Test code issuance, verification, expiry, retries, throttling, and failure handling separately from whether an email reaches an inbox or an SMS reaches a handset. Use deterministic fakes for routine tests, contract checks for provider integration, and a small, controlled set of live-provider tests.
What a reliable OTP test needs to prove
A verification flow usually has two distinct outcomes: the provider or application accepts a request, and the recipient actually receives the message. A successful API response is not proof of inbox or handset delivery. Test these outcomes separately, using provider responses and delivery callbacks where available for integration behavior, and application state for the verification result.

For a managed verification API, the core lifecycle is to start a verification, retain or inspect the relevant state, submit the supplied code, and check the resulting status. Twilio documents separate start and check operations in its Verifications API. Do not assume that other providers use the same endpoints or status semantics.

Build the test suite in layers
Unit-test application behavior with a deterministic fake
Use a fake provider adapter so tests can produce predictable success and failure responses without sending real messages. This lets you test application logic repeatedly without relying on carrier or email delivery, provider quotas, or external service availability.
#1 Best Overall
- Standard OATH compliant TOTP token (time based)
- 6-digit OTP code with countdown time bar
- Zero footprint: no need for the end user to install any software
- Secure, sturdy, and long-life hardware design
- Easy to use - Portable key chain design. These tokens will only work with Symantec VIP Access. These tokens will not work for any other Multi-Factor Authentication services, besides Symantec VIP Access.
- Successful request and accepted code
- Invalid destination and malformed provider response
- Incorrect or expired code
- Provider timeout, server error, and throttling response
- Resend or repeat request behavior required by your product
Keep code generation or validation policy tests separate from delivery-adapter tests if your application owns those responsibilities. Never put real OTP values, API keys, or authentication secrets in logs, screenshots, or test output.
Contract-test the provider adapter
Check that the adapter sends the expected method, endpoint, authentication, channel, destination, and required fields, then parses responses correctly. Include malformed and boundary inputs. Normalize phone numbers before an SMS request: Twilio’s documented Verify request requires phone destinations in E.164 format. See its request contract for provider-specific details.
Test verification state transitions
Write explicit tests for the states and transitions your product promises. At minimum, cover these cases:

- FIDO2 & Passkey Ready: Business-ready and FIDO2 L1 certified. This key is supported by major management suites and is ideal for both individual and enterprise deployment. Works seamlessly with Gmail, Facebook, GitHub, Dropbox, Coinbase, and more.
- Universal Connectivity (USB-A ): Features a built-in USB-A connector—simply unfold the key and plug it into your compatible PC or laptop for seamless authentication on the go.
- Dedicated Manager App: Use the Thetis Manager App for the initial hardware PIN setup. Setting the PIN on the device first ensures a smooth registration process. Once the PIN is configured, you can begin registering the key across your favorite FIDO2-compatible online services.
- Ultra-Durable & Portable: Featuring a rotating metal cover, this key is water, crush, and tamper-resistant. It fits easily on a keychain and requires no batteries or network connectivity.
- Check FIDO2 compatibility before purchase - Known limitations: ID Austria is not supported (requires FIDO2 Level 2). Windows Hello login only works with Windows Enterprise editions that support Entra ID, and NFC is NOT supported.
- A valid code is accepted once.
- An incorrect code is rejected.
- An expired code is rejected.
- A reused code follows the behavior defined by your product and provider contract.
- A resend or repeated request follows the intended policy.
- A rate-limited attempt is rejected and surfaced appropriately.
- A timeout or provider error does not incorrectly mark the user as verified.
Code reuse, resend rules, expiry, and status meanings are not universal across providers. Assert your chosen provider’s documented contract rather than assuming all OTP APIs behave alike.
Use live integration tests selectively
Reserve real-provider calls for checks that a fake cannot establish, such as credentials, project configuration, actual API compatibility, or delivery callbacks. Use a dedicated test service or project and destinations controlled by your team. Apply explicit request limits and, where supported, complete, cancel, or allow attempts to expire so test runs do not consume capacity unnecessarily.
Make expiry and retry policy explicit
Expiry changes both user experience and test outcomes, so treat it as a configured policy rather than an implicit provider default. Twilio documents a default Verify token validity of 10 minutes. It says the validity period can be configured from 2 minutes to 24 hours by contacting Support. These values describe Twilio Verify, not a general rule for email or SMS APIs; see Twilio’s Rate Limits and Timeouts documentation.

In tests, control time where possible instead of waiting for a real code to expire. Assert the boundary behavior your application requires, including whether a resend invalidates an earlier code if that is part of the contract. For provider-managed codes, use the provider’s documented behavior and avoid asserting an unsupported assumption about reuse or invalidation.
Test throttling as a normal outcome
Exercise both below-limit and over-limit behavior in an isolated configuration. Verify the HTTP status and provider-specific error contract, and ensure your application does not mistake a rejected request for an issued verification.
Twilio Verify supports configurable service rate limits. Its documentation states that exceeding a configured limit returns HTTP 429 with error 60203 and does not create a verification or send a message. This is Twilio-specific behavior, not a universal OTP response; refer to Twilio’s Service Rate Limits documentation.

- OTP token that provides secure remote access with strong authentication
- Easy to use and easy to carry
- Expected battery life is approximately 7 years
Account for sandbox and quota restrictions
Twilio Verify credentials and test cycles
Twilio’s generic test credentials are not compatible with Verify, according to its Verify testing guidance. That guidance describes completing a verification, waiting for expiry, or canceling a verification as ways to manage test cycles within limits. Plan live tests around the Verify service rather than assuming generic API test credentials cover it.
AWS SNS SMS sandbox
If your application sends SMS directly through AWS SNS, check whether the account is in the SMS sandbox. AWS documents sandbox verification for phone numbers; sandbox restrictions mean messages are limited to verified destinations. See AWS’s VerifySMSSandboxPhoneNumber API reference.

Firebase publishes SMS verification limits, including project- and IP-based limits, on its Authentication Limits page. Check the active project and current plan when configuring integration tests. Treat published quotas as changeable operational limits, not constants to hard-code indefinitely.

A practical execution sequence
- Define the contract. Record the supported channels, destination format, required request fields, expiry, resend and reuse behavior, status meanings, and error handling for your selected provider and application.
- Run unit tests against a fake. Cover accepted and rejected requests, incorrect and expired codes, repeated requests, throttling, malformed responses, timeouts, and provider errors.
- Run adapter contract tests. Verify request construction, authentication handling, destination normalization, and response parsing without exposing credentials or codes.
- Run state-transition tests. Confirm the application changes verification state only in response to the expected success result, and follows its defined policy for retries and reuse.
- Run a limited live check. Use a dedicated project or service and controlled destinations; respect sandbox and quota rules, and avoid turning a routine test suite into repeated real sends.
- Review observability and cleanup. Ensure logs do not leak secrets or OTPs, inspect delivery-related signals separately from API acceptance, and complete or clean up attempts when the provider supports it.
What to compare when choosing an OTP provider
Compare documented behavior that will affect your test design and operations, rather than assuming a shared OTP standard:
- Whether email, SMS, or both channels are supported
- Availability and scope of test credentials, sandboxes, or dedicated test projects
- Whether test destinations must be verified
- Code validity, resend behavior, and code reuse semantics
- Configurable rate limits and the exact status/error behavior on a limit breach
- Whether delivery callbacks or other delivery signals can be tested
- The operational impact of sending real messages during integration tests
Provider documentation shows meaningful differences in channels, quotas, and sandbox rules. Review the active API contract before encoding assumptions into shared tests; the Twilio Verify API reference is one example of provider-specific documentation.
