A Bootstrap survey form is a web form styled with Bootstrap—not a hosted survey service. You can build the interface for free with Bootstrap’s form controls, then connect it separately to code or a service that processes and stores responses.
What you need for a free Bootstrap survey form
Bootstrap provides form controls, layout utilities, and validation styling guidance. Its current forms documentation is labeled v5.3.8 and covers text fields, textareas, selects, checkboxes and radios, range controls, input groups, floating labels, layout, and validation. Start with the Bootstrap v5.3 forms documentation for current guidance. Bootstrap’s separate examples page is labeled v5.2.3, so do not treat that page’s version label as the current release.

A free example gives you frontend markup and styling. The cited Bootstrap and MDB survey examples do not establish response storage, distribution, reporting, or analytics; those capabilities require separate implementation or a survey service.
Do these 3 things before closing this tab:

The input type determines what a respondent can answer. MDBootstrap’s survey example illustrates the difference with radio-button single choice and checkbox-based multiple choice.
#1 Best Overall
| Question behavior | Control | Implementation detail |
|---|---|---|
| Choose exactly one option | Radio buttons | Give the options the same name so they form one choice group. |
| Choose one or more options | Checkboxes | Use a separate checkbox for each selectable answer. |
| Enter a written response | Text input or textarea | Choose the control and input type that fit the information requested. |
| Provide an email address | Email input | Use an appropriate input type such as email. |
For more examples of radio and checkbox survey patterns, see MDBootstrap’s survey form examples.
Build the form with explicit labels and useful grouping
Give every control an accessible name. Bootstrap says visible labels are generally best for accessibility and usability; connect each label to its control with matching for and id values. A placeholder should not be the only instruction or label. Bootstrap also documents visually hidden labels, aria-labelledby, title, and aria-label as alternatives for cases where a visible label is not suitable. See its form accessibility guidance.

Group related questions with a clear heading or fieldset and legend, and use Bootstrap’s layout utilities to arrange fields. The framework can style and lay out the interface; it does not define your survey’s data model or decide how answers are processed.
Handle required fields and validation at the right layer
Bootstrap documents browser-default validation as well as styled client-side and server-side validation states. Native browser feedback can handle basic constraints, while custom client-side logic or server-side checks may be needed for more involved rules. Server-side validation is important when the submitted data must be checked by your application rather than trusted from the browser.

For server-side feedback, Bootstrap supports .is-invalid and .is-valid. Associate an error message with the relevant control using aria-describedby, so assistive technology can identify the feedback that belongs to that field. Follow the Bootstrap validation documentation for the available patterns.
What the free code does—and does not do
- It does: provide frontend form controls and Bootstrap styling and layout options.
- It does not, by itself: save responses, email results, distribute the survey, or provide analytics.
- To collect answers: connect the form submission to your own backend or a separate survey service, and decide how responses will be stored and handled.
Bootstrap’s official examples and form documentation are available as code resources. MDBootstrap also offers free UI kits alongside optional paid products; a paid product is not required to build a basic Bootstrap survey form.
