Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errorsTo audit an AI agent, keep a protected, searchable record that lets an independent reviewer reconstruct an operation from its trigger through its outcome. Connect each attempted action to the human or system that initiated it, the agent and authority involved, the policy and approval decision, relevant evidence, and what actually happened. A transcript or tool-call list alone usually cannot answer all of those questions.
What an agent audit trail needs to show
NIST’s glossary defines an audit trail as a chronological record of system activity sufficient to reconstruct and examine events surrounding or leading to a security-relevant transaction, from inception to results. For an AI agent, that means recording more than the final tool call: reviewers also need the context and authority that led to it.

A useful record should let someone determine what started the operation, who or what authorized it, which agent and version acted, what policy applied, what information informed the decision, whether an approval was required and obtained, what the agent attempted, and what changed. Keep proposals, approvals, denials, executions, failures, retries, and rollbacks distinguishable. A blocked attempt is part of the evidence, not an event to omit.

There is not yet one settled agent-audit standard. NIST’s agent identity and evaluation work describes ongoing projects and public input, not a universal required schema. Treat the design below as a practical synthesis, then adapt it to your system, risk, and obligations.

Build the record around the operation’s lifecycle
1. Define which events and actions matter
Inventory the agent’s tools, target systems, accessible data, and possible state changes. Set event coverage and detail according to the sensitivity of the data and potential impact of an action. At minimum, distinguish reads, proposals, executions, approvals, denials, failures, retries, and rollbacks. Record blocked attempts as well as successful actions so reviewers can see whether controls worked.
NIST SP 800-12 notes that application logs can capture data opened or closed and specific read, edit, or delete actions. Parameters can make an entry more useful than a bare command name; record normalized parameters and the target where doing so is safe and necessary.
2. Give events stable identity and context
Assign stable event and run identifiers, timestamps, and correlation IDs that can join records across services. Preserve the initiating human or system principal, agent or service identity, relevant parent-child delegation relationships, tool identity, target resource, and agent, model, runtime, and policy versions where available. This makes it possible to distinguish who initiated work, which component acted, and which version and authority applied.

3. Separate the proposal, authorization, and execution
For each proposed action, retain a reference to its trigger, the proposed operation, normalized arguments and target, risk classification, policy decision, authorization result, and any required approval. Then record the execution result separately, including denials, errors, retries, and rollback references. This distinction prevents a proposed action from being mistaken for an executed one.
For high-impact actions, do not let the agent both decide that an operation is safe and execute it without an independent check. OWASP’s AI Agent Security Cheat Sheet recommends separating proposal from execution and having a policy or execution component independently validate scope, privilege, and approval. Bind an approval to the specific actor, tool, target, parameters, time, and expiry; a general approval should not silently authorize a changed action.
A practical event-record design
The fields below are a working design, not a published mandatory format. Store references rather than unnecessary sensitive content where that still allows a reviewer to find and verify the underlying evidence.
Recommended Free Tools
| Record group | Useful fields | What it helps establish |
|---|---|---|
| Correlation and time | event_id, run_id, parent_event_id, timestamp, workflow correlation ID |
Which events belong to the same operation and their sequence. |
| Principal and agent | Initiating human or system principal; agent/service identity; delegation lineage; agent, model, and runtime version where available | Who initiated the work, which agent acted, and how authority passed through a workflow. |
| Action and target | Event type; tool or API; target resource; normalized parameters; requested scope | What was proposed or attempted, against which resource, and with what arguments. |
| Policy and approval | Risk classification; policy version and decision source; authorization result; approval identity, scope, time, and expiry | Which controls applied and whether the precise operation was permitted. |
| Evidence and outcome | References to relevant retrieved documents; execution result; affected object or state-change reference; independent verification where available | What information supported the decision and whether the attempted change occurred. |
| Record protection | Integrity metadata; storage location; retention class; access history | How the record is protected, retained, and accessed. |
Require an independent gate for high-impact actions
Use a separate policy or execution component to validate an agent’s proposed action before it reaches a consequential system. The component should check the action’s scope and privilege, confirm that any approval matches the exact operation, and write the relevant decision to the audit record. The approval should become invalid when its stated expiry passes or the actor, tool, target, or parameters change.

