that some measure bacterial flora, or the microbiome. In the United States, FDA review depends in part on a test’s intended purpose and risk: the agency generally does not review low-risk general-wellness tests before they are offered, while moderate- to high-risk tests with medical purposes are generally reviewed for claim validity.
