A public service, an unpatched vulnerability, or a leaked password is evidence of risk—not proof of a broker’s foothold. Check exposure by combining an authorized external asset review with vulnerability, credential, identity, endpoint, and network evidence. If you find suspicious access, treat it as a potential incident, not just a patching task.
What “exposed to initial access brokers” means
An initial access broker is a criminal intermediary that obtains or arranges unauthorized access to an organization’s systems and sells or transfers that access to other criminals. Internet-facing systems and compromised credentials can provide routes into an organization, but a reachable service does not show that anyone has exploited it. Nor does a credential appearing in an exposure source establish that it is still valid, has been used, or has been offered for sale.

It is more useful to treat the question as a defensive risk assessment: What can an outsider reach? Which of those systems are weakly secured or vulnerable? Are organizational credentials exposed or being used unexpectedly? Is there evidence of an unauthorized session or foothold? No one finding answers all four questions.
The relevance of these routes is reflected in the CISA, FBI, and HHS Medusa advisory update of August 18, 2026. It describes actors gaining initial access through brokers, phishing, and exploitation of newly disclosed, unpatched internet-facing vulnerabilities. The agencies reported more than 500 Medusa victims as of April 2026 across multiple critical-infrastructure sectors. That is a figure about Medusa victims, not a measure of IAB activity overall or the probability that a particular organization is exposed.

Use several kinds of evidence, not one “broker check”
Different checks reveal different parts of the risk. An external scan can identify reachable services; it cannot establish whether a criminal has obtained access. Internal authentication logs can show suspicious use; they will not necessarily reveal every forgotten public asset. An effective review joins those views and validates results with system owners.
| Approach | What it can show | What it cannot establish by itself |
|---|---|---|
| Authorized external asset discovery and scanning | Public IPs, domains, and services that respond from the internet, plus potential vulnerability or configuration issues depending on the scan. | That an IAB has accessed, retained, or advertised access to an asset. |
| Firewall, cloud, and asset configuration review | Whether public exposure is intentional, which business need it serves, and whether access controls match policy. | That all externally visible assets have been inventoried or that credentials have not been stolen. |
| Credential exposure monitoring | That organizational credentials may have appeared in an exposure source, subject to validation and freshness. | That a password remains valid or has been used by an attacker. |
| Identity, VPN, endpoint, and network telemetry | Evidence such as unfamiliar sign-ins, unusual remote access, new privileged accounts, or other activity that warrants investigation. | That no access exists simply because the logs reviewed contain no alert. |
| Threat intelligence and analytics | Context linking known threat reporting to an organization’s incidents, assets, or vulnerability exposure, where the service and licensing support it. | A definitive broker-listing check or a guarantee that the organization is resilient. |
When evaluating any checking method, ask what assets and operating systems it covers, when its data was last observed, how findings are tied to an asset you own, what evidence it supplies, and what remediation or escalation path is available. A result is only useful if you can establish that it belongs to your organization and determine who can act on it.
Step 1: Define scope and authorization
Before scanning, establish the boundaries of the review and confirm that the organization has authorized it. Include known public IP ranges, domains, cloud environments, remote access gateways, and third-party-hosted assets that are within scope. Identify relevant subsidiaries or business units if they share the organization’s security responsibility. Record the owner or contact for each known asset where possible.

- Gather the starting inventory. Use the organization’s existing asset records, domain records, cloud inventories, firewall configurations, and remote access service lists. Include systems operated by vendors only where the agreement and authorization allow the review.
- Set the scan boundaries. Specify which IP ranges, domains, and environments may be checked, which techniques are permitted, and who should be contacted if a scan triggers an alert or disrupts a service.
- Confirm ownership before changing anything. External discovery can return stale, forgotten, or misattributed assets. Reconcile discoveries with administrators or service owners before blocking, shutting down, or modifying them.
The goal is not merely to produce a long list of addresses. It is to create a defensible map of the organization’s internet-facing footprint and connect each reachable service to an accountable owner and business purpose.

Step 2: Find what is reachable from outside
Compare the starting inventory with an authorized external scan and with firewall and cloud configuration. The important question is whether a service answers from the public internet, not whether a port or system exists somewhere inside the network. Reconcile each discovery against approved business needs: an unfamiliar public service may be a forgotten asset, a vendor-managed system, or an expected service that was omitted from the inventory.
CISA recommends regular vulnerability scanning, with particular attention to internet-facing devices, and advises disabling unnecessary operating-system applications and network protocols on public-facing assets. Its #StopRansomware Guide also emphasizes that exposed remote services are a common route for initial access. CISA states: “Threat actors often gain initial access to a network through exposed and poorly secured remote services, and later traverse the network using the native Windows RDP client.”
If your organization uses Microsoft Defender for Endpoint
Microsoft documents an internet-facing device investigation in the Defender portal. The feature uses external scans and incoming connections to identify internet-facing devices, and the portal can show the public IP and port associated with a detection and provide advanced hunting queries. Microsoft’s device investigation documentation currently says this feature identifies only onboarded Windows devices. Treat it as a useful view of those devices, not a complete inventory of other operating systems, cloud services, or assets outside the product’s coverage.

For every confirmed public service, establish who owns it, why it must be reachable, what software or firmware version it runs, whether it is patched, and how access is controlled. Prioritize known exploited or high-impact vulnerabilities on systems reachable from the internet. If a service has no current business need, removing the exposure is often safer than leaving it available and relying on monitoring alone.