Fail closed for high-impact operations if the system cannot classify risk, retrieve the applicable policy, validate approval, or record the event. In practice, that means the action does not proceed while a required control or evidence path is unavailable. For lower-impact operations, choose an explicit fallback rather than allowing an accidental bypass.
Keep the control decision and tool execution distinguishable in the record. A policy approval does not prove that an operation ran, and a successful tool response does not prove that the operation was authorized.
Link decisions to evidence that can be checked
When an agent makes a factual claim or relies on retrieved material, preserve a reference that lets a reviewer locate the source again, along with useful version or retrieval context. Apply data-protection rules when retaining prompts, retrieved passages, or outputs; a protected reference or redacted copy may be safer than duplicating entire source documents.
NIST’s Building Evaluation Probes into Agentic AI project describes machine-readable trails that map decisions to supporting documents and probes for three qualities: faithfulness (whether a source supports a claim), completeness (whether the agent captures the source’s full message), and sufficiency (whether the source meets the evidentiary burden). The project describes probes that can run during a workflow or after it. This is an evaluation approach, not a requirement that every deployment adopt that specific pipeline.

As NIST’s project puts its goal, the aim is to move beyond “the AI said so” toward understanding what it found, where it found it, and how the evidence supports its conclusions. A stored document pointer is only useful if reviewers can retrieve the relevant version and see how it relates to the claim.
Protect audit records and limit sensitive collection
Do not give the agent unilateral ability to rewrite or delete the evidence used to audit its own actions. Restrict write and delete access, separate routine operational permissions from audit administration, and consider append-only or write-once storage, digital signatures, secure timestamps, and independent monitoring. NIST SP 800-12 discusses signatures as a way to alert reviewers to alteration and write-once media as an integrity measure; each requires suitable access controls and operating procedures.
Logs can expose identities, prompts, retrieved information, tool parameters, and confidential outputs. Limit who can view them, encrypt sensitive records where appropriate, and avoid collecting content that is not needed for reconstruction. Set retention and review periods based on sensitivity, incident-response needs, organizational policy, and applicable obligations. NIST SP 800-12 does not set a universal retention duration, and the appropriate period depends on the deployment and jurisdiction.
Make reconstruction and review part of operations
Provide search and export by agent, principal, run, tool, resource, time, policy, and outcome. Review high-risk events promptly where the risk warrants it, and periodically examine broader patterns. Logs have limited value if nobody reviews them; NIST SP 800-12 emphasizes timely review as part of effective log use.

Rehearse with a sample operation and ask a reviewer who was not involved in it to reconstruct:
- What request or event started the operation?
- Which human or system principal, agent identity, delegation path, and policy applied?
- What evidence informed the agent’s decision, and can the referenced material be retrieved?
- What action was proposed, and was it approved, denied, or changed?
- What actually executed, what failed, and what state changed?
- Can the reviewer tell whether the record was protected and who accessed it?
If those questions cannot be answered from records and linked evidence, identify which missing field or system boundary caused the gap and correct the instrumentation before relying on the trail for incident review.
Evaluate audit and observability tools against the controls
Whether you build instrumentation in-house or assess a platform, test it against the evidence and enforcement needs of your deployment. A product’s ability to display traces does not by itself prove that it captures authority, prevents unauthorized execution, or preserves trustworthy records.
- Lifecycle coverage: Does it record triggers, proposals, tool calls, approvals, denials, errors, retries, outcomes, and rollbacks?
- Context and attribution: Can records be joined to the human principal, agent identity, delegation, policy, version, and target resource?
- Evidence linkage: Can reviewers trace claims or decisions to source documents and retrieve relevant versions?
- Independent enforcement: Are permissions and approvals checked outside the agent’s own decision process?
- Integrity and access: Can the agent or an ordinary operator alter or delete records? Are access restrictions, integrity measures, and export available?
- Review and portability: Can incident-response and audit teams search, retain, and export records in a usable form?
- Privacy and operating cost: Can sensitive content be minimized and retention configured without losing the evidence needed for reconstruction?
NIST’s evaluation work is relevant to evidence linkage and claim testing; OWASP’s guidance is relevant to action controls and approval checks. Neither establishes a vendor ranking, benchmark, or product endorsement.
What current guidance does—and does not—establish
NIST’s Building Evaluation Probes into Agentic AI page, created May 1, 2026, and updated May 5, 2026, described an ongoing project, early research, and an open-source testbed—not a completed standard. NIST’s National Cybersecurity Center of Excellence announced a concept paper on agent identity and authority on February 5, 2026, seeking public input on identification, authorization, auditing, non-repudiation, and related controls. Its April 2, 2026 comment deadline has passed; the announcement described a potential project, not a finalized requirement. NIST’s summary of public comments documents respondents’ proposals and concerns, not adopted rules.
Use those efforts as context for emerging questions, not as a substitute for defining and testing controls in your own environment. Legal or regulatory retention requirements cannot be determined without knowing the deployment, data, and applicable jurisdiction.