Remote access deserves particular scrutiny. CISA advises against exposing Remote Desktop Protocol (RDP) and other remote services directly to the web. Where remote access is necessary, limit its use, audit the systems involved, close unused RDP ports, enforce multifactor authentication (MFA), and log connection attempts. The relevant decision is not simply “Is RDP open?” but whether the access path is necessary, restricted to the right users and systems, protected by strong authentication, and monitored.
- Unneeded public service: Confirm with its owner, then remove or restrict its internet exposure through the organization’s change process.
- Needed service with a known vulnerability: Prioritize remediation based on internet reachability and impact; verify the patch or mitigation on the affected asset rather than assuming a deployment completed successfully.
- Needed remote access: Reduce who and what can connect, require MFA, close unused access paths, and retain authentication records for review.
- Unclear ownership or purpose: Keep the finding open and escalate it for validation. An asset should not be ignored simply because its owner is not immediately apparent.
The August 2026 Medusa advisory makes the practical importance of this work clear: its reported initial-access routes included brokered access and exploitation of newly disclosed, unpatched internet-facing vulnerabilities. The advisory’s stated mitigations include patching, network segmentation, and filtering untrusted sources from remote services. This connects those weaknesses to a current ransomware advisory; it does not mean every organization with a public service is being targeted.
Step 4: Check credentials and identity activity
A clean-looking perimeter does not rule out stolen credentials. Use approved organizational processes to check whether credentials may have been exposed, and review identity records for signs they have been used unexpectedly. CISA recommends phishing-resistant MFA, particularly for email, VPN, and critical-system accounts, and suggests considering credential-monitoring services in its ransomware guidance.
Look for signals that deserve validation rather than treating any single alert as proof of compromise:

- VPN or other remote-access sign-ins from unfamiliar locations or devices.
- Repeated authentication failures followed by a successful sign-in.
- Unexpected sign-ins to email, critical systems, or privileged accounts.
- New privileged accounts, unexplained privilege changes, or access that does not fit the account holder’s role.
- Authentication activity that conflicts with the organization’s expected user, device, or location patterns.
If an exposure source reports a credential, verify the account’s status using internal identity systems. Where warranted, reset or revoke the credential, invalidate active sessions, and inspect authentication records for use. Do not assume a leaked password is harmless because it might be old; equally, do not claim a breach solely because a credential appeared in an exposure source.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Step 5: Decide whether a finding is a security incident
Routine exposure findings and suspected unauthorized access require different responses. A reachable service or missing patch calls for risk-based remediation. Suspicious logins, unknown accounts, unexpected remote tools, or other indicators of unauthorized access call for the organization’s incident response process. Do not stop at patching the exposed system if there is evidence someone may already have used it.
- Preserve and escalate evidence. Notify the incident response or security team promptly and preserve relevant system, endpoint, network, and authentication logs according to organizational procedures.
- Identify affected systems and accounts. Determine which assets and identities are implicated, what access they permit, and whether related services or accounts show the same activity.
- Contain the access path as appropriate. CISA’s guide recommends containing affected access paths, which may include VPN or public-facing assets. Coordinate changes with incident responders and service owners so containment does not destroy needed evidence or create avoidable operational harm.
- Investigate beyond the first alert. Review relevant logs and hunt for persistence mechanisms or related activity. CISA advises threat hunting and extended analysis as part of incident investigation.
- Remediate and verify. Address the vulnerability, access control, or identity weakness that enabled the activity, then confirm the affected asset and accounts no longer expose the same path.
Organizations with an eligible Microsoft Defender XDR tenant may use threat analytics to relate threat reporting to organizational incidents, assets, vulnerability exposure, and recommended actions. Access depends on eligible Defender licensing and roles. Microsoft cautions that recommended actions do not guarantee complete resilience; its Threat analytics documentation describes an investigation aid, not a detector for whether an IAB has listed an organization.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteHow to interpret the results
Classify findings by what they actually establish. This prevents an exposure report from being mistaken for an incident declaration—or a quiet scan from being mistaken for proof that no broker has access.
Best Value
| Finding | What it supports | What to do next |
|---|---|---|
| A public service with no confirmed business need | An unnecessary or unexplained exposure may exist. | Validate ownership and purpose, then restrict or remove exposure through the approved process. |
| An internet-facing asset with a vulnerability or weak access controls | A potential initial-access weakness exists, with priority influenced by reachability and impact. | Confirm the affected version and exposure, prioritize patching or mitigation, and verify the fix. |
| A credential reported in an exposure source | The credential may have been exposed; its current validity and use are not established by that report alone. | Check the internal account, revoke or reset if warranted, and review sessions and sign-in records. |
| Anomalous sign-in or unexplained account or privilege change | Possible unauthorized access that merits investigation. | Escalate to incident response, preserve relevant evidence, and assess related systems and accounts. |
| No issue found in a particular scan or log review | No issue was observed within that check’s coverage and timing. | Confirm coverage and freshness; do not interpret a limited negative result as proof of no access. |
There is no general statistic in the cited sources that quantifies the share of organizations currently exposed to IABs. Ransomware victim counts, vulnerability prevalence, or broker listings are not interchangeable with that missing measure and should not be used to estimate an individual organization’s likelihood of exposure.
Keep the review useful over time
Internet-facing assets change as teams deploy systems, cloud services, and vendor connections. Repeat authorized discovery and vulnerability scanning regularly, reconcile new findings with owners, and revisit remote-access controls and identity activity. CISA’s ransomware guidance supports regular scanning, especially for internet-facing devices. The exact cadence should fit the organization’s change rate and security program; the available guidance does not establish one universal interval.
Record enough detail for each finding to be acted on: the asset and owner, public service or address, evidence and observation time, business purpose, vulnerability or control issue, severity rationale, planned action, and verification result. This creates a trail from discovery to remediation without confusing a risk indicator with confirmed criminal access.
